    Compassionate professional care restores independence

I'm a long-term resident and very pleased with the compassionate, professional care here. Nursing and therapy teams are knowledgeable and warm-therapy improved my mobility and helped me regain independence. The facility is clean and comfortable, staff treat residents like family, communication with family is proactive, and activities (bingo, cards, church, social hour) keep us engaged. Overall, a welcoming, well-coordinated place that gives me and my family real peace of mind.

    Summary of reviews

The reviews for River’s Bend Health Services describe a facility with clear strengths in hands-on caregiving, therapy services, and resident engagement, alongside recurring operational weaknesses. Many family members and residents conveyed appreciation for staff who are compassionate, attentive, and familial in their approach; nursing leadership and therapy teams are frequently cited as knowledgeable and effective, producing measurable mobility and rehabilitation improvements for residents. Several reviewers also praised organized discharge planning, accommodating dietary adjustments, thorough housekeeping and laundry, and a robust activities program that includes games, church services, and social hours.

At the same time, a consistent pattern of operational concerns emerges. Staffing levels and scheduling consistency are highlighted as persistent issues that contribute to delayed responses to call lights, slower assistance with toileting and transfers, and uneven availability of therapy and activities. Reviewers noted gaps in medication-administration controls and documentation, and a few accounts described coordination problems with external pharmacies and physicians. These clinical and coordination weaknesses were frequently tied to broader communication breakdowns across shifts and between the facility and families.

Dining and environmental maintenance appear variable. Some families reported accommodating meal planning and responsive dietary service, while others described frequent meal delays, cold or bland food, and missed service. Cleanliness assessments are similarly mixed: many reviewers describe a clean, comfortable interior and effective housekeeping, whereas others raised sanitation concerns, cluttered hallways, and odor issues near entrances. Infection-control practices also drew attention, with at least one outbreak in a rehab area noted alongside commendations for compassionate care during COVID-restricted periods.

Safety and leadership oversight are additional areas of concern. Several accounts point to fall-prevention and transfer-safety gaps, including inconsistent use of assistive devices and insufficient monitoring after incidents. Reviewers also described times when management presence and incident oversight were perceived as inadequate, which exacerbated worries about response times and care coordination. Staff conduct and tone were generally described in two contrasting ways across reviews: many families praised respectful, resident-centered interactions, while others reported brusque or unhelpful conduct from some employees.

In summary, River’s Bend presents a mixed picture: it offers strong therapeutic services, engaged activities, and numerous examples of compassionate caregiving and good discharge practices; however, persistent issues with staffing consistency, medication and clinical coordination, communication across shifts, and variable dining and environmental maintenance warrant attention. Prospective residents and families should weigh the documented strengths in therapy and family communication against the operational risks identified, and consider discussing staffing patterns, medication protocols, fall-prevention measures, and leadership oversight during a tour or intake conversation.

    About River’s Bend Health Services

River's Bend Health Services sits near the Manitowoc River, just north of Fleet Farm on Rapids Road, and it's easy to get to from Highway 42, Highway 151, and I-43, which comes in handy for families and visitors no matter where they're coming from, and you'll see it's right by two of the main hospitals in the area, making quick medical care possible when needed. The place has been around since 1997, so it's got a real history in Manitowoc, and the facility holds up to 100 people with private and semi-private rooms, each of those with their own cable TV and telephone so folks can reach out to loved ones or catch their favorite shows whenever they want, plus there's washers and dryers, maintenance, and good housekeeping to keep things tidy and comfortable. You'll notice right away safety is a focus, with plenty of handicap-accessible features, a full sprinkler system, and emergency gear like automatic defibrillators and life vests for peace of mind, as well as equipment and trained staff ready for infection control, isolations, and things like tracheostomy management or tube feeding if needed, so all kinds of health needs get taken care of.

River's Bend is both a nursing home and an assisted living center, offering skilled nursing care 24 hours a day, with a Medical Director, Dr. Pradeep Giriyappa, who oversees everything along with a strong nursing team and personal care assistants, and they help with walking, wheelchair transfers, bathing, toileting, dressing, grooming, and make sure medications are given right and on time, plus staff help with daily weights, glucose checks, nebulizer treatments, and incentive spirometry as ordered by the doctor. They also have podiatry, pharmacy, wound care with rounding doctors, wound vacs, and special tests like bladder ultrasounds, venous dopplers, and swallow studies right on site, and there's in-house lab and X-ray services, along with EKG and psychiatric care when necessary, so your health is always looked after without having to leave. Folks with memory needs get extra support, and there's a solid brain injury rehab program, so it's got specialty services for different conditions.

Therapy options really stand out here-you'll find a big 3,000-square-foot rehab gym, and physical therapy, occupational therapy, and speech therapy are each available seven days a week, including pre-registration for those planning their therapy after surgery, and all the therapy services focus on building up strength and independence with a plan that fits each person. People staying for short-term rehab, as well as those needing long-term skilled care, can count on attention to their personal needs, and both insurance and Medicare/Medicaid are accepted, along with VA benefits, making things easier for families.

Meals are homestyle and nutritious, cooked by chefs and meal planners right there in the facility's kitchen, with restaurant-style dining available in the dining room or snacks on a cart when you want something quick, and all meal plans get checked by registered dieticians with liberalized diets for those who need them, so you always get something good to eat. The property has a game and activities room, spaces for social events, resident lounges, a salon/barbershop for haircuts, wifi for internet access, and cable TV, encouraging folks to stay connected and relaxed. Activities like arts and crafts, social outings, wellness programs, and educational events keep everyone busy and engaged, and the team here is known for friendly attitudes, with folks often saying staff help everyone feel heard and cared for in a supportive, people-first setting.

Other services include guest parking, transportation support, laundry and dry cleaning, personal care, and both housekeeping and maintenance, so day-to-day living feels easy and safe. There's even pre-registration for rehab after planned surgeries. River's Bend is recognized for its high standards and accreditations, with a reputation for kindness and skilled care, all while being owned by Extendicare Health Services, which has facilities across the United States and Canada. The wooded surroundings make it a peaceful spot, and overall, River's Bend Health Services offers a mixture of skilled nursing, rehabilitation, long-term care, assisted living, and respite services, all in one place, aiming to help everyone reach their highest level of comfort and independence in a warm environment.
