    Clean facility but unsafe care

I have mixed feelings. The facility is extremely clean, odor-free, well-managed and many nurses/CNAs are friendly, accommodating and went above and beyond - admission was easy and communication can be transparent. But I experienced serious lapses: long call-button delays, unresponsive or disrespectful staff, poor or nonexistent physical therapy, unsafe handling (Hoyer lift not used), missed/incorrect treatments, theft of personal items and failures to inform family about serious events - issues that required Ombudsman involvement. Overall I give it 3 stars: nice environment and caring people at times, but unacceptable clinical and administrative lapses.

    Summary review

Overall sentiment: The reviews for Pacific Care Nursing Center are highly mixed, with a strong polarization between families who experienced compassionate, professional care and those who reported serious clinical and administrative failures. Many reviewers praise specific staff members and cite clear examples of dedicated, skilled, and compassionate caregiving. At the same time, an overlapping set of reviews describes alarming lapses in clinical monitoring, communication, and safety that resulted in rapid health decline, readmissions to higher levels of care, or death. The combined picture is of a facility that can deliver excellent care under certain circumstances but also displays significant inconsistency and risk.

Care quality and clinical concerns: Multiple reviewers describe high-quality nursing and therapeutic care—patients improving, coming off ventilators, and families feeling comfortable and well-informed. However, a substantial subset of reviews details clinical negligence: medication dosing errors, failure to monitor blood pressure, inadequate management of respiratory secretions leading to mucus build-up, delays in escalating to the hospital, mishandling of colostomy care, and unresponsiveness to urgent needs. These clinically serious reports sometimes culminate in ICU admissions or death. Physical therapy is a recurring concern: some families report beneficial therapy and progress, while many others state that PT was inconsistent, poorly followed-through, or not started for long periods (one review claimed no PT started for four years). Safety practices are also questioned—specifically non-use of Hoyer lifts and manual handling of non-ambulatory residents, which families described as a direct safety risk.

Staffing, training, and behavior: Praise centers on compassionate CNAs, nurses, and named staff who 'go the extra mile.' Multiple reviews highlight polite, friendly, and dedicated caregivers and cite rapid, transparent communication. Yet, short staffing and staff turnover are repeatedly mentioned and appear to contribute to inconsistent care. Several reviews describe rude or untrained nursing staff, long call-button response times, and some CNAs characterized as lazy. Administrative issues are serious in a number of accounts: disrespectful behavior, threats to call police, failing to notify next of kin about deaths, sending inappropriate surveys after deaths, and even alleged HIPAA violations. Registry staff treatment and morale concerns are reported, suggesting systemic staffing/management problems in some units or shifts.

Facilities, cleanliness, dining and activities: The facility itself is frequently described as clean, odor-free, and well-maintained, with thorough daily cleaning and a pleasant environment. Several reviewers mention a variety of engaging activities and an overall low-stress atmosphere for residents. Dining receives mixed remarks: some families appreciate the food and kitchen staff, while others find the meals bland and in need of more flavor. Overall, the physical environment and housekeeping appear to be consistent strengths.

Communication and family experience: Communication is another polarized area. Numerous families commend staff for honest, transparent communication and quick responses, praising those who kept them informed and comfortable with their relative's care. Conversely, other families report minimal nurse communication, failures to contact next-of-kin during critical events, and even insensitive administrative actions following resident deaths. One reviewer reported Ombudsman involvement due to safety and handling concerns, indicating that external escalation has occurred in at least some cases.

Notable patterns and takeaway: The dominant pattern across reviews is variability—some wings, shifts, or particular staff members deliver excellent, attentive care and meaningful clinical improvement, while other times critical lapses in monitoring, escalation, and respectful communication occur. Compliments are consistent around cleanliness, particular compassionate staff, and stories of major patient recovery. Criticisms cluster around clinical neglect, safety violations, poor escalation during emergencies, handling of sensitive events and property, and staffing shortages or morale issues.

Implications for families and oversight: Based on the reviews, prospective residents and families should be aware that experiences at Pacific Care Nursing Center may differ widely depending on shift, unit, or staff on duty. Key areas to monitor if choosing this facility include medication administration practices, respiratory care and suctioning, timely response to call signals, proper use of lifting equipment, prompt escalation to hospitals when needed, and clear communication about critical events. Families who reported problems sought Ombudsman involvement or escalated concerns, suggesting there are paths for recourse when serious issues arise. Positive reports indicate the facility has the capability to provide high-quality, compassionate care—but the inconsistency and the number of serious negative reports represent a meaningful risk that families should consider and monitor closely.

    About Pacific Care Nursing Center

Pacific Care Nursing Center is a medium-sized, for-profit skilled nursing facility with a capacity of 99 beds. Positioned in Long Beach, California, it accepts both Medicare and Medicaid, providing residents with access to a range of funding options for short-term rehabilitation and long-term residential care. The center operates independently and is not part of a larger continuing care retirement community. Its ownership is classified as corporate, which frequently suggests a standardized approach to care, policies, and facility management.

The care home has received an overall rating of average for both its short-term rehabilitation and long-term care services. For short-term rehabilitation, residents at Pacific Care Nursing Center benefit from a focus on recovery after hospitalization for events such as stroke, heart attack, injury, or infection. Approximately 40.5% of residents are able to return home after discharge, indicating a moderate rate of successful rehabilitation. Nurse staffing levels reflect approximately 5 hours and 36 minutes of direct care per resident each day, with weekend staffing rated as consistent with weekday levels. Short-term patients are monitored for key safety and health measures, with 1.2% experiencing major falls, 9.3% having infections necessitating hospitalization, and 11.8% requiring emergency room visits during their stay.

The facility maintains a universal influenza vaccination rate among its residents, reflecting a proactive approach to preventative healthcare. Emergency room visit rates among long-term residents are relatively low at 0.8 visits per 1,000 patient days, suggesting a degree of stability and effective management of day-to-day health needs.

Inspection reports from state and federal agencies serve as a measure of the home’s ongoing commitment to safety and quality improvement. The center’s recent inspections highlight both resolved concerns and ongoing efforts to enhance resident care. Instances requiring corrective action have included areas such as accident hazard prevention, setting up quality assessment groups, pain management, timely communication with doctors and family members, abuse reporting, honoring resident preferences, and ensuring doctor’s orders on admission. Additional attention has been given to specialized care areas including pressure ulcer treatment, colostomy and dialysis care, proper bed rail use, chart reviews by pharmacists, infection control, and nutrition management. While some violations were flagged as having the potential for low to moderate harm, Pacific Care Nursing Center addressed cited deficiencies and implemented necessary changes within reported time frames.

In reviewing recent performance, the facility’s track record includes the payment of fines for regulatory non-compliance over the past three years, amounting to a cumulative sum. Despite these challenges, the center continues to participate in ongoing assessment and enhancement of the quality of care delivered. Pacific Care Nursing Center demonstrates a commitment to providing a supportive, clinically attentive environment for seniors who need either temporary rehabilitation after a hospital stay or long-term residential care, emphasizing nursing support, resident safety, and recovery potential.
