    Pleased resident praises caring staff

I live here and am very pleased - the staff are kind, professional and loving (Quita and Brandi truly stand out), rehab/therapy is excellent, and the rooms are large and homelike. Meals are provided, activities are engaging, and administration is responsive and supportive. I feel well cared for and would recommend this community.

    Summary of reviews

The reviews for The Blossoms at Midtown Rehab & Nursing Center present a mixed and at times polarized picture. Many families and residents praise front-line caregivers, therapy staff, and select clinical personnel for compassionate, attentive care and effective rehabilitation services. Reviewers who had positive experiences commonly cite large therapy spaces, individualized IV and skilled-procedure care, engaging activity offerings (for example Bingo and movie gatherings), and the ability of some staff to create a homelike, supportive environment. Meals and dietary accommodations are viewed favorably by a number of reviewers, and proximity to local hospitals and roomy patient accommodations are noted as practical strengths.

Counterbalancing those positives are recurring operational concerns that appear across reviews. Staffing levels and turnover were frequently described as inconsistent, with associated delays in responding to call lights and attending to personal-care needs. Multiple accounts point to communication breakdowns between families and facility staff, inconsistent nursing rounds, and gaps in clinical oversight for residents with complex needs. Medication-management processes and consent practices were a salient concern for several families; those reports include claims of adverse clinical events and subsequent investigations, and reviewers referenced regulatory actions and personnel suspensions in some cases.

Facility condition and environmental factors were another common theme. Reviewers describe maintenance shortfalls such as nonfunctional air conditioning, broken showers or furniture, stopped-up toilets, and delayed repairs. Sanitation and odor concerns, as well as pest-control issues in some units, were raised alongside reports of inconsistent housekeeping. Dining experiences were reported as variable: some families praised nutritious meals and accommodations, while others noted inconsistent meal delivery, limited evening offerings, and additional costs for food items or vending.

Management and administrative themes are mixed. Some reviewers commend specific administrators and social-work or therapy staff for strong leadership and communication; others describe inconsistent policy enforcement, limited responsiveness, and efforts to control public feedback. Safety-related themes — including falls, mobility-assistance gaps, and equipment/transfer practices — appeared in multiple accounts and suggest room for stronger protocols and staff training. Taken together, the pattern indicates that quality of experience at this facility may depend heavily on unit-level staffing, shift-to-shift coverage, and which specific staff members are on duty. Prospective residents and families should weigh the positive experiences with therapy and committed caregivers against the recurring operational and oversight concerns, and consider direct conversations with management about staffing levels, clinical oversight, infection-control measures, maintenance plans, and family communication protocols before making placement decisions.

About The Blossoms at Midtown Rehab & Nursing Center

The Blossoms at Midtown Rehab & Nursing Center offers skilled nursing and intermediate care for people who are very frail and need help from nurses every day, and they've got space for up to 154 residents in certified beds but usually see about 86 residents on any given day, and they're part of The Blossoms Nursing And Rehab Center group with a few locations in the area. They often take care of folks who need short-term rehab services, like when someone leaves the hospital but isn't quite ready for home, and their Rehab-to-Home program helps residents get back to daily routines. They try to create a place where people can stay with dignity and respect whether they're staying a short or long time, and you find a wide variety of health care services with a focus on personalized, patient-centered care, paying attention to both physical needs and emotional wellbeing, and there are also recreational activities and amenities meant to help residents enjoy life as much as possible.

The nurse staffing averages about 3.75 hours for each resident each day, which falls below the state average of 4.1 hours, and the nurse turnover rate is pretty high at 66.0%, compared to the state average of 50.9%. The facility's inspection history shows they had 26 documented deficiencies, including ones about infection control-at least one infection-related deficiency was noted in a recent May 8, 2024 inspection, where nine total deficiencies were found-and there are also issues related to food, like not always getting food from approved sources or making sure menus meet all the nutritional needs, and other deficiencies have affected residents' quality of life and care. It's clear that The Blossoms at Midtown focuses on building a personal relationship between caregivers and residents, with a disciplined and compassionate approach, but families should know about the reported staffing and inspection details when considering it.
