    Compassionate professional care and recovery

I've been in their care for over two years and I'm very satisfied - the nursing, medical and rehab teams (special thanks to Administrator David and Lourdes Zuniga) are compassionate, professional and proactive. They resolved serious medical issues, supported my recovery and independence, provide excellent meals, daily activities, clear communication and a clean, family-like environment.

    Summary of reviews

The reviews present a strongly mixed picture of care at this facility. Several families describe clinically positive outcomes — notably successful respiratory weaning, removal of tracheostomy, regained consciousness, and measurable rehabilitation progress — along with attentive nursing, thorough morning care, and staff who add individualized therapy or proactive health monitoring when needed. Many reviewers also praised on-site social services, discharge-planning support, engaging daily activities and special events, and an orderly appearance in common areas.

Counterbalancing those positive accounts are multiple operational concerns that recur across reviews. The dominant themes are variability in staff conduct and responsiveness, and inconsistencies in clinical oversight. Families described situations suggesting gaps in medication administration and vital-sign monitoring, delayed responses to resident needs (including incontinence-care and bathing schedules), and uneven physician involvement and documentation. There are also sanitation and odor concerns in some resident rooms and common areas, and reports of personal belongings being misplaced or mismanaged.

Staffing and management appear to be uneven. Several reviews note proactive, responsive administrators and individual staff members who go above and beyond, while others describe rude or cold interactions and unreliable staffing at certain shifts. These contrasting accounts point to inconsistent training, supervision, or staffing levels. Reviewers also raised concerns about transparency of therapy costs and the effectiveness of some therapeutic interventions. A small number of reviewers raised serious concerns following a resident's death and questioned end-of-life communication and documentation.

Facility condition and amenities also show variability. Many families appreciated private rooms and a generally clean, welcoming environment, but others described an aging facility that would benefit from refurbishment and better maintenance in parts of the building. Activity programming is generally seen as a strength, with frequent events and daily engagement noted by satisfied families.

For prospective residents and families: this facility demonstrates capacity for strong rehabilitative outcomes and compassionate care from individual staff members, but there are recurring operational risks to evaluate. Before admission, families should ask about current staffing ratios and training, physician coverage and documentation processes, medication-administration safeguards, incontinence-care and bathing routines, property-management protocols, and an itemized estimate of therapy costs and expected outcomes. A pre-admission tour focused on cleanliness, room condition, activity offerings, and direct conversations with nursing leadership can help assess whether the facility’s strengths align with the resident’s needs.

About A Grace Subacute & Skilled Care

A Grace Subacute & Skilled Care sits in San Jose, California, and belongs to a larger social care network, so you'd find that they focus a good bit on people who need complex medical help and recovery, rather than just basic lodging or everyday care, and they do this with subacute and skilled nursing services. The staff works round the clock and knows how to handle things like ventilators and tracheostomies, and there's care for people on peritoneal dialysis and those who need wound management too. Over 160 beds fill the building, and the rooms are a mix of private and shared-the new renovations give everything a home-like feel, and there's lots of space both inside the common areas and outside in the fresh air. Seniors here get help with everything from physical, occupational, speech, and respiratory therapy to pain management and intravenous treatments, and they run specialized therapies such as tone normalization and seating assessment, which are useful for folks working to get their strength and communication back. There are daily activities with morning exercise, arts and crafts, parties, and visits from salon and barber folks, so the days have a little structure and company. There's always licensed nurses and doctors on-site, and there's also pharmacy, laboratory, ophthalmology, and dietary services, so residents who need medicines, eye care, special meals, or lab tests don't have to leave the place. Everyone works together to develop a care plan that fits each person's needs, and the staff-they're trained and attentive-make sure care is delivered with respect and kindness. The whole place is growing and getting updated so it stays safe, modern, and comfortable for the people living there. The goal is to give high standards of care, whether someone is staying for a short time to recover, or has moved in for longer-term nursing and support.
