    Clean rooms, excellent caring staff

I'm very pleased with my mother's stay - the facility is clean, welcoming, and the rooms are spotless with beds made and thorough reports. The inpatient skilled services and communication were excellent; staff (CNAs/nurses and administration) were patient, kind, and informative, making it a solid choice for short- or long-term stays. I'm grateful for the caring, professional team and would recommend this place.

    Summary of reviews

The reviews for Bailey Creek Health & Rehab are strongly mixed, with a clear pattern of polarized experiences. Many families and residents praise individual caregivers — particularly CNAs, nurses, and hospice staff — for kindness, patience, and detailed care reporting. These positive accounts emphasize clean, well-arranged rooms, helpful guidance around benefits such as Medicaid, and effective short-term skilled therapy for some patients. The facility also carries a Medicare 5‑star rating, which several reviewers cited as reassuring.

At the same time, a number of reviewers describe operational and consistency problems that materially affect resident experience. The most frequent themes are slow or inconsistent responsiveness from staff, delayed toileting and incontinence care, and variability in staff tone and communication. Understaffing and high turnover are referenced as contributing factors; families described long wait times for assistance, delayed medication administration, and inconsistent assistance with mobility. These patterns raise concerns about timely attention to residents' basic needs and about continuity of care over longer stays.

Dining and therapy experiences are similarly inconsistent. Multiple reviewers described cold or poorly timed meals and issues with meal-service continuity, while others said food was satisfactory. Therapy and rehabilitation receive mixed assessments: some families recommend the facility for short-term inpatient skilled services, whereas others felt rehabilitation was inadequate or poorly coordinated during transitions of care.

The physical plant and environment are another area of contrast. The building is characterized as older and in need of remodeling, with small rooms, worn fixtures, dim lighting, cluttered corridors, and specific maintenance issues. Several reviewers nevertheless noted spotlessly maintained resident rooms and positive odor control in some areas, indicating variability across units or shifts. Cleanliness and sanitation appear to be inconsistent rather than uniformly deficient.

Management, billing, and communication practices drew repeated critical comments. While some administrators and admissions staff were described as helpful, others — particularly in accounting — were described as insensitive or difficult to engage. Families also cited poor phone responsiveness and a sense that financial considerations sometimes influenced discharge timing. A few reviewers raised serious concerns following adverse outcomes after discharge; these are individual, high-severity accounts that warrant careful follow-up and review by prospective families.

Overall, Bailey Creek demonstrates strengths in individual caregiver compassion, short-term skilled services, and certain aspects of room-level cleanliness and documentation. However, persistent operational weaknesses — especially inconsistent responsiveness, staffing instability, meal-service reliability, maintenance needs, and gaps in discharge planning — recur across reviews. Prospective residents and family members should arrange an in-person tour, observe staffing levels and mealtime service, review recent care-plan documentation and discharge policies, ask about staff turnover and after-hours response procedures, and confirm how the facility coordinates with hospital or hospice partners for transitions of care.

About Bailey Creek Health & Rehab

Bailey Creek Health & Rehab sits in Texarkana, Arkansas, as an average-sized, licensed nursing home that covers skilled nursing, memory care, short-term rehab, and long-term care for seniors who need different support options, and while there's no Resident or Family Council, they do accept Medicare and Medicaid, which can help a lot for people watching their budgets, and the place keeps up with strong inspection grades, earning an impressive A- on facility inspections in recent years, even if their overall grade sits at a C, and nurse quality got a B-minus, which means the nursing staff is solid but not the very top, and there's a focus on things that matter to both health and daily life, like registered staff, wound care, occupational therapy, medication help, transportation, and specialized personal care for needs like bathing, dressing, or even just help getting ready for the day, since some folks need routine and steady hands. The facility's equipped for safety and comfort, with handicap features, a sprinkler system, guest parking, cable TV, as well as areas for dining, a fitness center, salon/barbershop, activities room, kitchens or kitchenettes, and washers and dryers, which makes it feel more homelike, they handle cleaning and maintenance, and even make laundry and dry cleaning easy, so people don't have to fuss about chores or the details of keeping things tidy, and for keeping life active, there are organized activity programs, arts and crafts, educational classes, social gatherings, and ongoing health and wellness events to help residents stay engaged, and if someone is dealing with memory issues, Bailey Creek does offer specialized dementia and Alzheimer's care, with trained staff and routines set up to promote safety and comfort which is good for peace of mind, and there are health services covering things like podiatry, nurses, and wound care, showing they're set up to address different health needs as they come up, and they make comprehensive, personal care plans that let each resident get just the right amount of help, whether for a short rehab stay or long-term support, including respite care for families that need a break. Bailey Creek Health & Rehab is open about their record, encourages visits or virtual tours for families wanting to look around, and they link to social media platforms, being straightforward about what they do, and the kind of teamwork they have with families to help ensure good care for residents from physical to emotional needs, so folks can expect compassionate, professional support that doesn't forget about the small comforts and everyday routines.
