    Caring professional staff and rehab

I'm very pleased with the care my family member received - staff were consistently caring, professional, and attentive (excellent CNAs, nurses, PT/OT) and management was accessible. The building is clean and well-maintained, dining and activities (pet therapy, themed programs) were engaging, and rehab helped restore mobility; overall a welcoming, thorough team I'd recommend.

    Summary of reviews

The review set for Terrace Health & Rehabilitation Center contains a pronounced mixture of positive experiences and serious operational concerns. Many families and residents praise individual staff members โ€” including CNAs, nurses, and therapists โ€” for compassionate bedside care, patience, and practical help with rehabilitation goals. The facilityโ€™s rehabilitation program (physical and occupational therapy) is frequently described as effective, with documented functional gains for some patients. The building and common spaces are often described as attractive and well maintained, and activity programming (bingo, painting, pet therapy, holiday music) is recognized as a meaningful part of resident life when active.

At the same time, a consistent theme across the reviews is variability in day-to-day operations. Staffing appears uneven: while some staff are singled out as excellent, reviewers also describe frequent turnover, surly or unresponsive shifts, and periods when help is delayed. Those staffing gaps are linked to operational consequences such as late or omitted medications, slow response to call lights, delayed assistance with personal care tasks, and occasional failures in discharge or transport coordination. Several reviews describe medication-timing issues and errors, which indicate weaknesses in medication-management processes.

Sanitation and safety concerns appear in multiple accounts. Cleanliness and housekeeping are praised in some reports but criticized in others, with specific mentions of room and bathroom cleanliness issues, odor concerns, and pest-control worries. Clinical-safety patterns emerge as an area for scrutiny: reviewers describe falls with subsequent hospitalization, skin and wound issues, and infection-related problems in a way that suggests gaps in monitoring, wound prevention, and infection-control consistency. PPE adherence and infection-control practice consistency are also cited as variable.

Dining and activities are similarly mixed. Activity offerings can be robust and well-run when staffed and scheduled, but programming has been suspended at times for infection-control reasons and when staff resources are constrained. Dining is described as adequate to good in some cases, with special diets accommodated; however, recurring complaints about cold or bland meals and minimal portions point to inconsistent food-service execution. Facility rooms range from newer, attractive accommodations to older rooms needing refurbishment; double rooms and private options are available.

Management and communication are recurring friction points. Reviewers describe miscoordination of external appointments and transport, missing or delayed personal belongings, and difficulty reaching social-work or administrative staff at key moments. While some visitors commend accessible and helpful managers, others experienced poor follow-through and a lack of accountability in resolving problems.

Overall, reviews present a polarized picture: families who interact primarily with skilled therapists and attentive caregivers often report good outcomes, whereas those who encounter staffing shortages, medication or sanitation lapses, or communication breakdowns report significant dissatisfaction. Prospective residents and their families should consider on-site interviews that focus on staffing levels for specific shifts, medication-administration protocols, infection-control and housekeeping schedules, fall-prevention and skin-care practices, meal-service timing and menu samples, and recent incident or quality-improvement records. Asking for direct references from current families and observing a mealtime and an activity session can help clarify whether the facilityโ€™s strengths are consistently in place for a particular residentโ€™s needs.

About Terrace Health & Rehabilitation Center

Terrace Health & Rehabilitation Center sits at 7207 SW 24th Ave in Gainesville, Florida, and provides both short-term and long-term rehabilitation services for people who need specialized care, which can mean regular nursing care, different therapies, or dialysis right there on site because they have the area's only in-house dialysis suite, and that does make things a bit easier for folks who need that sort of treatment. The facility has 138 certified beds and usually about 127 residents each day, and you'll see a mix of private and semi-private rooms with touches like a bed, a chair, windows for light, and space for personal items, and some rooms even have private bathrooms and furnishings picked for comfort. You'd notice the inviting covered entrance surrounded by landscaping and palm trees, a gazebo out in the courtyard, and inside, there's soft yellow paint on the walls, floral furniture, fresh flowers at the front desk, plus a lounge with couches and big windows that look out onto the mobility garden, so there's plenty of light and room to walk.

The staff at Terrace Health & Rehabilitation Center includes nurses around the clock, though they do have a nurse turnover rate of 75%, and they report about 3.79 nurse hours for every resident each day. They also handle medication, occupational therapy, physical therapy, speech therapy, and case management, with a concierge program for extra help. Folks here can use a 3,080 square foot rehab complex with a modern gym and an ADL suite, and there's a special garden set up for getting around safely, which helps with recovery and confidence. You'll find activities most days, like morning stretches, chair yoga, board games, movies, trivia, and arts and crafts, and people can join social outings, use the beauty and barber shop, or just sit out in the patio area to relax.

Dining at the center means you get meals served in a space meant to feel a bit fancier, and housekeeping and laundry take care of the basics, with staff also able to help with transportation and errands. Residents have access to pharmacy support, furnished suites, emergency call systems, and different programs like respite care and specialty care.

Ownership for Terrace Health & Rehabilitation Center belongs to Gold Fl Trust II, set up as a for-profit limited liability company, and the facility gets inspected like others in the area. It's picked up 17 total deficiencies in past reports, including a recent standard inspection from October 2024 with four deficiencies, one of them about infection control. There are also two separate infection-related deficiencies, as well as issues with documenting medical records, keeping resident information safe, following care orders, and setting clear plans for every resident's needs-these are areas for improvement. Even with these things to work on, plenty of people value the comfort and convenience at this center, where the main focus stays on supporting health, rehabilitation, and daily living for those who need it.
