    Very positive visit, attentive staff

I had a very positive visit - the director was kind and the care team genuinely attentive. Rooms are nicely kept and the amenities (gym, reading area, balcony, lounges) are pleasant and secure; I appreciated the staff and would recommend it with a few minor reservations.

    Summary of reviews

Reviews present a mixed picture: reviewers consistently identify attractive physical amenities and an engaged care team alongside recurring operational shortcomings. The facility's built environment—well-appointed resident rooms, a gym, reading/library space, balcony access, multiple lounges, controlled-access floors, and an overall clinical/hospital-like layout—is frequently described positively. Several comments also note approachable administration and instances of caring staff behavior.

Care quality and staff performance show variation. While reviewers praise individual staff members and teams for compassion and attentiveness at times, there are repeated descriptions of slow response times for basic assistance and delays in attending to resident needs. Incontinence-care delays and constrained shower schedules are highlighted as operational weaknesses. There are also concerns about inconsistent wound- and bathing-care protocols that suggest variability in clinical practice and oversight.

Dining and nutrition emerge as another area of inconsistent performance. Reviewers describe uneven meal quality and gaps in meal-service consistency, including missing components and unappetizing preparation on occasion. These comments point to weaknesses in kitchen oversight, portioning, or tray delivery rather than a uniformly poor food program.

Facility maintenance and sanitation practices are a notable pattern. Although rooms are described as kept up, reviewers report sanitation and waste-management issues in some common areas, accumulated or uncleared meal trays in corridors, and ad hoc equipment fixes that indicate maintenance-process gaps. These items collectively suggest uneven housekeeping and facilities management rather than a uniformly maintained environment.

Management and system-level observations are mixed. The director and some staff receive positive mention for accessibility and caring interactions, yet operational inconsistencies—staffing responsiveness, housekeeping, meal-service reliability, and specific clinical protocols—point to variability in day-to-day execution. There is also an expressed need for more robust psychiatric or higher-level specialty services for residents with complex behavioral or mental-health needs.

For prospective residents and families: an in-person visit is advisable, with targeted questions about staffing levels and shift coverage, protocols for bathing and wound care, incontinence-care processes and response times, meal-service schedules and substitutions, housekeeping and waste-management routines, and routine maintenance practices. Verifying availability of psychiatric or specialty support and asking for documentation of clinical protocols and recent inspection findings will help clarify whether the facility’s strengths align with the prospective resident’s needs.

About Astoria Place Living & Rehab Center

Astoria Place Living & Rehab Center, connected with Legacy Healthcare, offers senior living and skilled nursing care with 164 certified beds, and there's usually about 155 residents each day. The facility provides short-term rehabilitation, orthopedic rehabilitation, wound care managed by certified specialists, pain management, and a secured memory care unit for residents dealing with Alzheimer's and Dementia. There's around-the-clock nursing care with staff including experienced nurses, certified nurse assistants, social workers, activities staff, and rehabilitation specialists, though nurse staffing levels average 3.07 nurse hours per resident per day, which is below the state average. Nurse turnover at 44.9% is just under the typical state rate.

Residents can use amenities like a beauty salon, a barbershop, laundry, a grand library with a fireplace, and family dining rooms. Suites and accommodations come with comfortable furniture, flatscreen TVs with cable, and personal storage space. There's also access to hospitality rooms for quiet time or social visits, a movie area, and enrichment activities such as dominoes, bingo, puzzles, card games, and bowling. Special programs include a Russian & Cantonese Program and regular activities like Shabbat candle lighting, Sunday programs, and monthly birthday parties. The center offers dietary services, housekeeping, and maintenance as well.

Astoria Place keeps a HIPAA policy for privacy and supports both in-person and virtual tours, which folks can schedule if they're interested. The building is described as inviting, with earth-tone colors and plenty of natural light in the hospitality rooms. Clinical services are on-site with therapy equipment ready, personalized rehab, strength-building, neuromuscular therapy, and speech or occupational therapy offered to meet individual needs. The memory care unit has safety features and programming specifically for residents needing extra supervision.

While Astoria Place was named to America's Best Nursing Homes 2025 by Newsweek for the second year running, state inspection reports list eight deficiencies as of November 2024, including infection control issues and problems with food procurement and preparation, plus a quality of life and care deficiency for not keeping the environment free from accident hazards. There've been 27 documented deficiencies over time, two related to infection, and one from March 2025 tied to insufficient supervision to prevent hazards.

Staff and administration focus on maintaining dignity and respect, but the inspection records and staffing figures can give families important information to consider when looking at this facility.
