    Compassionate professional care and rehab

I'm very pleased with the care my mom received - compassionate, professional and responsive staff who kept her comfortable and kept our family informed. The facility is clean, bright and secure, with tasty meals, engaging activities and effective PT rehab that made her stronger. I recommend a tour - it's well-run, welcoming, and the team genuinely cares.

    Summary of reviews

Overall impressions of The Highlands of Memphis Health & Rehabilitation are mixed, with a clear split between families who experienced consistently strong clinical care and those who encountered operational shortcomings. Positive accounts emphasize professional nursing, CNAs, and physical-therapy teams that helped residents make measurable improvement; reviewers highlighted effective rehabilitation, respectful treatment, and attentive end-of-life support. Many families praised the facilityโ€™s clean, bright rooms, family-friendly visiting space, a pleasant dining room (including a piano and communal tables), and engaging activities such as bingo, movie nights, picnics, and live music.

At the same time, a substantial portion of feedback points to operational inconsistencies. The most common themes are uneven staffing levels and delays responding to resident calls, which reviewers linked to delays in personal-care assistance and toileting support. Reviewers raised sanitation concerns and odor control problems in some areas, and there were several accounts of delayed meals, variable food quality, and inconsistent meal-service timing. Laundry management and tracking of personal items were also noted as an area needing improvement.

Communication and management follow-up surfaced as recurring concerns. While some families praised hands-on administrators and staff who kept them informed, others described poor follow-through from desk or supervisory staff, limited weekend coverage, and errors or confusion around insurance and billing. Staff conduct was described as largely compassionate by many, but other families reported encounters with staff who displayed a brusque tone or failed to communicate effectively; this variability in interpersonal conduct contributed to a polarized overall impression. Reviewers also described inconsistent clinical outcomes between units and shifts, suggesting variability in adherence to care plans or staffing practices.

For prospective residents and families, the facility offers notable strengths in rehabilitation services, social programming, and several areas of compassionate clinical care. However, the pattern of inconsistent responsiveness, occasional sanitation and meal-service issues, and mixed experiences with management communication suggests potential risk for families who require consistent around-the-clock assistance. When evaluating this facility in person, consider observing mealtime service, asking about staffing ratios and call-response metrics (including weekend coverage), reviewing sanitation and incontinence-care protocols, verifying billing and insurance coordination in writing, and requesting to meet therapy and nursing leadership. Speaking with current family contacts and scheduling a tour during an activity or meal period can help clarify how consistently the facilityโ€™s positive features are delivered.

About The Highlands of Memphis Health & Rehabilitation

The Highlands Of Memphis Health & Rehabilitation, also known as Bay at Highlands Health and Rehabilitation Center LLC, sits at 3549 Norriswood Avenue in Memphis, Tennessee, and offers skilled nursing care for up to 180 residents, usually serving about 152 people each day, and people there can get healthcare for various needs like diabetes, incontinence, and higher-acuity medical problems, along with short-term rehabilitation, outpatient therapy, memory care, and comfortable long-term stays, and though meals provided focus on nutrition and quality, and daily activities happen to keep folks engaged, the center has had some challenges with care-state inspectors found 23 deficiencies during reviews, with problems like not always making full care plans for residents and not keeping all areas safe from accident risks, and there have been four infection-related citation notices too, plus the nurse turnover rate at 53.1% stays above the Tennessee average, and nurse staff hours per resident per day, about 3.24, sit below the state average of 3.9. The building's common areas and indoor spaces are set up for social use, and the staff offers personalized care and therapy, with CNA training programs also available, but the place hasn't had a standard inspection in over two years, and while the management's handled by Champion Care LLC and direct ownership belongs to The Bay At Highlands Holdco LLC, residents and families often say staff try to offer compassionate care and work hard to keep the building clean. Right now, the facility isn't taking new patients, but those already there get services in English and have access to both spiritual and offsite activities, and there's a continued focus on comfort and meeting clinical needs as much as possible in a busy setting.
