    Compassionate professional care; excellent therapy

I had a very positive experience - the staff were professional, compassionate, and proactive; my private, spotless room felt safe and dignified. Therapy, wound care and the in-house DaVita dialysis were top notch, meals were excellent, and leadership/case management made transitions smooth. I felt cared for and would recommend this facility for rehab or long-term recovery.

    Summary of reviews

Haven Health Sky Harbor presents as a modern, well-appointed skilled nursing and rehabilitation campus with a substantial number of positive caregiver- and facility-focused attributes. The physical plant and amenities receive frequent praise: light, private rooms with en-suite bathrooms, a clean and bright environment, a new dialysis unit, and an active activities program (Bingo, outings, haircuts, and pet visits). Many families and patients describe a warm, family-like culture and highlight strong admissions coordination, proactive case management, daily housekeeping, and conveniences such as outside-food and delivery acceptance.

Clinical strengths cited across reviews include a number of highly praised nursing staff members, capable respiratory therapy and wound-care teams, and a robust therapy department (PT/OT/speech) that supports many successful rehabilitations and discharges to home. The facility’s ability to manage higher-acuity services — including tracheostomy, ventilator support, and on-site hemodialysis — is a notable capability that attracted many referrals. Several reviewers singled out individual nurses, CNAs, and therapists for consistent bedside presence, timely medication administration, and effective family communication.

Alongside these strengths, a clear pattern of variability in care quality emerges. Multiple reviewers describe inconsistent staffing levels and coverage gaps, especially during night and weekend shifts, which are associated with delays responding to call lights and medications. There are repeated concerns about routine personal-care maintenance (bathing, turning, incontinence care) and about infection-control and sanitation practices in specific instances. Several accounts also point to medication-administration and documentation inconsistencies and to variability in therapists’ experience for specialized neurological or complex rehabilitation cases.

More serious clinical concerns appear in a subset of reviews: delays in clinical monitoring or emergency response that led to hospital transfers, and individual allegations of significant adverse events. These appear to be isolated but consequential; they underscore the importance of clarifying current clinical-monitoring protocols, staffing ratios, and escalation pathways during any family’s evaluation of the facility. Operationally, reviewers describe episodes of staff burnout and turnover, which may contribute to uneven performance across shifts, as well as occasional administrative issues around discharge paperwork, social-work availability, and billing communication.

For prospective residents and families: the facility offers many advantages — up-to-date accommodations, comprehensive therapy services, in-house dialysis, and numerous staff members who are consistently described as compassionate and effective. At the same time, families should explicitly assess operational reliability for their loved one’s specific needs: ask about night and weekend staffing levels, response-time metrics, medication-administration procedures, infection-control policies, and the facility’s experience with very high-acuity or complex medical conditions. When possible, request current staffing ratios, recent quality-survey information, and examples of how critical incidents are reviewed and addressed. This balanced review suggests that Haven Health Sky Harbor can provide high-quality rehabilitation and skilled-care for many patients, but that careful vetting is advisable when a resident requires continuous high-acuity monitoring or highly specialized nursing oversight.

    About Haven Health Sky Harbor

Haven Health Sky Harbor stands as a three-story, for-profit skilled nursing facility with space for up to 120 residents, offering all private rooms and private bathrooms with showers, so everyone gets a bit of privacy, and you don't have to worry about sharing unless you want to. The place is known for its skilled nursing and rehabilitation services, providing long-term care, assisted living support, Alzheimer's memory care, and help with joint replacement recovery, and it's also got rooms and services set up for folks who need more complex care, like advanced respiratory care, tracheostomy and ventilator support, and in-house bedside hemodialysis for dialysis patients, something not every facility can provide.

Residents can count on teams of qualified nurses, therapists, and doctors who're trained in physical therapy, occupational therapy, speech-language pathology, pain management, infectious disease, psychiatry, pulmonology, and wound care. Folks can use the in-house rehabilitation gym, exercise programs, multiple therapy gyms, and even an outdoor therapy area if they want some fresh air while they recover. For people needing behavioral or memory care, there's a dedicated unit for that, and the staff is prepared for assisted living services and behavioral therapy as well.

The building has dining rooms on each floor, so residents don't have to travel far for meals, and the kitchen does full-service dining. Laundry is handled in-house, and there are both common living spaces and quiet lounges for socializing or privacy as needed, with the grounds including open and accessible outdoor areas. The space has amenities built around comfort and convenience, with things like modern accessibility features and sprinkler systems throughout to increase safety.

The facility's got a history of strong industry recognition for its patient care, working closely with area hospitals and medical professionals and keeping a firm focus on compassionate support every day. Personalized attention stands as a priority, and the staff listens to the unique medical and daily needs of every resident, whether it's specialty infusions, infectious disease monitoring, or complex wound care. Insurance-wise, they accept Medicare and Medicaid. The entire setup is designed to maintain dignity, comfort, and the privacy of every resident, especially with the single-occupancy rooms and thoughtfully designed therapy and gathering areas. So all in all, folks here get comprehensive medical care, comfortable private spaces, plenty of ways to stay active, and attention from people who're trained to handle all sorts of health needs.
