    Attentive home-like care and rehab

I received very good, attentive care in a clean, home-like facility. Nurses, CNAs (Diana, Estela and Cristina) and the therapy teams went above and beyond-great rehab, tasty meals, fun activities, and regular family updates. I felt treated like family and would recommend this place for recovery and long-term care.

    Summary of reviews

The reviews for Windsor Terrace Healthcare Center present a highly mixed picture. Many families and former residents describe strong rehabilitation outcomes, compassionate front-line caregivers, and an admissions team that facilitates a smooth entry. Physical, occupational and speech therapy are repeatedly highlighted as strengths, along with a wound-care clinician noted favorably in several accounts. Multiple reviewers describe personalized family communication (including phone and FaceTime outreach), an engaging activities program, and portions of the building that feel home-like and well maintained.

At the same time, there are recurrent operational concerns that appear across reviews. Staffing inconsistency and understaffing are frequently mentioned and are tied to delayed responses to call lights, inconsistent bathing and meal assistance, and variable medication administration and documentation. Several accounts describe shortcomings in clinical escalation and wound management that culminated in serious complications; these indicate gaps in clinical oversight and infection-control practices that prospective families should investigate. There are also sanitation and environmental-maintenance complaints (peeling wall coverings, furniture and fixture issues, climate-control variability) that suggest uneven upkeep across the facility.

Dining experiences are uneven: some reviews praise varied, nutritious meals and an attentive kitchen, while others report poor taste, undercooked or inappropriate preparations for special diets, and missed meals when staffing is limited. Activities and therapy programming receive consistent positive feedback when present; however, reviewers note that therapy availability and quality can decline over a stay, and reliance on temporary or registry staff can disrupt continuity of care.

Staff performance and management responsiveness are polarized. Several caregivers, CNAs, therapists, and specific administrative liaisons are named for exemplary service and family-focused communication. Conversely, other reviewers report unprofessional conduct, communication tone issues, and difficulty reaching nurses or administrators when problems arise. Serious concerns about personal-property security (including allegations of staff theft) and about psychotropic medication oversight are present in the record and warrant direct inquiry.

Notable patterns: positive experiences often center on therapy-driven rehab stays, attentive admissions interactions, and specific staff members who go beyond routine expectations. Negative experiences cluster around certain operational traits: inconsistent staffing, delayed clinical responses, variable hygiene and meal support, maintenance problems, and lapses in medication and wound-care processes. Some reviewers describe an initial “VIP” admission experience that does not reflect ongoing daily care, suggesting a potential gap between marketing/arrival presentation and typical resident experience.

For families considering Windsor Terrace, a recommended approach is to perform an in-person visit across multiple shifts; observe mealtime and activity periods; ask about current staffing ratios, use of registry staff, wound-care and infection-control protocols, and medication-administration auditing; request documentation on property-security measures and any facility investigations; and review recent state inspection reports. Those steps will help contextualize the polarized experiences reflected in these reviews and clarify whether the facility’s strengths align with an individual resident’s clinical and safety needs.

    About Windsor Terrace Healthcare Center

Windsor Terrace Healthcare Center holds a Medicare/Medi-Cal certification and runs 133 licensed certified beds, though on an average day you'll find about 116 residents staying there, and you'll notice the nurse staffing works out to about 3.92 nurse hours per resident per day, which is a detailed way of saying there tend to be nurses around but sometimes they do see a nurse turnover rate of 38.3%, and as with any place, ownership is a bit of a web, with Windsor associated and direct ownership by Antelope Holdings I, LLC, plus several indirect owners including Pico Ar LLC and the names Aaron Robin and Avrohom Tress, but they haven't given details about who exactly manages or runs the daily workings and there have been 119 deficiencies noted over inspections, with about 9 tied to infection control and a notable complaint from March 2025, related to a quality of life and care incident, and there have been citations, including one for failing to prevent unnecessary loss of daily abilities and one for missing the standards for infection prevention and control, so it's fair to say that inspectors have called attention to these matters, and still, the facility is clean and focused on offering care for long-term health, rehabilitation, and memory care, as well as independent living, assisted living, and more for seniors who need help with daily activities like bathing and medication management, and you'll see friendly staff around and get the sense of a culture that values respect and kindness, with a set up that aims for a homelike feeling, and the place has a five-star Medicare rating for quality measures, so while there have been issues, there are also strengths, like skilled nursing available around the clock, a secured memory care unit for people with Alzheimer's or dementia, and rehabilitation programs for physical, occupational, and speech therapy, even including Respiratory, Pet, Infusion, IV, and Ultrasound therapy, and post-hospital, sub-acute, and complex wound care for those who need extra healing, and then in the everyday details, every room has TV, cable, phone hookups, WiFi, and new wood floors, with private rooms for those who want them, open courtyards for fresh air, and extras like movies, Nintendo Wii Fit sessions, hair salon, business help, spiritual services, plus all sorts of daily activities, outings, parties, and a resident council to help people stay involved, and you'll also see meal plans directed by doctors, regular nutrition checks by a dietitian, and meals made with good ingredients, and for anyone thinking about moving in, tours run daily, and family can send cheery greetings too, and while it's important to know the place has had a history of federal deficiencies including infection control and quality of life care, there are also lots of programs and staff in place to try and keep residents comfortable, active, and as independent as they can be.
