    Caring staff and clean facility

I moved my mom here and am very pleased. The staff are genuinely caring and attentive - nurses, CNAs and therapy (especially physical therapy) were excellent - and helpers like Barry, Sherri and Mary were wonderful. The facility is clean, well-organized and homey with a diner-style dining room nearby, great food, large common areas, regular worship and activities, chaplain/hospice support, and thoughtful integrative therapies. Convenient location and good parking - warm, competent care throughout.

    Summary of reviews

Reviews of Woodbury Health Care Center describe a facility with clear strengths in rehabilitation, compassionate caregivers, and active programming, paired with recurring operational challenges that affect long-term care experiences. Many families cite strong physical and transitional therapy services, attentive hospice partnerships, and a number of staff who provide warm, individualized attention. The facility offers a variety of social and spiritual activities — including resident-led groups, musical performances, art activities, and worship services — and several communal areas and dining spaces that contribute to a social environment for short-term rehabilitative stays.

Care quality appears mixed depending on unit and length of stay. Rehab and transitional-care recipients consistently receive positive marks for therapy effectiveness and organized recovery services. For longer-term residents, however, reviewers describe patterns of delayed assistance (including bathing and toileting schedules), inconsistent medication timing and documentation, and uneven responsiveness to pain-management requests. These clinical gaps are frequently attributed to inadequate staffing levels and to variability in staff experience and training.

Staff descriptions are largely dual: many reviewers name caring, dedicated nurses and aides who connect well with residents, while others note inexperienced or overstretched staff who miss chart details or fail to follow care plans. Communication breakdowns between staff and families are noted, with examples of families needing to cue staff about resident capabilities and needs. Safety and supervision concerns are raised in multiple accounts — for example, wandering residents entering rooms and limited on-unit security oversight — which prospective families should consider, particularly for residents with cognitive impairment.

Dining and daily living services also show divergence. Several reviewers praise food quality and the cafeteria atmosphere, while others report frequent meal inaccuracies, tray issues, and inconsistent menu fulfillment. Housekeeping and environmental maintenance receive mixed comments: some units are described as clean and well maintained, whereas other areas are described as showing age, with odor concerns and infrastructure issues such as small rooms, shared bathrooms, and dated finishes.

Management and administrative practices are another recurrent theme. Positive experiences highlight organized kitchen and therapy leadership; negative feedback centers on slow or dismissive complaint handling, billing disputes, and inconsistent resolution of missing personal items or paperwork. There are isolated, serious claims involving mishandling of personal documents and alleged theft; these should prompt direct inquiry during a tour and review of the facility’s policies for belongings and incident reporting.

Notable patterns: 1) a clear distinction between the stronger, well-resourced rehab/transitional units and the more challenged long-term units; 2) staffing shortfalls appear to underlie many operational issues (delayed meds, slow call response, inconsistent personal care); and 3) activity and spiritual programming are strengths that enhance resident quality of life when staffing allows implementation. Prospective residents and families should tour the specific unit under consideration, ask about current staffing ratios and training, review medication administration and incident-reporting practices, confirm bathroom arrangements, and inquire about security/supervision protocols and how the facility documents and resolves complaints.

    About Woodbury Health Care Center

Woodbury Health Care Center has 155 certified beds and usually about 102 residents daily, and you'll find it at the corner of Lake Road and Woodland Drive in Woodbury, Minnesota, right east of Ojibway Park with walking paths close by, so residents don't have to feel closed in. This place sits as part of a senior living campus with Woodbury Villa and Woodbury Estates connected, and you can hear the kindness in how staff try to provide for residents' needs, working from doctors' orders but also listening to their preferences and goals. There's support to help people manage chronic illnesses, plus a program for folks healing up after surgery called The Therapy Suites at Woodbury Health Care Center, and they make room for all sorts of care, like skilled nursing, transitional care, memory care, and both short-term and long-term stays. The health care center brings in trained nurses and caregivers who handle complex medical needs, and there are specialists on-site to help with therapy like physical, occupational, and speech therapy, and you can get IV therapy, tube feedings, pain management, and wound care if needed.

The Enriching Connections memory care program helps residents with memory and cognitive troubles, with dementia-certified staff working on cognitive therapy, stroke rehab, continence, and all the things people need when memory changes grow. The place also offers integrative therapies, so you'll find music therapy, guided imagery, essential oils, massage, and other ways to ease the day. There's an on-site chaplain for spiritual support, and the apartments-including private rooms with electric recliners, TV, Wi-Fi, and telephones-are close to the rehab department, so it's not far if you need extra treatment, and you won't have a private room surcharge either. Amenities include a computer station for games and internet, and the activity calendar covers gardening, trivia, dinner, movies with a popcorn machine, and social times to keep folks busy or social if they want, plus exercise and recreational activities for dexterity and flexibility.

Transitional care includes private rooms, an activity room, a beauty shop, and discharge planning to help people get back home if they're able. Assisted living services help with things like bathing, dressing, feeding, toileting, and moving about, and there's 24-hour supervision with laundry, housekeeping, dietary, and medication help for those who need it. The memory care floor has areas set up just for residents, aiming for calm and a feeling of safety, and supervised care is constant. Woodbury Health Care Center is Medicare and Medicaid certified and partners with Lifespark for some services, and you'll find cardiac care, pulmonary care, neuro, orthopedic support, and all sorts of therapies under one roof, as well as memory support, pain management, and wound services for those who need those kinds of help.

While this community tries to keep things peaceful and resident-focused, inspection reports have shown some problems, with 48 documented deficiencies, some infection-related, including citations for failing to keep residents safe from hazards, not always providing care per doctors' orders or residents' preferences, and lapses in respecting residents' dignity. There are both complaint and standard inspection reports, pointing out areas needing attention in safety and care practices. The center aims to offer apartments and programs that feel like home but still have secure and supervised care. The staff provides both short-term and long-term nursing support, and you'll see discharge planning so people don't stay longer than they have to if they're able to return home. This might be a good option for those needing many levels of medical care close by, with activities, support, and therapies, but it's important to review inspection histories if that's a concern for you.
