    Caring team and peaceful surroundings

I placed my mother in this beautiful, brand-new Northwoods facility and I'm very pleased. The staff are warm, kind and attentive, the home-like setting and nature views are lovely, and there are plenty of activities (bingo, exercise, seasonal decorations), beautician services, and outside hospital healthcare available. My mother is eating well and happy - I'd recommend it for its caring team and peaceful surroundings.

    Summary of reviews

The reviews present a mixed picture of Country Terrace of Wisconsin. Positive comments emphasize a recently constructed, attractive building in a rural Northwoods setting, a home-like atmosphere, and a small residential scale that some families find comforting. Several reviewers praised warm, kind, and accommodating staff interactions, active programming (bingo, exercises, seasonal decorations), on-site grooming services, and coordination with external medical providers. Those elements suggest the facility can provide an engaging social environment and close personal attention for some residents.

At the same time, a number of concerning operational patterns emerge across reviews that prospective families should evaluate carefully. Several accounts indicate incontinence-care delays and inconsistent bathing assistance, and reviewers raised broader sanitation and cleanliness concerns in resident care areas. Staffing levels appear uneven: while some staff are described as attentive, others are characterized as overworked, and that strain is linked to gaps in timely assistance. Reviewers also noted limited onsite dining resources (including the absence of an on-site cook in at least one account) and inconsistent housekeeping and maintenance coverage, which can affect daily living comfort and hygiene standards.

Staff behavior and management practices are another recurring theme. There are comments about concerns with staff conduct and communication transparency, including instances described as deceptive behavior by personnel. More seriously, reviewers referenced regulatory and licensing actions involving leadership and staff — an indication that there have been compliance or oversight challenges. These are distinct from day-to-day care needs and suggest prospective residents and families should verify current licensure status and any corrective actions taken.

In summary, Country Terrace exhibits strengths in facility condition, location, and social programming, and some families report positive staff engagement and resident satisfaction. However, persistent operational weaknesses are also reported: incontinence-care and bathing assistance inconsistencies, sanitation and housekeeping gaps, limited meal-preparation resources, staffing strain, and documented regulatory issues. Prospective residents and their families would be well advised to tour the facility, review recent inspection and licensing records, clarify staffing patterns and meal/housekeeping provisions, and ask for specifics about how past compliance concerns have been addressed before making a placement decision.

    About Country Terrace of Wisconsin

Country Terrace of Wisconsin sits on a quiet residential street in Tomahawk, offering a homelike place for people aged 55 and over who might need assisted living, memory care, independent living, or even a short-term stay for respite care, and the whole building is one level, making it easy for residents to get around even if they have mobility concerns, and with features like wide hallways and accessible rooms, folks can move about without much trouble. The facility offers 33 beds in both private and semi-private suites, as well as shared and companion options, each with its own emergency call system, individually controlled air conditioning, and wall-to-wall carpeting, and there's plenty of storage with cabinet space in the rooms, which all feels quite comfortable and secure, and the staff are always around, providing 24/7 supervision, with an extra level of security in the memory care area for those living with Alzheimer's or dementia. A registered nurse conducts regular wellness reviews, and skilled nursing services are available around the clock to help with medication, personal care, bathing, dressing, transfers, wound care, and even health monitoring, and there's a part-time nurse on staff who helps keep an eye on everyone's health. The facility provides a secure and home-like place where people are encouraged to stay as independent as possible, and caregivers help residents do things for themselves whenever they can, including taking care of daily activities like bathing, laundry, housekeeping, and linen changes, and there's even in-house laundry and drycleaning if someone needs it. For folks who might want to bring a pet, the facility allows some pets-though you'll want to check with them about the current policy. Three hot, home-cooked meals come out of the kitchen every day, served either restaurant-style or in the cozy dining room, and the menus are dietitian approved, with options for special dietary needs. Residents can relax with friends or visitors in the warm and inviting living room, spend time outdoors in the garden, or sit on the outdoor patio if the weather's nice. Every day, the community has a full schedule with bingo, crafts, cooking, concerts, and board games, and there's pet therapy, exercise, and community outings to visit local parks or shops to keep things interesting and offer chances to socialize, so everyone can choose to do as much or as little as they like, whether they're interested in being part of planned group activities or just enjoying quieter time. People moving in can count on help through move-in coordination, and everyone can take advantage of transportation services to get to medical appointments, stores, or even just explore a bit. There's also a beautician onsite, daily housekeeping, regular maintenance, cable TV, and kitchenettes in some rooms for those who prefer a bit more independence. The community aims to be a welcoming, inclusive place where folks can grow, stay active, and live as fully as they're able, and it holds a state license, carries high marks for staff quality and facility tidiness, and is managed by WALA.
