    Outstanding rehab; attentive, caring staff

I placed my mom here after surgery and was very pleased - the rehab is outstanding with a large therapy gym and therapists who got her strong enough to go home. Nurses, CNAs and staff (Hannah stood out) were courteous, attentive and anxiety-friendly; Dr. Williams, Dr. Chandler and Dr. Raven and the dental team gave excellent, professional care. The facility is very clean, secure, welcoming and has a lovely courtyard, and the meals were surprisingly tasty. Communication with family was excellent - caring, goal-oriented staff I would choose again for therapy.

    Summary of reviews

Delta Nursing & Rehabilitation Center elicits a mix of strongly positive and concerning impressions. Many families praised the rehabilitation program, noting a large therapy gym, skilled therapists, and goal-oriented therapy teams that helped patients regain strength and return home. Reviewers frequently described nursing and CNA staff as caring and attentive during daytime hours, and highlighted effective family communication, good discharge assistance, onsite dental/physician services, and a secure, well-maintained campus with a pleasant courtyard and pet-friendly ambiance.

At the same time, there are consistent operational concerns that prospective residents and families should weigh. The most recurring theme is variability in responsiveness and staff conduct: several accounts describe slow response times to call lights and assistance requests, inconsistent overnight coverage, and variable communication tone from individual staff members. Staffing instability and turnover were mentioned as contributing factors to uneven care continuity. Related issues include sanitation and odor concerns in some common areas and inconsistencies in incontinence-care and bathing schedules.

Dining and activities present a mixed picture. Meals are often described positively โ€” fresh salads and some notably good dishes were singled out โ€” though comments also indicate occasional small or inconsistent portions (for example, light breakfasts). The facility offers an active events program (bingo, light fitness) and an event coordinator, but reviewers also requested more diverse social programming and intergenerational opportunities; activity availability and engagement appear uneven.

Facility attributes are generally favorable in terms of cleanliness, security, and a small, intimate setting that can support individualized attention. However, room size and privacy are limitations: many rooms are shared two- or three-person rooms with limited private space. A few reviewers noted maintenance or equipment reliability issues (e.g., bed function) that merit inquiry during a tour.

Administrative and management issues appear as recurring operational concerns. Families described unclear billing or invoice breakdowns, confusion about insurance/Medicare acceptance, and occasional disputes over parking or fees. Admissions and tour responsiveness showed variability, with reports of long waits and inconsistent check-in procedures. There are also a few serious individual claims โ€” including sanitation and clinical-response concerns and questions following a residentโ€™s decline โ€” that suggest asking direct, specific questions about clinical incident response, staffing ratios, and infection-control practices.

Recommendations for families touring the facility: ask about current staffing ratios (day vs. night), average staff turnover, typical call-light response times, policies for incontinence and bathing assistance, details of therapy schedules and expected outcomes, billing/invoice formats and insurance acceptance, and room/privacy options. Overall, Delta offers notable strengths in rehabilitation, therapist skill, and an attentive daytime staff in a small, well-kept setting, but it also demonstrates operational variability in responsiveness, overnight care, sanitation consistency, and administrative transparency that families should evaluate closely.
