    Caring staff, excellent rehab, recommended

I'm glad we chose Ivy Creek - the staff are caring, skilled and attentive, especially Maria, Amanda and Lana who helped with the transition. My mom recovered quickly with excellent rehab (including Mandarin/Cantonese-speaking therapists), enjoyed activities like mahjong and dim sum deliveries, and felt at home. The facility is very clean, nurses and CNAs are friendly and responsive, and front-desk staff were helpful. Grateful our family picked this place and would recommend it.

    Summary of reviews

Reviews of Ivy Creek Healthcare and Wellness Centre present a mixed but coherent picture: many families and residents praise front-line caregiving, rehabilitation results, and the facilityโ€™s atmosphere, while a set of operational and oversight concerns recurs. Positive remarks emphasize compassionate, skillful nurses and CNAs, an attentive therapy team, and a rehabilitation program that many families credited with measurable recovery. The presence of Mandarin and Cantonese-speaking therapists and culturally appropriate meal deliveries (including dim sum to rooms) is a clear strength for families seeking language-congruent care. Activity programming โ€” mahjong, volunteer singing, social opportunities and communal engagement โ€” is commonly cited as contributing to resident satisfaction and social well-being.

Staff performance is a central theme. Numerous comments describe staff as kind, helpful, and responsive, and several reviewers named individual caregivers and therapists as making transitions easier. At the same time, reviewers also describe variability in staff attitude and competence: some frontline employees are viewed as excellent while others are characterized as requiring further training in communication and bedside conduct. Language barriers were noted as an impediment in some interactions despite the facilityโ€™s multilingual rehabilitation staff.

Clinical and safety patterns are mixed. Positive reports of attentive nursing and effective therapy coexist with descriptions that suggest gaps in medication management and in physician engagement. There are mentions of medication errors and of perceived inadequate clinical oversight; these indicate potential weaknesses in medication-administration controls and in timely physician consultation. Separately, a limited number of very serious allegations appear in the review set; those accounts point to concerns about incident handling and administrative accountability. These items suggest families should inquire specifically about clinical governance, incident-reporting practices, and medication-safety protocols when evaluating the facility.

Facilities and dining generally receive favorable comments for cleanliness in common areas, well-kept grounds, and a convenient location near amenities and hospitals. However, some reviewers reported sanitation and odor concerns in particular rooms or shared bathrooms, and others described small room sizes and occasional overcrowding in certain units. Dining receives largely positive feedback for culturally familiar options and overall acceptability, though some mention variability in meal quality.

Management and family communication are other notable themes. Several families praised helpful administrative and marketing staff who eased admissions and transitions. Conversely, there are consistent reports of communication lapses โ€” for example, failures to notify families promptly after adverse events or end-of-life situations and a reliance on billing notices for formal communication in some cases. Some comments characterize organizational attitudes as profit-oriented and describe a culture in which staff fear supervisory repercussions; these perceptions link back to concerns about transparency and complaint resolution.

Overall, Ivy Creek appears to offer substantive strengths in hands-on caregiving, rehabilitation, and culturally sensitive services, anchored by many staff members who are experienced and compassionate. Prospective residents and families should balance those strengths against the operational concerns identified here: ask detailed questions about medication-safety procedures, physician availability, incident-reporting and notification policies, room sanitation practices, and staff training/linguistic capabilities. An in-person tour that inspects private-room conditions and a discussion with nursing leadership about clinical oversight will help clarify whether the facilityโ€™s day-to-day operations align with a familyโ€™s expectations for safety and communication.

About Ivy Creek Healthcare and Wellness Centre

Ivy Creek Healthcare & Wellness Centre sits in San Gabriel, CA, and has 98 certified beds for residents who need nursing home care, short-term rehab after a hospital stay, or help with everyday living. The place has been managed by Shlomo Rechnitz since June 2012, but there's no direct owner information listed and the indirect owners aren't detailed anywhere official, so it's hard to say much about that except that the company is privately held with a staff between 51 and 200 people taking care of everyone. The nursing staff provides about 3.86 hours per resident each day, which is a bit below the state average of 4.5 hours, though the nurse turnover rate at Ivy Creek stands at 21.0%, which is lower than the state's average of 40%, meaning the nurses don't leave as often as at other places. Inspectors have reported 40 total deficiencies in recent years, including four for infections, and the facility's received citations for not always handling pharmacy services properly or keeping medication error rates low enough and also for not disposing of garbage and refuse the right way in their nutrition and dietary services.

Care options at Ivy Creek include Skilled Nursing, Assisted Living, Independent Living, Memory Care, and even Adult Care Home support, along with Home Care, Home Health, Hospice, and various referrals when residents or families ask about other needs. The skilled nursing section looks after very frail residents and provides 24/7 nursing care, and they try to offer a well-rounded set of services-the team gives wound care, physical therapy, occupational therapy, and speech therapy, and there's a medical social worker around to help with social needs, too. Some people use the rehab programs to build back up and get home safely, and others stay long term.

The place tries to keep a lively feeling, with daily activities and recreation for folks who want to join in, and the staff take time to plan care tailored to each person's situation, with specialized nursing care and therapy schedules that aim to help people be as independent as they can. Staff cover wound care with updated techniques and use the newest technology available. Ivy Creek was built to be modern and wants to give a safe, pleasant setting for both short stays and longer ones, and while they say they're focused on compassion and excellent service, the inspection record and nurse hours might be worth looking into if someone is making decisions about where to stay. Amenities in the facility are arranged to help people feel at home and cared for, and skilled nursing and rehab services are the main focus, along with giving support to both the body and spirit through their different programs.
