    Kind, professional, attentive, welcoming care

I'm very pleased with the care my family member receives here. Staff are kind, professional and attentive - Tracy and the rehab team (PT/OT) have been phenomenal - management listens and addresses concerns. The facility is clean, welcoming and well-maintained, with excellent food, a cozy activities room, and a true family-like atmosphere that gives us peace of mind.

    Summary of reviews

Pleasant Valley Rehabilitation and Nursing is frequently described as a clean, well-maintained facility with a warm, family-like atmosphere. Common-area spaces are noted as comfortable and nicely furnished, and reviewers commonly mention a fresh-smelling environment and attentive housekeeping. Overall facility upkeep and the presentation of shared spaces appear to be consistent strengths.

Clinical care and therapy received strong positive comments. Rehabilitation services (PT/OT) are repeatedly characterized as skilled and dedicated, and many families reported that therapy staff made focused efforts toward resident progress. Nursing and caregiving staff are often described as friendly, caring, and attentive, with personalized checks for comfort and individualized attention contributing to a sense of safety and peace of mind for families.

Dining and activities are also areas of consistent praise. Food quality was described positively, with specific mentions of enjoyable meals. The activities program and the Activities Director were highlighted as creating an engaging daily routine. Reviewers cited social programming and a welcoming activities room as contributing to resident well-being and social engagement.

Management and social services earn mixed but mostly favorable comments: several reviewers noted helpful, professional social services and managers who listen and act on concerns. However, administrative functions show recurring operational weaknesses. Multiple reviews identified inconsistent responsiveness from administrative and finance staff, including billing or payment-processing errors and missed callbacks. These issues manifest as frustration in family communications and delays in resolution of financial or scheduling questions.

A smaller subset of reviews raised more serious concerns about staff conduct, communication tone, and how critical incidents are handled. While many families described compassionate, above-and-beyond care, others reported variable professionalism from some staff members and an insensitive interaction from clinical leadership. A few reviewers raised concerns following a resident's death and questioned aspects of safety and incident follow-up; these comments suggest gaps in clinical-incident response and family notification that merit direct inquiry during a visit.

For prospective residents and families, Pleasant Valley appears strong in rehabilitation, everyday caregiving, sanitation, activities, and dining. When evaluating the facility, families should confirm current administrative practices around billing and family communication, ask for examples of how the facility handles clinical incidents and family notifications, and observe staff interactions across multiple shifts to assess consistency of professionalism and responsiveness.

About Pleasant Valley Rehabilitation and Nursing

Pleasant Valley Rehabilitation and Nursing, also known as Pleasant Valley Living Ctr, sits on Hinson Road in Little Rock, AR, and serves as a senior living community with 97 certified beds and an average daily count of about 76 residents, and this place mainly provides short-term and long-term nursing care, memory care at The Retreat community for those with dementia or Alzheimer's, and offers on-site therapy like physical, occupational, and speech therapies, which can help folks bounce back from illness or injury or just keep steady if they're dealing with ongoing health problems, and the staff includes licensed nurses, caregivers, and therapists who give 24-hour care and work out personalized care plans for each resident. The place is managed by Alexark1 LLC and Jej Management, LLC since June 2022, and it operates under a for-profit model with ties to Southern Administrative Services. There's been a record of 22 inspection deficiencies, some linked to food safety-like getting food from approved sources and proper food handling-others dealing with pharmacy services and making sure residents don't have significant medication mistakes, and some about infection control and preventing the spread of illness. The nurse turnover rate reads higher than average at 59.6%, compared to the state's 50.9%, and the reported nurse hours per resident per day are just below the state average at 3.90 hours. The facility itself is designed to offer comfort and convenience, with amenities like private bathrooms, cable TV, kitchenettes, air conditioning, Wi-Fi, and furnished rooms, and there's housekeeping, laundry, concierge service, meal prep with options for special diets, and restaurant-style dining, and you'll find transportation to appointments and parking available. Outdoor spaces include gardens and walking paths, while indoor options range from a fitness room, a library, and an arts room to a spa room and game room, and there are social activities, fitness programs, daily events, movie nights, and resident-run clubs. The Retreat, the memory care community, stands out with specially trained staff, and a focus on helping people with memory loss live in a safe, calm place where routines are built around their needs and interests. Residents get help with bathing, dressing, transfers, and other daily living activities, along with medication management, pain care, and wound care. The facility keeps a 24-hour call system, provides regular nursing supervision, and works to support seniors with physical or mental health conditions. Services also include transportation, help moving in, and community-sponsored activities that aim to help folks feel connected and supported. The community focuses on friendship and a homelike feel, aiming for a good quality of life even while handling health needs and safety through a trained staff, making sure residents have ways to enjoy their days, indoors or out, and try to meet the needs of seniors needing different levels of care, in a place run by steady hands, but like any facility, there have been inspection issues and staff changes worth being aware of if thinking about making Pleasant Valley Rehabilitation and Nursing home.
