    Outstanding rehab caring attentive staff

I was very satisfied with my stay - the staff were caring, friendly and consistently attentive, and administration was responsive and professional. Therapy (PT/OT) and wound care were outstanding, the facility was clean and well managed, and my post-surgery transition and recovery went smoothly. Meals, activities and clear visitation procedures made it feel home-like; I'd happily recommend their rehab care.

    Summary of reviews

San Bruno Skilled Nursing is consistently described as a facility with strong clinical capabilities and an experienced rehabilitation program. Reviewers frequently highlight the physical and occupational therapy teams as a primary strength, and wound-care services are repeatedly characterized as professional and knowledgeable. Nursing leadership and many direct-care staff, including CNAs and nurses, receive positive mention for compassionate, attentive care; several accounts describe smooth hospital-to-facility transitions and effective coordination when standard processes are followed.

Dining and nutrition receive mixed feedback. The facility operates an on-site kitchen and multiple reviews praise healthy, well-prepared meals and flexible menu options. At the same time, other accounts describe inconsistent food quality and express concern about the suitability of meals for residents with complex dietary needs such as diabetes. Prospective families should confirm current dietary accommodations and clinical-diet protocols during evaluation.

Activities and ancillary services are another relative strength. The activities team, daily programming, laundry service, and supportive medical social work are noted as contributing to a family-like atmosphere and to resident engagement. Visiting policies are reported as flexible, and staff are often described as helpful with family needs and transitions. Management is commonly portrayed as proactive and responsive, with named administrators drawing praise for leadership and follow-up on issues.

Operational and facility-level issues appear repeatedly and warrant attention. Noise, crowding, and limited privacy in some rooms are frequent themes; physical infrastructure items such as lighting, bathroom exhaust, the courtyard, and gym space have been identified as needing upgrade or better scheduling. There are consistent mentions of delays in call-light response and occasional lapses in basic-care timeliness, which suggest staffing or workflow challenges at certain times. Importantly, there are instances of discharge coordination failures leading to gaps in medication continuity and broader concerns about medication-management processes; families should specifically confirm medication-reconciliation and discharge protocols prior to placement. Communication challenges, including language barriers and variable staff professionalism or tone, are also noted, and there are a small number of serious allegations of staff misconduct that families may want to investigate further.

Overall, San Bruno Skilled Nursing appears well suited for patients requiring focused rehabilitation and skilled wound care, supported by many reviewers who experienced positive outcomes and attentive staff. At the same time, prospective residents and family members should ask targeted questions about noise and room occupancy, current renovation impacts on outdoor access, meal planning for clinical diets, call-response expectations, and discharge/medication reconciliation procedures to ensure the facility’s operational conditions match the resident’s clinical and comfort needs.
