    Warm attentive staff clean facility

I'm very pleased with this clean, welcoming facility - the staff from administration to nurses, CNAs, activities and therapy are warm, attentive and truly take personal interest in residents. My mom loves the compassionate one-on-one care, good food, dog-friendly visits and lively activities; I feel she's in very good hands and would recommend it.

    Summary of reviews

Reviews for Menomonee Falls Health Services are strongly polarized. Many families and residents praise the clinical and rehabilitative teams, noting compassionate nursing and aide care, effective therapy programs, and a social work team that assists with benefits and transitions. The facility receives repeated positive mention for its activities programming — including themed events, personalized engagement, and therapy-animal visits — and for generally clean common areas and a patient-centered atmosphere in many units. Several reviewers also highlighted helpful administrative staff and recent leadership initiatives that appear to have led to measurable improvements in care for some residents.

At the same time, recurring operational concerns appear across reviews. Staffing is described as inconsistent, with reliance on temporary or agency personnel and periodic shortages; that staffing variability is associated with delayed responses to call bells, interruptions in routine care, and inconsistent supervision. There are noted weaknesses in medication-management processes and incontinence-care/personal-hygiene practices that families characterize as lapses in routine care. Several accounts raise concerns about fall-prevention and supervision practices during high-risk periods, suggesting that safety-monitoring protocols and staffing alignment to resident acuity may be uneven.

Dining quality and food-service execution are inconsistent. The facility is sometimes praised for the kitchen staff and variety, yet other reviews describe inadequate meal presentation or limited menu fulfillment relative to marketing. Activities are consistently described as a strength — engaging, customizable, and well staffed — which can mitigate some quality-of-life concerns for residents.

The physical environment is described as clean but dated. Reviewers note an older building with areas that feel austere, some linens and housekeeping inconsistencies, limited private-bathroom options, and rooms that lack individualized climate controls or adequate privacy measures for semi-private arrangements. Positive physical attributes cited include bright rooms with many windows, convenient parking, and generally tidy common spaces.

Management and communication present a mixed picture. Multiple reviewers credit new leadership with positive changes, improved responsiveness, and better clinical oversight; others report problematic discharge coordination, communication breakdowns with families, and broader concerns about administrative consistency. A small number of reviews allege serious operational failures and raise regulatory concerns; those comments suggest prospective families should investigate incident-handling and oversight processes directly.

For prospective residents and families: consider an in-person visit that includes inspection of the actual room(s) under consideration, direct questions about staffing ratios and use of agency staff, a review of medication administration and fall-prevention protocols, clarification of toileting/bathing schedules and linen policies, and discussion of dining options and therapeutic programming. Ask to speak with the current administrator, therapy director, and social worker to evaluate recent leadership changes and continuity of care before and after admission.

    About Menomonee Falls Health Services

Menomonee Falls Health Services sits in Menomonee Falls, WI, just a short drive from Milwaukee, and it's been around for over 20 years, so folks in the area know it well as a place that tries to meet the community's elder care needs through both long-term and short-term care. The facility offers over 75 beds, with certified beds totaling 50, and there's independent living, assisted living, and nursing home options, including memory care for those who need it, and a focus on skilled nursing for residents who need round-the-clock medical attention. The place handles everything from daily activities like games, holiday and birthday celebrations, to full rehabilitation services such as physical, occupational, and speech therapies, and they add in infusion therapy too, so there's a lot of medical support on site. There are always nurses and CNAs on call, and you'll find licensed registered nurses and licensed practical nurses offering skilled care, though staff turnover does run high compared to the state average, with 65.5% nurse turnover and a reported nurse staffing ratio at 3.39 nurse hours per resident per day.

Menus focus on nutritious meals planned by chefs and dietary staff, and there's always an eye on dietary needs, as they've got special meal plans and nutrition support for folks with certain health requirements. Memory care programs, chapels for faith-based consulting, and even outpatient options like therapy, wound care, and IV care are all included, and the team works with outside acute care providers to monitor residents' changing needs, using medical innovation where needed for treatment of cancer, kidney, and digestive issues, or for wound, palliative, respite, and hospice care. There's a strong sense of partnership and teamwork which extends to activity services that focus on keeping people social and engaged - you'll see physical and recreational activities alongside cultural programs, plus housekeeping help, transportation, and plenty of parking for visitors and staff. The community is owned and managed by Extendicare and North Shore Healthcare, both known for experience in elder and home health care, and folks often mention the staff's professional approach and commitment to customer and employee satisfaction.

Medical offerings are broad, covering lab tests, X-rays, EKGs, glucose monitoring, IV antibiotics, total parenteral nutrition, psychiatric and dental in-house consultations, as well as podiatry and swallow studies, plus there's a helpline that's answered 24/7 throughout the year for any concerns or questions. The building has seen some wear, so it's not brand new, but maintenance crews keep the environment clean and safe, and infection control is a constant focus, though there've been past inspection deficiencies-including with infection control and pressure ulcer care-but the facility's working to improve these areas, and it currently holds accreditation and recognitions for quality care, with some evaluations finding it deficiency-free. Residents and families say the place feels homelike, and they appreciate that it's close to loved ones and nearby hospitals, with lots of specialized amenities, a safe and secure environment, and plenty of ways to stay active and connected. CNA training is available through programs like WisCaregiver Careers, and the values of trust, respect, and competence show in how the staff deal with people each day. The place covers a lot of ground, so folks looking for rehabilitation, assisted living, hospice, and even post-operative recovery and respite care will find specialized offerings, and the overall approach centers on respect for resident independence and personal care needs.
