    Warm, family-like facility with care

I placed my mom here and I'm very pleased with the caring, professional staff - friendly, attentive CNAs and nurses and a compassionate therapy team who really listen. The facility is clean and odor-free, feels warm and family-like, and offers lots of activities, music, church and events; visitors are welcome. Food and rehab have been good, staff resolve issues promptly, and I would recommend this place to family.

    Summary of reviews

The review set describes a facility with clear strengths in day-to-day caregiving, rehabilitation services, and resident engagement, paired with recurring operational weaknesses. Positive accounts emphasize caring, attentive nurses and CNAs, a competent therapy team that supports recovery, and an activities program that includes spiritual services, outdoor time, special events, and seasonal decorating. Commonly cited environmental strengths include a pleasant lobby, neat grounds, spotless hallways in many areas, and a generally welcoming, home-like atmosphere that families and residents find comforting.

At the same time, a consistent theme is variability in staffing and management that affects care consistency. Several reviewers described delays responding to call lights and assistance requests and raised concerns about CNA staffing levels, training, and compensation. Those operational gaps are linked by reviewers to slower assistance for residents with mobility or communication needs and, in some cases, to gaps in clinical monitoring and escalation (for example, missed signs of infection leading to hospital readmission). These concerns point to systemic issues in workforce capacity, clinical oversight, and continuity of care rather than isolated interpersonal problems.

Feedback on dining and housekeeping is mixed. Many families praised cleanliness, neutral-to-pleasant odors in common areas, and periodic improvements in food and resident nutrition; others described sanitation and pest-control concerns in specific areas and inconsistent meal quality. Physical-facility limitations were noted as well: parts of the building are older, some entries involve stairs, and certain bathrooms were described as not fully accessible, suggesting attention is needed to accessibility and long-term maintenance in some units.

Management and communication present a split picture. Multiple reviewers complimented friendly, helpful front-desk staff and described admissions and discharge as smooth experiences; others criticized administrative responsiveness, resource allocation, and continuity when agency staff are used. Family communication and advocacy experiences vary: some families felt listened to and involved in care planning, while others experienced difficulty getting timely attention to concerns. Prospective residents and families should weigh the facilityโ€™s evident strengths in therapeutic programming and staff compassion against its operational risksโ€”particularly staffing consistency, clinical oversight, and localized cleanliness/accessibility issuesโ€”and ask targeted questions about current staffing ratios, CNA training, call-response times, infection-prevention protocols, facility maintenance plans, and recent inspection findings during a visit.

Siloam Springs Nursing & Rehab, sometimes called Shiloh Nursing & Rehab, sits in Siloam Springs, AR, where it offers both housing and care for seniors dealing with physical or mental health conditions, and you'll find The Springs of Siloam Springs health and rehabilitation center as part of what they do there, which is good for folks who need either short-term or long-term skilled nursing care. The staff provides 24-hour nursing and personal care services, so there's always someone available if a resident needs help, and they accept a broad range of insurance, like Ambetter, Arkansas and Oklahoma Medicaid, and plans like Medicare, Humana, United Healthcare, or Blue Cross and Blue Shield, making it easier for people to get admitted, often right from the hospital without any long waits, and since the admissions operate 24/7, there really isn't a time when someone can't move in if that's what's needed. The facility focuses on helping people get stronger and supporting them as they heal, with experienced clinical professionals who listen closely to residents' needs and make sure that everyone, no matter what they're going through, gets treated with respect and love. Recreational programs and home-like amenities help residents find some enjoyment day to day, and there's a gentle, warm, and peaceful environment designed to promote healing and make the place a little more joyful and calm than a regular care setting, plus the leadership and a lot of the staff have been there for decades, which helps give things a steady, reliable feeling, and with skilled nursing, rehabilitation, and a holistic plan of care that's tailored just for each resident, they really try to put the person first, keeping things fulfilling, comforting, and safe no matter how long someone stays.
