    Caring staff, effective therapy, smooth

I'm very pleased with my experience - the staff were consistently kind, attentive, and went above and beyond (special thanks to Ms. Angel Watson and Ronni). Therapy was excellent and helped improve mobility; the facility is clean, comfortable, secure, and meals are good most days. Communication with family and outside doctors was strong and the transition was smooth, so I recommend this place.

    Summary of reviews

Reviewers describe a facility with clear operational strengths in rehabilitation, cleanliness, and social engagement alongside significant operational inconsistencies. The physical-therapy program and associated clinical expertise receive positive mentions, including training provided to families to support post-discharge care. Long-term staff members are frequently characterized as knowledgeable and compassionate, and the building and public spaces (including the lobby and coffee area) are described as clean and well maintained.

Care quality appears uneven. Several accounts praise consistent nursing care from experienced personnel and effective therapy services, while others identify reliability gaps in day-to-day personal careโ€”specifically inconsistent bathing schedules, irregular linen changes, and delays addressing incontinence-care needs. Reviewers also raised concerns about delayed bedside response times and inadequate pain-management responsiveness, indicating variability in how promptly staff attend to urgent resident needs.

Staffing and supervision emerge as central themes. Comments indicate a tension between strong long-term staff and high overall turnover, which reviewers say contributes to service delays and inconsistent resident interactions. There are concerns about staff screening and supervision practices, including reports suggesting potential impairment on shift and questions about background-check rigor. Communication skills and professionalism are described as uneven across staff, with some caregivers singled out for exceptional kindness while others were characterized as less capable in direct resident communication.

Dining, activities, and facilities are generally viewed positively. Meals are described as โ€œgood most days,โ€ and the facility appears to offer an active calendar of activities. Housekeeping receives favorable notes: rooms are tidy, floors are regularly mopped, and common areas are maintained. However, operational weaknesses such as missing personal items and lapses in medication or supply management point to inventory-control and clinical-administration gaps that should be addressed.

Management and cost considerations are important for prospective families. The facilityโ€™s monthly fee level was described as relatively high, and reviewers suggested that management should improve staffing levels, enhance supervision and screening, standardize personal-care routines, and tighten medication and diagnostic protocols (for example, ensuring appropriate testing and follow-up when clinical concerns arise). Given the mixed reports, prospective residents and families may wish to inspect recent staffing patterns, ask about turnover, confirm clinical and background-screening policies, and observe care routines during different shifts before deciding.

In summary, Chardon Center shows strengths in rehabilitation services, cleanliness, communal spaces, and activity programming, but reviewers identify recurring operational issuesโ€”most notably staffing capacity and consistency, personal-care scheduling, clinical monitoring, and administrative controlsโ€”that affect the day-to-day experience for some residents. These are actionable areas for management to target to produce a more consistently reliable standard of care.

    About Chardon Center

Chardon Healthcare Center sits at 620 Water Street in Chardon, Ohio, and stands as a skilled nursing facility that offers both long-term and short-term care for older adults, and being in the nursing home category, it's part of a large family-owned company known for post-acute care, so you'll find all kinds of healthcare specialties here, like skilled nursing services, wound care, hospice support, and palliative care, along with physical, occupational, and speech therapy, plus specialized programs like cardiac care and the Advance 360 program which aims for the best health outcomes. The staff runs support programs and keeps up a welcoming environment for residents and their families, trying to look after emotional as well as physical well-being, and for people working on getting stronger or healing after a hospital stay, you'll find short-term recovery and senior rehabilitation services, with long-term care for those who need support every day.

The center wins points for having Medicaid certification, and there are specific amenities meant for comfort, plus equipment for patient use and programming for veterans, where folks who've served get special recognition and dedicated services, so veterans don't feel left out. Social care is in focus too, with all sorts of activities and community events like bingo nights, animal visits, and live entertainment, so residents have chances to join in and stay connected, and these features match the needs of elderly care, with patient rooms and shared spaces made for easier living. Families of residents often find a supportive environment, with guidance for both guests and loved ones built into the routine.

Chardon Healthcare Center also has some unique programs and term names for supports you'll see around, and you can check their information on the official website at http://www.communicarehealth.com/ or the additional site at www.chardonhealthcare.com, and while the main campus is in Chardon, they've got an alternative location at 4700 Ashwood Drive #200, Blue Ash, Ohio. All in all, the center brings together nursing care, specialized medical services, therapy, social events, and family support under one roof, taking a practical approach so older adults get what they need, whether that's for recovery, daily living help, or emotional and physical well-being in their later years.
