    Clean, attentive, family-run facility recommended

I placed my family here and was very pleased - the facility is clean, bright and family-run with owners and staff who are genuinely involved. The nurses, CNAs and therapists are attentive and caring (special thanks to David, Cindy, Wanda, Erica, Maiko, Paul, Karla, Jeanie, Anthony, Jen and Stan Stukov); 24-hour nursing, prompt responses, excellent PT/OT and helpful discharge planning made a big difference in recovery and comfort. I highly recommend this compassionate team - excellent care overall, with just a few small improvements that could make it outstanding.

    Summary of reviews

San Francisco Health Care and Rehab Inc. elicits strongly polarized impressions. Many families and former residents highlight high-quality rehabilitation outcomes, continuous nursing coverage, and a generally compassionate care culture. Positive accounts frequently single out the facility’s physical and occupational therapy teams for marked mobility improvements, as well as attentive CNAs, multilingual nursing staff, and hands-on ownership/management involvement. Several reviews also note private, roomy accommodations with en-suite bathrooms, routine housekeeping, active dietary monitoring, and useful social-work support for discharge planning.

Care and staffing present a mixed picture. Numerous reviewers praised individual caregivers, therapists, and administrators for responsiveness and clinical skill, and the facility is described by some as providing dignified, hospital-like care. At the same time, there are repeated operational concerns: inconsistent response times to call lights, variability in staff conduct and communication tone, and several accounts describing gaps in supervision and transport. Reviewers also raised concerns about medication administration and communication between clinical staff and families. These patterns suggest unevenness in day-to-day staffing reliability and in how standard procedures are executed.

Dining and therapies are likewise variable. Positive comments point to diverse meals, weight monitoring, and dietary support; strong rehabilitation services are a consistent strength in many accounts, with notable functional gains after stays. However, other reviewers describe inconsistent meal quality and delays in meal or hydration delivery. Prospective families should expect that therapy programs appear robust, while food service consistency may fluctuate.

Facility condition and operations show both strengths and weaknesses. Many reviews describe a clean, bright environment and effective daily housekeeping, while a number of other reviews identify sanitation and pest-control concerns, dated furnishings, torn linens, and ongoing renovation needs. The building is described as older in places; management appears to be addressing maintenance in some areas but physical upgrades are incomplete in others. Communication systems — including phone and admissions processes — were singled out for unreliability by multiple reviewers, which can complicate family contact and coordination of care.

Management and governance prompts mixed feedback. The family-owned, hands-on leadership is praised by several families for accessibility and proactive administrative responses. Conversely, there are serious allegations concerning billing, privacy questions, and administrative irregularities that warrant attention; prospective families should review licensure records and any regulatory actions as part of intake. There are also isolated claims about review manipulation and operational transparency that further justify direct verification.

Overall, the facility demonstrates clear clinical strengths—notably rehabilitation services and dedicated caregivers—alongside recurring operational weaknesses in responsiveness, sanitation controls, and administrative reliability. Given these contrasting patterns, prospective residents and families should arrange an on-site visit during active hours, observe nurse and therapy interactions, review recent inspection reports, ask about staffing ratios, pest-control schedules, medication-management protocols, and clarify billing/privacy procedures before admission.

About San Francisco Health Care and Rehab Inc.

San Francisco Health Care and Rehab Inc. sits at 1477 Grove Street in San Francisco, California and's been open since at least September 2010, running as a skilled nursing facility, officially recognized by the state and always in active standing, and the place runs under Svetlana Stukov as the Chief Executive Officer and Chief Financial Officer, with Mehran Jamali and Edil Basa helping as directors, and Edil Basa also serves as the Secretary, and honestly, they keep a close watch on legal compliance, keeping steady through the rules of the California Secretary of State. The place's got a mix of 201 to 500 employees and can serve up to 168 residents in Medicare and Medicaid certified beds, but the most notable thing about this address is how many services line up in rows for folks who need long-term care, post-acute care, short-term rehab, or even daily help, offering round-the-clock skilled nursing, rehabilitation, and room and board for residents of all levels of need. There are nurses, aides, occupational therapists, physical therapists, and even speech-language pathologists on staff to help with everything from simple bathing to complicated swallowing disorders, so families don't have to worry about little things like medication errors because they've got a computerized pharmacy system, in addition to full digital X-ray services, laboratory services for clinical trials, and even visiting dentists and podiatrists who look after feet and teeth right there, and no matter where you are in the building, staff wander about who speak Cantonese, Mandarin, Tagalog, Spanish, Russian, and plenty of other languages, so residents can always feel like someone hears them. The folks here also make sure to provide memory care for dementia and Alzheimer's, palliative care, and even hospice and respite care for those who need it, and when a doctor orders a special diet, the kitchen's got a monthly changing menu and a private dining room for the family to celebrate. The facility cares about social life too, offering recreational activities, parties, arts and crafts, and even pet therapy, hoping to keep everyone active and engaged, and with social workers around, there's always someone to help with the practical steps for health and wellbeing. They work hard to create a calm and welcoming place where seniors can find as much independence as possible, focusing on helping each person live life with purpose, whether that means learning to walk again, keeping up with old hobbies, or just getting the help needed for day-to-day things, and all these services-right down to regular housekeeping, laundry, and linen service-track to the mission they have, which is to maintain comfort and dignity for everyone living there.
