    Clean facility, caring staff, progress

I'm very pleased with the friendly, professional staff-nurses, CNAs and rehab therapists were knowledgeable and helped my dad make strong progress with walker steps. The facility is clean, well-run, with tasty meals, strict safety protocols and responsive communication that made our visits pleasant. Overall a great, caring team; a few small tweaks would make it excellent.

    Summary of reviews

Overall impression: Pine Grove Healthcare & Wellness Centre receives frequent praise for its clinical rehabilitation services, caregiving staff, and physical environment, but also shows recurring operational and administrative weaknesses that prospective residents and families should evaluate. Strengths center on therapy-oriented care, bedside compassion, and the facility’s cleanliness and amenities. Concerning patterns involve medication controls, management communication, billing and contract handling, and several process failures around transitions and incident response.

Care quality and staff: Clinical care elements are commonly described as a strong point. Physical and occupational therapy resources, including a dedicated training room and exercise equipment, are repeatedly credited with producing measurable functional gains. RNs, CNAs and therapy teams are often characterized as knowledgeable, patient, and engaged; reviewers highlight attentive bedside care, effective wound management, and supportive aftercare coordination. At the same time, there are consistent indications of unevenness in frontline performance — examples include perceptions of rushed interactions, occasional rudeness, and specific medication-administration errors. These suggest gaps in consistency and supervisory oversight rather than a uniformly poor standard of caregiving.

Dining and activities: The facility’s activity programming and therapy-driven engagement are described positively; residents are reported to be active and upbeat, with organized group activities and rehabilitation-focused options. Many families praise meal variety and presentation, calling food fresh and well-balanced, though there are isolated but consequential concerns about meal safety and quality control that point to inconsistent kitchen processes. Prospective families should ask about recent food-safety audits and menu oversight.

Facilities and environment: Multiple reviews emphasize the facility’s cleanliness, pleasant décor, comfortable rooms, and maintained outdoor spaces. Therapy equipment and communal areas are noted as assets that support recovery. Noise and privacy levels appear to vary by location within the building, with some units described as louder or less private; visitors should tour intended rooms and common areas to assess fit.

Management and operations: Administrative performance is the area with the greatest divergence in reviewer experience. Some families describe proactive, communicative business and admissions staff who facilitate smooth transitions and timely medication updates. Others report problematic behaviors at the management level, including poor communication, billing and vendor-payment disputes, delayed or inadequate handling of personal belongings, and slow discharge processes. There are also serious, individual allegations related to end-of-life handling and post-incident communication; these are significant outliers that warrant direct inquiry with facility leadership and review of any regulatory findings or legal actions.

Notable patterns and recommendations: The facility’s core clinical and rehabilitative strengths are appealing for residents needing PT/OT and hands-on caregiving. However, the mixed administrative and process-related feedback indicates that outcomes will depend in part on unit assignment, admitting staff, and supervisory follow-through. Families touring Pine Grove should: 1) observe therapy programming and staffing levels during a typical weekday; 2) ask for documented policies on medication administration, incident response, death notification, and property management; 3) review recent inspection reports and any documentation of vendor or billing disputes; and 4) meet the specific nursing and unit management team who will oversee the resident’s care. These steps will help balance the facility’s evident clinical strengths against the operational risks some reviewers identified.

About Pine Grove Healthcare & Wellness Centre

Pine Grove Healthcare & Wellness Centre is a for-profit skilled nursing facility with 75 Medicare- and Medicaid-certified beds, and it usually cares for about 67 residents each day. Managed by Shlomo Rechnitz since June 2012, the centre has over 40 years of experience offering healthcare and rehabilitation services. It provides personalized skilled care around the clock, including in-house hospice care and a full in-house rehab unit with physical, occupational, and speech therapy. Residents work with a dedicated care coordinator, and a team of experienced nurses and therapists is available 24-7. Residents see physicians when they're admitted and about every 30 days after that. Nurse staffing averages 4.33 hours per resident per day, though there's a nurse turnover rate of 36.4%. The centre reports 51 total deficiencies in inspections, with 2 related to infection spread, and follows federal standards on infection control, resident rights, and nutrition. The centre features a clean front lobby and hallways, offers a warm environment, and holds events like a petting zoo. Amenities focus on creating home-like surroundings, while staff encourage family visits, including children and pets, following certain safety steps. Recommended visiting hours go from 8 am to 8 pm, but other hours are possible with arrangements. Leaves of absence for residents can happen with a physician's okay. Medicare can pay fully for the first 20 days, with co-insurance applying on days 21-100 if the resident qualifies. The admissions coordinator helps families navigate rules, forms, and insurance, and the Business Office Manager helps with billing or admission issues. The social services department assists with care planning, discharge, and arranging specialty services like dental, vision, podiatry, audiology, and psychiatric help. An Ombudsman serves as an outside advocate, checking on facility activities and addressing concerns. The centre also values engaging social programs and activities and tries to keep families involved in therapy and care decisions. The goal is to restore independence where possible and enhance life quality, and staff are trained to deliver both daily care and rehabilitation support.
