    Excellent rehab care and facilities

I had rehab at River Ridge and was very satisfied - top-notch physical therapy, knowledgeable, compassionate and attentive staff, and strong discharge planning and care coordination. The facility and grounds are beautiful and very clean, the food and rooms are good, and leadership is visible; I would recommend it.

    Summary of reviews

Reviewers describe River Ridge Living Center as a facility with clear strengths in rehabilitation services, facility upkeep, and aspects of daily life, alongside persistent operational challenges. Many reviewers praised the therapy department โ€” physical and occupational therapy is repeatedly characterized as effective and instrumental in helping residents regain strength and return home. The building and grounds are commonly described as clean, modern, and attractive, with spacious rooms, a pleasant dining area, and outdoor spaces such as a courtyard that contribute to a home-like atmosphere. Dining quality is generally viewed positively, and a range of activities (including visiting entertainers) is noted as available.

Direct-care staff elicit strongly mixed impressions. Several caregivers are highlighted for their compassion and attentiveness, and some family members found nursing leadership approachable and communicative. At the same time, reviewers consistently point to variability in staff performance: frequent turnover, reliance on agency staff, and reduced staffing on weekends contribute to uneven day-to-day experiences. This variability manifests operationally as delays in assistance, inconsistent medication and care-administration practices, and uneven staff conduct and competence.

Safety and operational practices are recurring areas of concern. Multiple reviewers describe falls and other safety-related events, alongside equipment or maintenance problems that can affect resident comfort and safety. There are also repeated concerns about long response times for resident needs and gaps in how clinical incidents are handled and communicated to families. A small number of reviewers described isolated serious incidents that involved external responders or significant clinical complications; these accounts underscore the importance of clear incident-response protocols and timely family notification.

Communication and management responsiveness receive mixed evaluations. Some families report proactive discharge planning, strong care coordination, and visible leadership involvement; others describe difficulty reaching administration, inconsistent family communication, and a perception of limited accountability after adverse events. Weekend staffing and the variable use of agency personnel are specifically noted as times when communication and responsiveness can decline.

For prospective residents and families: River Ridge appears to offer strong rehabilitation services, clean facilities, pleasant dining, and a generally welcoming environment when regular staff are present. However, operational variability โ€” especially around staffing levels, agency-worker consistency, incident response, and weekend coverage โ€” is an important pattern to probe during tours and admissions discussions. Suggested questions include current staffing ratios (including weekends), use and training of agency staff, fall-prevention protocols, equipment-maintenance processes, and the facilityโ€™s family-notification and incident-reporting procedures. These inquiries can help align expectations with the facilityโ€™s strengths and the operational issues noted by reviewers.

    About River Ridge Living Center

River Ridge Living Center sits on a quiet 30-acre property in a residential area, with a total of 120 certified beds and an average of 113 residents daily, so things don't usually feel too crowded, and you'll find a range of care types including long-term care, short stays, and subacute rehab, while rehabilitation services come with a big, modern gym full of high-tech equipment for physical, occupational, and speech therapy, which therapists adjust for each person's needs, and they do aim to help short-term rehab patients get home in about two to three weeks whenever possible, but there have been some issues in recent inspections, like a nutrition and dietary deficiency related to food handling, and another when some residents didn't get the special eating utensils they needed, though there wasn't any actual harm, there was a citation for pest control, and federal standards on infection have been cited too, with a total of 22 deficiencies reported just in March 2025, and 37 across several years, so the inspection record's not spotless, but the facility does have round-the-clock care from skilled nurses and licensed practical nurses who take care of medication, wound care, and vital signs, while a certified nursing assistant helps residents with daily routines, and an activity assistant runs group events and things to fill the days, plus big annual gatherings like a community carnival make life more social, though the nurse turnover rate is higher than the state average and daily nurse staffing hours are a bit below the average too, at 3.44 per resident compared to 3.7 statewide, but the rooms and common spaces are newer and bright, with lots of windows, greenery, and scenic views, and there are partnerships with big local hospitals that help ensure care continues without interruption when needed, while the culinary department serves meals described as both nourishing and good-tasting, always paying attention to health, and there's a dedicated wound nurse for things like wound vacs or IV antibiotics, plus outpatient therapy, dialysis coverage, wellness programs, and a discharge planner for smooth transitions, the ownership's family-based, split between Edward Braunstein, Michael Braunstein, Benjamin Einhorn, and Louis Gellis, which helps keep a home-like environment, and recent renovations mean spaces feel more modern and comfortable, with a general focus on supporting independence and making the place feel welcoming, though detailed info on management practices isn't available.
