    Compassionate staff, excellent rehab care

I was very pleased with the compassionate, attentive staff and consistently clean facility. Therapy and rehabilitation were excellent, and nurses, CNAs and reception were helpful and well informed. Staff went out of their way to keep my mom comfortable and addressed our needs promptly. I would return for rehab and recommend this center.

    Summary of reviews

Reviewer feedback for Vernon Healthcare Center is mixed, with clear strengths in direct resident care and rehabilitation offset by recurring operational and communication issues. Many families and residents praised individual caregivers โ€” nurses, CNAs, therapists, and reception staff โ€” for compassionate, attentive interactions and effective therapy outcomes. The facilityโ€™s rehabilitation department and physical-therapy services received consistently positive comments, and volunteer involvement and monthly physician rounds were noted as beneficial to resident engagement and oversight.

At the same time, reviewers frequently described variability in staff behavior and facility processes. Common operational weaknesses include slow response times to assistance requests, uneven sanitation and odor control in some areas, and inconsistent meal-service continuity. Several accounts imply gaps in medication-management procedures, including unfamiliarity with residentsโ€™ medications and lapses in secure medication-handling. Families also raised concerns about safety practices related to transfers and fall prevention, and some noted instances where clinical escalation or timely medical outreach appeared limited.

Communication and management practices are another area of divergence. Some families reported helpful, solution-oriented administration and positive problem resolution, while others experienced limited transparency, poor notification about transfers or clinical changes, and a perception that administrative attention prioritized billing and insurance issues over family engagement. Social-services follow-through and coordination of care were described as inconsistent, and there were isolated mentions of privacy and documentation process concerns.

Dining and facility amenities receive mixed remarks: several reviewers described acceptable, hospital-style food and generally clean common areas, whereas others noted inconsistent cleanliness of resident rooms, missing personal items, and occasional odor concerns. Activities and volunteer presence were seen as positive contributors to resident well-being when they were available, and family involvement was frequently cited as improving attention to individual residents.

Overall, the pattern in the reviews suggests a facility with competent, caring frontline staff and strong rehabilitation capabilities but with systemic operational weaknesses that affect consistency of care. Prospective residents and families may wish to assess current staffing levels, medication-management protocols, communication practices, sanitation procedures, and social-services responsiveness during a visit and to ask specific questions about emergency response times and family-notification policies.

    About Vernon Healthcare Center

Vernon Healthcare Center is a medium-sized skilled nursing facility located in Los Angeles, offering both short-term rehabilitation and long-term care services to its residents. With a capacity of 99 beds and operating under for-profit LLC ownership, the center participates in both Medicare and Medicaid programs, making it accessible to a broad range of individuals needing various levels of nursing care. It is not part of a continuing care retirement community but provides vital rehabilitation support for patients recuperating from hospital stays due to conditions such as stroke, heart attack, infection, or accidental injury.

The facility provides a mix of medical and non-medical assistance to support residents in their daily routines. Residents receive an average of three hours and fifty-one minutes of nurse staffing per day, a figure that applies consistently across both weekdays and weekends, reflecting excellent weekend-to-weekday staffing consistency. For residents in need of short-term rehabilitation, the centerโ€™s services are designed to support recovery and a safe return home. However, only about 38.6% of residents are able to return home after their discharge. The occurrence of emergency room visits during short stays stands at 9.9%, with serious infections requiring hospitalization affecting 9.0% of short-stay residents. Notably, the rate of falls resulting in major injury among short-term residents is 1.5%.

In its long-term care offering, the center maintains a focus on residentsโ€™ independence and quality of life. About 90.3% of long-term residents retain the ability to independently perform daily activities such as moving, eating, and using the bathroom. The nursing team ensures that all residents are vaccinated for influenza, achieving a 100% flu vaccination rate. Emergency room visits for long-term residents average 0.7 visits per 1,000 patient days, indicating a relatively low rate of acute incidents.

Vernon Healthcare Center is subject to regular inspections by health authorities focused on health, fire, and safety standards. The most recent inspection cycles have identified areas needing improvement, including the prevention of abuse, neglect, and theft, and ensuring all residents receive accurate assessments and comprehensive care planning. Additionally, the facility has faced issues related to the adequacy of accident prevention, pharmaceutical services, safe food handling, the labeling and storage of medications, sufficient room size, and honoring each residentโ€™s right to dignity and personal possessions. These inspection findings have generally been resolved within a short timeframe.

Within recent years, the facility has incurred monetary penalties relating to compliance with regulatory standards, reflecting ongoing attention to improving the quality of care and living conditions for its residents. Services offered cater to a diverse population, with access to funding through both Medicare and Medicaid, aiming to support residents and their families though various stages of recovery and ongoing care. Vernon Healthcare Center emphasizes safe and compassionate nursing care, balancing clinical oversight with a commitment to supporting residentsโ€™ independence, health, and well-being.
