    Life-saving wound care compassionate staff

After a prompt relocation I spent five wonderful months here; the wound care was life-saving and the wound nurse went above and beyond. The compassionate nurses and CNAs treated me like family, were honest from the start, and took great care of me.

    Summary of reviews

The reviews paint a mixed picture of clinical strengths alongside persistent operational weaknesses. Positive feedback centers on wound-care nursing and certain direct caregivers: several families described individualized, compassionate care from particular nurses and CNAs, and some residents experienced effective physical and occupational therapy. Recreational programming such as church singing and bingo is present, and there are reports of staff who have delivered prompt, intensive interventions when needed.

At the same time, a consistent pattern emerges around inconsistent care quality and responsiveness. Reviewers describe long delays in attending to resident calls, missed or withheld medications, and concerns about medication-access controls. Multiple accounts suggest falls, pressureโ€‘injury risk, and hospital transfers that families felt were not communicated promptly. While specialized wound care received praise in some cases, the facilityโ€™s overall clinical performance appears variable across units and shifts.

Staffing and staff conduct are frequent areas of concern. Accounts indicate staffing shortfallsโ€”particularly on weekendsโ€”and behaviors described as unprofessional or dismissive by admissions, discharge, and frontline staff. Families reported delays in routine personal-care tasks (bathing, dressing), inconsistent room cleaning, and instances that raised dignity and privacy concerns. Conversely, several reviewers singled out individual caregivers for compassionate, familyโ€‘like attention, indicating uneven performance between staff members.

Facility operations and administration also show recurring issues. Reviewers cite long hold times when calling the facility, unresponsiveness from discharge/admissions staff, billing disputes, and requests for regulatory contact information that they say were not fulfilled. There are sanitation and odor concerns in some common areas, along with reports of lost clothing and inconsistent housekeeping standards. A few reviewers reported that OTC medications or supplies were not adequately secured, indicating potential gaps in supply controls.

Dining and activities present a mixed impression: some families found therapy and certain activities beneficial, while others described inconsistent food temperatures and meal quality. Overall, the facility appears capable of providing good care in certain circumstancesโ€”notably advanced wound management and some rehabilitative therapyโ€”but that quality is not consistently experienced across all residents or shifts.

For prospective residents and families: confirm current staffing ratios and weekend coverage, ask for details about medication-management protocols and supply security, review the facilityโ€™s incident-reporting and family-notification policies, and request to meet the wound-care and therapy leads if those services are important. Visiting multiple times and speaking directly with nursing leadership about cleaning schedules, billing procedures, and emergency response expectations may help assess whether the facilityโ€™s strengths align with a specific residentโ€™s needs.

About Tallasee Health and Rehabilitation, LLC

Tallassee Health and Rehabilitation, LLC sits in Tallassee, Alabama, and operates all day and night providing health and rehabilitation services that cover both inpatient care for those who need to stay for treatment and outpatient therapy for folks who come in for sessions and go home after. The facility has 111 certified beds and usually cares for about 103 residents each day, with a nurse staffing rate almost equal to the state average. The staff includes certified nurses' aides, nurses, and therapists, and they all work to deliver personal care, especially with a focus on skilled nursing and physical rehabilitation. Managerial control of the place has shifted a few times in the last couple of years and right now falls under Bryan Baggett, Cameronesha Nettles, Lynn Rasco, and Julie Richburg, with Millennium Health Operations, LLC owning the facility, and some indirect ownership through James Estes, two dynasty trusts, and Jennifer Agee's trust, plus a connection to NHS Management. The environment feels home-like and compassionate, with staff aiming to make residents feel comfortable, and there are daily activities and outdoor courtyards where people can relax. The common areas are nicely decorated and rooms come with cable television and free Wi-Fi, plus residents have restaurant-style dining, weekly menus, beautician, and laundry services. Folks can take virtual or in-person tours, and families can visit or join virtual visits, which helps everyone feel more at ease. While COVID-19 testing is available and HIPAA privacy and nondiscrimination policies are in place, reports from recent inspections raised concerns about infection control and resident protection from abuse and neglect, with two notable deficiencies and a total of 11 documented issues, and the most recent standard inspection happened more than two years ago, showing delays in review compared to most places. The nurse turnover rate sits above the state average at 54.1%, and the place has been cited for not fully protecting residents from harm, though no actual harm was found. The admissions process uses a counselor to help new residents get settled. Tallassee Health and Rehabilitation, LLC provides a comprehensive range of long-term care and rehab, with amenities like outdoor spaces and activities, aiming to offer an alternative to other long-term care options.
