    Caring professional staff, clean facility

I placed a family member here and have been very pleased - the staff are caring, professional and well trained, from nursing and CNAs to therapy, dietary, housekeeping and admissions. The building is clean, warm and welcoming, activities keep residents engaged, and they handled COVID responsibly. Residents are treated like family and every department works together, so I feel confident recommending this facility for short-term rehab or long-term care.

    Summary of reviews

Crossville Health and Rehabilitation is characterized by a consistent emphasis on person-centered caregiving and rehabilitation. Reviewers most commonly praise the nursing staff, CNAs, and therapy teams for compassionate, patient-focused care and for producing good short-term rehab outcomes. Several families specifically highlighted knowledgeable therapists, attentive nursing, and confident rehabilitation planning, suggesting the facility is capable of supporting both short-term post-acute recovery and longer-term care needs.

Staffing and management are frequent strengths. Multiple commenters described a warm, family-oriented culture supported by visible leadership โ€” administrators, the director of nursing, social workers, and admissions staff received positive mention for accessibility and responsiveness. Front-desk and admissions interactions were described as welcoming, and the social-work function was noted as a helpful resource for families. These elements combine with cross-departmental teamwork to create an environment many families felt was safe and well supervised.

Operational aspects such as cleanliness, decor, and food service are generally viewed positively. The building appears renovated in parts, with reviewers noting clean common areas, well-groomed residents, and pleasant dรฉcor. Dietary staff and housekeeping were repeatedly praised; several family members called out the quality and attentiveness of kitchen and environmental services. The activity program is active and varied, with music, bingo, and singing groups cited as regular engagement options that contribute to a homelike atmosphere and resident socialization.

Notwithstanding the many positive comments, there are recurring operational concerns families should consider. Communication with families is uneven: reviewers mentioned gaps in updates and coordination that affected satisfaction. A limited number of critiques point to inconsistent staff conduct โ€” examples ranged from inattentive behavior to unprofessional interactions โ€” indicating variability in bedside manners or frontline performance. There were also specific concerns about ancillary-care coordination, notably dental care, which suggests families should confirm how specialty and preventive services are arranged. Room configuration and availability may pose limitations for some residents; small or shared rooms were described, so privacy preferences and room-size requirements warrant direct inquiry. Finally, a few accounts described episodes implying delayed responsiveness to daily or clinical needs; while these appear to be isolated relative to the overall positive comments, they suggest monitoring staffing continuity and response protocols during evaluations.

In summary, Crossville Health and Rehabilitation presents as a largely well-run facility with strengths in caregiving culture, rehabilitation services, cleanliness, and activities. Leadership presence and interdisciplinary teamwork are positive attributes families repeatedly cited. Prospective residents and their families would be well advised to confirm specifics during a visit: ask about private-room availability, typical staffing levels and shift coverage, processes for family communication and incident notification, and how ancillary services (dental, specialist appointments) are coordinated. For those who prioritize a compassionate staff, active programming, and an environment described as clean and family-oriented, this facility aligns with those priorities; for those with strong preferences for private-room space or intensive ancillary care coordination, additional due diligence is recommended.

About Crossville Health and Rehabilitation, LLC

Crossville Health and Rehabilitation, LLC sits over in DeKalb County at 8922 Alabama Hwy 227 N., Crossville, AL 35962, and it offers nursing home and rehabilitation services for seniors and people needing care, and you'll see it's got 143 certified beds and usually about 129 residents a day, so it stays active. The facility's known for having a caring and home-like environment, with a staff that includes professionally trained nurses, certified nurse aides, and therapists, and you might notice a friendly welcome when you visit since the staff seems open to showing folks around if they're thinking about moving in. The place has a restaurant-style dining area for meals, common areas that are nicely decorated, and outdoor courtyards and sitting areas where folks can sit and relax, which gives it a pleasant and comfortable feeling.

Residents can do daily activities to help stay involved, while those who need therapy can get physical, occupational, and speech therapies, and there's support for caregivers and legal services, even prescription help or nutrition programs. There's also Wi-Fi in rooms, cable TV, laundry, beautician services, and public transportation for health appointments like dental and optical visits, and they run community programs too, including support for aging at home and living well, Medicare counseling, and senior wellness resources. The facility also provides COVID-19 testing, has an admissions counselor for tours, and tries to keep things easy for those moving in or out.

The ownership and direction come from Northport Holding Operations, LLC, with affiliated managers like Tammy Cox, Derek Patterson, Lynn Rasco, and Sonny Ward, and you'll see the place is also connected with NHS Management. The facility has a nurse staffing level of about 4.00 hours per resident daily, which is a bit above the state average of 3.9, and the nurse turnover rate is 50.7%, which is a little higher than the state's average of 48.1%.

Crossville Health and Rehabilitation, LLC has had deficiencies found in inspection reports over the years, with thirteen total deficiencies documented, three of those tied to infection-related issues not meeting federal standards, and during a standard inspection in November 2021, the inspectors found two problems, including one linked to infection control. There was also a Resident Rights Deficiency (F0550) for not fully honoring residents' rights to dignity and communication, though there wasn't actual harm-only the potential for more than minimal harm-and a complaint inspection from July 2023 turned up another deficiency too. The reviews online average out to a 4.5 rating from 23 people, and the community tries to provide quality care in a pleasant setting, although like anywhere, it has its share of challenges and areas that need attention.
