    Excellent compassionate care and rehabilitation

I placed my mom at Rim Country Health & Rehabilitation and was very pleased - the nursing and therapy teams were attentive and knowledgeable, helped her regain strength and speech, and the admissions director truly went out of her way to help. The facility is spotless, welcoming, and comfortable with delicious meals, engaging activities, beautiful grounds, and staff who consistently go above and beyond, providing compassionate palliative care and peace of mind. I recommend it for rehab or long-term care.

    Summary of reviews

Rim Country Health & Rehabilitation presents as a physically appealing, single-story facility with well-maintained interiors and attractive outdoor spaces. Reviewers frequently highlight the central courtyard, remodeled independent-living apartments, and available specialty equipment. The site offers typical skilled-nursing supports — full nursing coverage, visiting physicians, a therapy program, and transportation for appointments and outings — and many families describe the facility as clean and comfortable for residents.

Experiences with clinical care and staff conduct are mixed. Numerous families praise compassionate, knowledgeable nursing teams and therapists whose interventions led to measurable improvement for some residents. Therapy and rehabilitation are commonly cited as strengths, with several accounts of patients regaining function under the facility's program. At the same time, a set of serious concerns appears repeatedly: inconsistent responsiveness to family calls, difficulties reaching staff by phone, and reports of limited night coverage. Several families described delays in escalation and in answering emergencies, and some noted perceived gaps in coordination between physicians and nursing staff — for example, problems obtaining diagnostic orders or timely medication adjustments — that they believed contributed to recurrent clinical issues.

Dining, activities, and daily life also receive divergent assessments. Many reviewers commend the food quality, regular meal schedules, available snacks, and a broad calendar of activities and community outings. Volunteers and activity staff are viewed as supportive. Conversely, other families raised dietary-management concerns, including portion size and unintentional weight loss for certain residents. Transportation for community events and medical appointments is available and used for outings, though a number of comments describe schedule or pickup reliability issues.

Administrative and management themes are similarly mixed. Some families singled out admissions staff and individual caregivers for going above and beyond, and several noted attentive palliative or end-of-life care. Other comments raise concerns about customer-service tone from administrative staff, billing transparency (notably around respite-care costs and refund handling), and overall responsiveness to family questions. Taken together, the reviews portray a facility with substantial strengths in environment, therapy, and dedicated caregiving staff, but with operational variability in communication, staffing coverage (particularly overnight), clinical coordination with attending physicians, and some aspects of dietary and incontinence care. Prospective residents and families may benefit from direct questions about staffing ratios and night coverage, emergency response procedures, physician coverage and order processes, incontinence-care protocols, dietary planning and weight monitoring, transportation scheduling, and the facility’s billing/refund policies before admission.

About Rim Country Health & Rehabilitation

Rim Country Health & Rehabilitation sits over at 807 W. Longhorn Rd. in Payson, AZ, where you'll find a big senior living community with all sorts of care in a quiet, safe spot not tied to a hospital or any large retirement chain, and folks can move in for independent living, assisted living, skilled nursing, or memory care depending on what they need at the stage they're in, and there's a special wing just for people with dementia or Alzheimer's who need extra support day and night, with 24-hour supervision, memory programs, and secure grounds. The nursing home part has 109 beds where registered nurses, therapists, and caregivers provide care around the clock, whether it's long-term help, ongoing rehabilitation, wound care, or therapy like physical, occupational, or speech, and they even help people coming out of hospitals and those who need hospice or palliative care or just need a little time to recover before heading home, with services that include things like dialysis, cardiac care, and behavioral health help for seniors with depression or anxiety. You've got room choices like semi-private rooms or apartment-style units, and the place is designed for both privacy and community, since there're shared spaces like a dining hall for restaurant-style meals that care about nutrition, a fitness room, small library, computer center, beauty salon, wellness center, and outdoor courtyard space for enjoying fresh air or walking the dog, because they allow pets, and activities like bridge, games, and various programs run throughout the week, so there's always something to do for folks who can and want to join in. Rim Country also arranges rides to the doctor and gives complimentary in-state medical transport for admissions, while keeping a 24-hour emergency alert system and having staff nearby at all times, plus they provide adult day health care, respite services for families that need a break, placement help, and discharge planning, with extra support and counseling for both residents and families, and a resident council meets to have their say about things. Kitchen aide service is available, and they're always open to hiring kitchen aides or cooks at an hourly wage, with all shifts open, so there's work there if someone's interested, and everything from basic help with daily activities to medication management and housekeeping gets covered. Rim Country participates in Medicare and Medicaid, and it's run by a for-profit corporation that's worked with the VA, especially for veterans needing long-term skilled nursing. You have checklists and guides to help choose the right level of care, and the place's community sits near churches, pharmacies, and stores for easy errand runs. The most recent Medicare quality review gave Rim Country Health & Rehabilitation an overall rating of 2.4 out of 5 stars with 8 noted deficiencies, and even though that's average, you'll see a real focus here on health and rehabilitation services. The property offers a range of amenities meant to keep things comfortable and as homelike as possible, with thoughtful transitions between care levels as needed for aging in place, and while the building's large, each area tries to keep a close-knit feel among neighbors. For more details about care levels, staff, services, or activities, you can always visit the facility's website.
