    Compassionate staff, inconsistent care, warning

I had a very mixed experience. Many nurses, therapists and CNAs (Victor, Kimberly and others) were compassionate, attentive and helped my relative regain strength in a clean, welcoming rehab environment with activities - but care was inconsistent: delays in procedures, late/inconsistent meds, ignored/unsafe symptoms, occasional rough or rude staff, and even a missing resident. Administration and communication were poor (undisclosed/retroactive charges, pressure to readmit, no doctor contact), some areas/outdoor space were dirty, food and A/C were problems - I'd only recommend with heavy caution: verify billing, medical oversight and staffing before admitting.

    Summary review

Overall sentiment is deeply mixed, with some reviewers reporting exceptional, compassionate care and others describing serious lapses in safety, cleanliness, and management. A large portion of comments praise individual staff members—nurses, CNAs, therapists, and select directors—who are described as caring, respectful, and willing to go beyond their roles. Several families shared long-term positive experiences, noting that doctors and nurses saved lives, helped patients recover, and created a welcoming, dignified, family-like environment. Rehab and physical therapy services receive positive mentions, and many reviewers appreciated daily housekeeping, routine bed changes, and a generally clean facility on certain visits. Amenities such as an outdoor patio, activities (bingo, crafts, church services), good email communication, and staff who accommodate special requests contributed to strong endorsements in many reviews.

However, these positive impressions coexist with frequent, serious complaints. The most common negative themes are inconsistent care and neglect: reviewers reported slow or no responses to call lights, residents left in filth overnight, inadequate personal hygiene (old sheets, missed bed baths), and staff distraction (phones). Several accounts describe neglect of concerning medical signs (including alleged ignored stroke symptoms), delayed coordination for medical procedures, inconsistent medication timing, and instances where weekend or off-shift doctors were dismissive. Safety concerns escalate in some reviews to reports of missing residents, patients left unattended in hallways, rough handling that caused injuries, and feelings that care was unsafe. These issues point to episodic failures in supervision, clinical oversight, and on-shift staffing consistency.

Dining and environment are another split area. Multiple reviewers complained about bland, cold, or stale food and bread, and some patients experienced ignored special diets leading to hunger. Others reported customizable meals and positive meal experiences. The facility environment was described as clean and well-maintained by some, while others found it depressing, unclean, or excessively hot due to reported lack of air conditioning—visitors and residents noted discomfort from heat. Outdoor access and patio use appear inconsistent: some praise pleasant outdoor areas, while others say outdoor time is restricted.

Management, communication, and billing were recurring concerns. Many reviewers noted poor communication channels: difficulty reaching doctors/administration, no callbacks, phone system issues, or calls passed between operators. There are also allegations of poor management oversight—lack of accountability, staff left unsupervised, and pressure on families to readmit residents. Financial transparency was questioned by multiple reviewers who reported retroactive or undisclosed charges that caused stress. Conversely, some families found directors communicative and administrative leadership outstanding, showing a sharp contrast in experience that suggests variability by shift, department, or time period.

Patterns indicate significant inconsistency: initial admissions or first visits often received praise for cleanliness and responsiveness, while follow-up or later stays were more likely to be criticized for decline in care quality. Positive reviews frequently name specific staff members as exemplary, which underscores that individual caregivers deliver high-quality care even when systemic problems exist. Negative reviews concentrate on systemic issues—staffing shortages, management lapses, inconsistent medication and medical oversight, and billing transparency problems.

In summary, York Healthcare & Wellness Centre appears to provide very good care for some residents, particularly in rehab, therapy, and when strong individual staff are assigned. At the same time, there are multiple reports of serious lapses in safety, neglect, cleanliness, food quality, communication, and billing practices. Prospective residents and families should weigh the strong testimonials about compassionate staff and effective clinical recoveries against recurring reports of inconsistent care, safety incidents, and administrative problems. If considering York, ask targeted questions about staffing levels, weekend/after-hours medical coverage, medication administration protocols, incident reporting procedures, billing transparency, food/diet accommodations, current air conditioning status, and how the facility addresses reported incidents of neglect or missed care. Also consider visiting multiple times and requesting to meet shift supervisors and the director of nursing to evaluate consistency and oversight before making placement decisions.

About York Healthcare & Wellness Centre

York Healthcare & Wellness Centre sits in Los Angeles, right in the heart of Highland Park, and it's got 107 certified beds, with about 92 residents each day. The centre is licensed by the California Department of Health Care Services and takes both Medicaid and Medicare. Folks here can get nursing care, physical therapy, occupational therapy, speech therapy, and medical social services, and there's around-the-clock care with help for both short-term rehab and long-term stays, plus plenty of attention for each person. The rooms are big, recently remodeled, and have private bedrooms and personal flat-screen TVs. There's complimentary valet parking, restaurant-style dining, and menus that the dietician approves, plus special therapeutic diets.

You'll find things to do like arts and crafts, live music, gardening, and exercise programs, and there are trips out to restaurants, malls, and museums for those who want to get out a bit. Services also cover medication management, wound care, orthopedic and dialysis care, and you'll see a team that tries to keep things peaceful and compassionate for everyone. The spaces are bright and cheerful, with décor and amenities meant to feel comfortable and clean.

Now, York Healthcare & Wellness Centre is a for-profit limited liability company run by Shlomo Rechnitz since June 2012. It gets a slightly below-average rating from CMS. It's had 56 deficiencies on inspection reports, including five infection control issues and two recent complaint reports, and there have been citations over pressure ulcer care and residents' rights about coming back after hospital stays. Nurse turnover is high at nearly 50%, above the state average, but nurse staffing hours are about the same as other places in California. The centre does offer skilled rehab, treatment plans for mobility and strength, and a mix of activities for both mental and spiritual well-being. Overall, it's got a close-knit team trying to cover a wide range of elder care needs in a setting that feels open, relaxed, and as comfortable as possible.
