    Caring staff and excellent rehab

I'm very pleased with the caring, professional staff and proactive administration - they provide thorough medical oversight and excellent emotional support. The facility is exceptionally clean and home-like, the rehab/therapy room is beautiful with skilled therapists who helped my loved one make real progress, and communication (family meetings, caseworker/head nurse) has been helpful. Meals are decent, activities are engaging, and COVID precautions felt thorough.

    Summary of reviews

Lake City Scranton Healthcare Center elicits sharply mixed impressions. Many reviews highlight strong clinical and rehabilitative capabilities: nurses, therapists, and case managers are regularly described as caring and effective, with a well-equipped therapy room and evidence of measurable progress for residents in rehab. Several accounts praise nursing leadership and medical oversight, and social-work support and family meetings are noted as helpful. Activity programming and resident celebrations (for example, nursing home week) are frequently mentioned as positive contributors to resident engagement and morale.

Dining and day-to-day comforts receive mixed assessments. A number of families appreciate home-style meals and menu variety, while others cite inconsistent food quality or seasoning. Cleanliness is similarly variable: some reviewers describe excellent room- and equipment-level cleanliness, but others indicate uneven housekeeping standards and odor concerns in particular common areas and back corridors. The facility is characterized as older in parts, with visible wear and maintenance needs that can affect the overall impression.

Significant operational concerns appear repeatedly and represent patterns to watch. Multiple reviews describe inconsistent staff conduct and responsiveness across shifts and roles, which can translate into delayed assistance, delays in attending to toileting and hygiene needs, and delays in clinical escalation after incidents. Communication with families is another common issue: phone access, long hold times, and limited proactive information-sharing were cited, and some families found visitation and room-viewing procedures burdensome or appointment-driven.

Safety and clinical management concerns are present in some accounts, ranging from worries about medication and pain-management practices to inconsistent infection-control observations during respiratory outbreaks. At the same time, other reviewers commend the facility's COVID precautions and post-infection care, underscoring variability in experience across time or units. Taken together, reviews suggest a polarized picture: the facility demonstrates clear strengths in therapy, certain elements of nursing care, and occasional excellent cleanliness and administration, but those strengths coexist with operational inconsistenciesโ€”particularly in housekeeping, staff responsiveness, family communication, and facility maintenance.

For prospective residents and families, the predominant patterns suggest asking specific, operational questions during a tour: staffing levels and shift coverage, protocols for toileting and timely assistance, wound and pain-management policies, infection-control measures, housekeeping schedules (including problem-area follow-up), and the facility's approach to family communication and visitation logistics. A focused visit that inspects common areas and back corridors and confirms current staffing and clinical processes will help clarify whether the facilityโ€™s strengths align with the prospective residentโ€™s priorities.

About Lake City Scranton Healthcare Center

Lake City Scranton Healthcare Center sits on Boyd Street in Scranton, South Carolina, and serves seniors needing skilled nursing care and rehabilitation. The center has 88 certified beds and usually takes care of about 82 residents every day. It's a for-profit corporation, owned and operated 100% by Palmetto Health Care LLC, with indirect ownership by Murray Forman, and it does have an affiliation with Fundamental Healthcare. This place focuses on recovery from illness, injury, or surgery, offering physical, occupational, and speech therapy, as well as skilled nursing services tailored to each person's needs. Some residents are active seniors, while others may have Alzheimer's or dementia, and there are programs to help support both groups-like therapy to reduce confusion or prevent wandering, and activities that keep people socially and mentally engaged.

Nursing staff turnover runs at 41.9%, with an average staff level of 3.02 nursing hours per resident each day, and people say the staff here are friendly, helpful, and do their best to communicate with families and residents. The meals aim to be nutritious, and the environment's meant to be supportive and respectful. Lake City Scranton Healthcare Center tries to treat everyone equally, no matter their background or condition.

However, inspection reports, including a full one from May 23, 2024, show 22 deficiencies, with specific problems in protecting residents from abuse, neglect, and exploitation, as well as gaps in respiratory care and help with activities of daily living. There have also been violations of federal infection standards, including an infection-related deficiency, and complaint and infection-control reports have been reviewed in inspections over the last three years. The center calls itself a healing environment with high-quality services and aims to improve life for people in recovery, but it's important to know there have been several documented issues through state and federal inspections, which families may want to review. The administrator is Rachel McKenzie, and the center keeps policies for fair housing and equal opportunity. Activities and amenities are similar to what you find at many health care facilities, aiming to support both daily life and medical needs.
