    Upbeat staff, spotless facility, happy

I was very satisfied with my visit. The staff were wonderful-upbeat, knowledgeable, and they treated residents like family, often going above and beyond. The newer facility is spotless, well-organized and beautifully landscaped; rooms are bright, cheerful, nicely set up with TVs and mostly semi-private options (private rooms available). The short-term rehab hall seemed especially upbeat and residents appeared happy.

    Summary of reviews

Shiloh Nursing and Rehab presents as a relatively new, well-organized facility with a bright, cheerful interior and attractive grounds. Physical features that reviewers consistently noted include clean, nicely arranged rooms, good lighting and color schemes, in-room TVs, and curtain separation in semi-private rooms. The campus also includes a dedicated short-term rehabilitation hall, which may suit families seeking focused rehab services.

Staff impressions are mixed but polarized. Many descriptions emphasize compassionate, upbeat, and knowledgeable staff who establish family-like relationships and who at times "go above and beyond." These accounts describe staff–resident interactions as warm and attentive. Contrasting accounts raise concerns about inconsistent clinical care and responsiveness: some reviews described unhelpful interactions, delays in personal-care assistance, and at least one account linking delayed assistance to a subsequent urinary infection. Together these comments suggest variability in caregiving consistency and clinical oversight rather than uniformly poor or uniformly excellent care.

Dining and programming appear to be weaker operational areas. Several reviewers said meals did not resemble home-style cooking and were generally not well liked; overall dining quality and menu appeal were described as inconsistent. Activity offerings were characterized as limited, and the facility’s capacity to support residents with progressive dementia is constrained. Those needs may not be well met on-site given the limited dementia-care capacity reported.

Operational themes to note for prospective families include housekeeping and sanitation variability, intermittent lapses in responsiveness to toileting and personal-care needs, and uneven clinical oversight. The facility’s strengths — contemporary physical environment, landscaped grounds, and a dedicated short-term rehab unit staffed by many engaged caregivers — may make it a good fit for short-term rehab or residents who prioritize facility amenities. Families seeking long-term memory-care services or consistently high dining and activity programming should verify current staffing levels, dementia-care availability, housekeeping protocols, and menus before deciding. Additionally, the facility’s location may be farther from some families, which could affect visitation and care coordination.

    About Shiloh Nursing and Rehab

Shiloh Nursing & Rehab, LLC sits over on 1092 Stultz Rd. in Springdale, AR, and is a licensed nursing facility with both long-term and short-term health care services for older adults and those needing rehabilitation. The facility provides skilled nursing care, including 12 to 16 hours of regular nursing care and 24-hour supervision, so residents always have someone nearby, and assistance is available for things like bathing, getting dressed, medication management, or even just getting around when days are rough. The medical staff and therapists work together to create treatment plans for each person, aiming to help them get better, stay healthy, and, when possible, return home. The care here keeps a focus on dignity and well-being; people can expect a homey and welcoming atmosphere, with both private and semi-private furnished rooms, cable TV, telephones, Wi-Fi, air conditioning, and private bathrooms.

Folks have access to a state-of-the-art rehab center, where there are special programs like music therapy and horticultural therapy, and a therapy pool for rehabilitation exercises. The facility encourages independence but also steps in when help is needed, and every day there's a full activities calendar-there are movie nights, fitness classes, organized games, and even an arts room, game room, movie theater, and fitness room on site. Residents can spend time in outdoor spaces and walking paths, while community events and even resident-run activities keep social connections alive. For meals, the dining room offers restaurant-style service with special diet meal prep managed by a professional chef.

The facility has 140 beds, and as of June 2025, 36 are available, with care types that range from surgical recovery to therapeutic and supportive rehabilitation. Both Medicaid and Medicare are accepted, and families can rely on housekeeping, laundry, moving help, and support services as needed. Shiloh Nursing & Rehab offers both a family council and a residents' council, so people can voice concerns and help improve things for everyone. The tranquil setting remains quiet and aims to make everyone feel not only safe but comfortable and at home from the first day. The for-profit Limited Liability Company that owns the place keeps the facility up as a certified nursing home, and while it's not a hospital, it operates under the same kind of health care organization standards, focusing on person-centered care in a professional and caring environment.
