    Clean caring staff; excellent rehabilitation

I placed my mom here and have been very pleased - the building is clean, staff are professional, compassionate and attentive, and rehab/therapy gave excellent care. Teresa Edwards (DON), Leah Cothran, Tabitha Smith and Jessica in the business office stood out for communication and going above and beyond. It feels like a caring, family-like place and I'd recommend it.

    Summary of reviews

Yanceyville Rehabilitation and Healthcare Center elicits strongly polarized impressions. Many families and residents praise the facility's frontline caregivers — CNAs, RNs, therapists, activities staff, and several members of the administrative team — for compassionate, individualized attention. Reviewers frequently cite a family-like atmosphere, an engaged activities program that blends physical and cognitive options, effective short-term rehabilitation outcomes, and helpful business-office support for benefits navigation. Housekeeping and maintenance are also noted as strengths in multiple accounts, and some unit-level leaders and the social-work/psychiatric supports are described as responsive and proactive.

Concurrently, recurring operational concerns appear across reviews. Staffing inconsistency is a prominent theme, with reports of thin coverage at night and on the memory-care unit leading to delayed responses to call lights and interruptions in routine care. Those delays extend in some accounts to medication administration and assistance requests. Relatedly, several reviewers describe safety-related lapses such as insufficient supervision and fall-risk management; these are framed as gaps in safety protocol adherence rather than isolated events.

Communication and management practices show variability. While some families describe clear, frequent updates and collaborative care meetings, others report difficulty reaching staff by phone, unreturned requests, or late responses from social-work or hospice contacts. There are mixed assessments of leadership: certain nursing leaders and the activities director receive high praise for hands-on involvement and staff support, whereas other comments describe administrative turnover and periods of poorer oversight that coincide with declines in day-to-day operations.

Facility and service-level inconsistencies also emerge. Cleanliness and odor-control are seen as adequate or better by many, yet some reviews note sanitation and odor-control concerns in particular areas or units. Dining quality is similarly variable: some residents and families commend the meals and nutrition during rehab stays, while others find menu choices limited or of uneven quality. Reviewers raise clinical-quality concerns beyond routine care issues as well, including alleged mishandling of diagnostic specimens and at least one serious allegation related to an injury and subsequent documentation; these statements point to potential gaps in clinical-incident documentation and transparency that would merit targeted review.

For prospective residents and families, the pattern suggests a facility with strong, committed front-line caregivers and meaningful programming, but with operational vulnerabilities that can affect continuity of care. Key due-diligence steps would include asking about current staffing ratios (day/night), memory-care staffing and supervision practices, medication-administration protocols, clinical-incident reporting and family notification policies, and recent leadership or administration changes. Visiting multiple units at different times, meeting unit nursing leadership, and reviewing recent state inspection or complaint records will help clarify whether the strengths described are consistent and whether the operational issues have been addressed.
