    About Heritage Park Nursing Center

    Heritage Park Nursing Center offers both assisted living and skilled nursing care for seniors needing different levels of support, with a 70-bed capacity and certified beds, and in June 2025, four beds will be available. The staff, including registered nurses, licensed vocational nurses, certified nursing assistants, and social workers like Sandra Frank, provide care day and night, and there's both 12-16 hour nursing care and 24-hour call systems, plus 24-hour supervision and emergency call buttons in residents' rooms for peace of mind. Residents get help with everyday things like bathing, dressing, grooming, transfers, eating, managing medications, and ambulation, along with incontinence management, safety monitoring, and support for people who can't walk. The community helps with more complicated needs too, like diabetes care, oxygen therapy, catheter and ostomy help, plus full assistance with feeding and meal help.

    Heritage Park Nursing Center accepts Medicare and Medicaid and is for-profit, owned by a Limited Liability Company, and it shares a campus with a skilled nursing facility. The building's from the 1980s but stays well-kept, with spacious resident rooms that have private bathrooms, mountain views, wall-to-wall carpet in apartments, and hardwood floors in hallways. Both independent and assisted living apartments are available, already furnished, and the grounds have flower gardens, a courtyard with walking paths, a patio with seats and fountains, outdoor and indoor common areas, and a community room for gathering. Residents can spend time in the in-house hair salon or barbershop, join social and educational programs, and take part in a full schedule of activities like fairs, museum visits, and shopping. There's also devotional services, wellness programs, restaurant-style dining, and transportation from Monday through Thursday, as well as at-cost and complimentary rides.

    This center organizes resident and family councils to help address concerns and plan improvements, and offers support for both individuals and families with organized meetings and events. There's an orthopedic specialty unit for mobility and strengthening, skilled rehabilitation and therapy services with a fully contained therapy unit on-site, and a special program for behavioral health and mental health rehabilitation. Guests with Alzheimer's or dementia benefit from memory-stimulating activities and dedicated care, including a secured 30-bed unit with on-site psychiatric and psychological services. The medical team manages rehabilitation after hospital stays, physical therapy (focusing on things like strength, balance, and fall prevention), occupational therapy (helping with self-care and adaptive techniques), and speech therapy for communication and swallowing needs.

    Residents get meals based on MD-prescribed diets, with weekly housekeeping and laundry, assistance with using outside pharmacies if needed, and special intercom systems for staff response. The care team supports power wheelchair users, offers max assist with transfers and showers, and provides hospice and home health services. Special attention goes to promoting independence, dignity, and choice, while daily medical assistance, medication administration, individual care plans, and transportation aim to make life safe, comfortable, and engaging for every resident.
