About Foster Health & Rehab Center

Foster Health & Rehab Center is a nursing home with 46 certified and licensed beds that usually has about 36 residents each day, and the direct owners are Mendel Schneider with 75% and Judd Schneider with 25%, and the facility runs as a for-profit limited liability company that's part of a corporation rather than a continuing care retirement setup, so you've got a place where they do a lot of daily nursing, aide care, respite care, hospice care, wound care, and set up all kinds of therapies from speech to psychiatric to IV, even though they don't offer physical therapy services since those staff hours come out as zero, but they'll still have things like occupational, psychological, and speech therapies that are led by licensed professionals, plus there are licensed physicians, consultants, and a social services team to help residents through admission and beyond. There's a registered nurse onsite day and night, and the staff includes licensed wound care specialists, licensed social workers, and a mix of aides and nurses, but the nurse turnover rate is higher than usual at 52.8%, with nurse staffing a bit below state averages at 3.26 hours per resident per day since you should expect about 25 minutes with an LPN and 28 minutes with an RN, so that's under what you'd get in most Illinois homes, but state requirements say they're supposed to provide at least 0.55 RN hours and 2.45 hours with a CNA each day for every patient, aiming for a minimum of 3.45 total nursing hours per day.

Residents get help with everyday needs like eating, getting dressed, bathing, and there's 24-hour care plus therapy, pharmacy, lab, X-ray, dental, optometry, dietary help overseen by a registered dietitian, and podiatry, so there's always skilled and assisted care on hand and meals are planned with birthdays and anniversaries in mind. For social life, there's always games, movies, holiday parties, religious seminars, arts and crafts, birthday and anniversary celebrations, and counseling or talk therapy options for individuals, families, or groups, and all the services can be offered in English, Spanish, or with interpreters, so pretty much everyone can get what they need.

This facility takes Medicaid and Medicare, so residents and families often rely on those programs, but there's been a long history of inspection deficiencies-63 in total-including specific issues with abuse, neglect, and exploitation, because sometimes staff didn't protect residents or report suspected incidents, and they've also had problems with infection control, shown by 7 infection-related infractions, some nurse call lights have been stuck behind furniture or out of reach, and safety issues have come up, like a metal rod sticking out from an unlocked utility room door and exposed wires in certain utility rooms. There have also been concerns over how residents' money is managed, including when trust fund accounts go over $1,800, as well as not always meeting the needs and wishes of each resident. Still, the Illinois Department of Health and Human Services comes in for inspections about every 18 months.

On one hand, it's worth noting they're above the state and national average for making sure residents get their pneumococcal vaccines, and 99.2% of long-stay residents have received theirs, which is pretty good, and they haven't had any reports of short-stay residents getting new or worse pressure sores, and fewer short-stay residents are ending up in the emergency department compared to other places, though 9.4% of long-stay residents have urinary tract infections, which is higher than usual for both Illinois and the country. The staff say they're focused on empathy and non-discrimination, picking workers for their skills and compassion, but the official reports say there's room to improve on safety and infection control. Overall, Foster Health & Rehab Center aims to provide extensive care and a range of therapies and activities, but also has a documented need to make things safer and respond better to residents' concerns.
