    Comfortable compassionate clean safe care

I placed my uncle here after surgery and felt very comfortable: staff were friendly, attentive and compassionate, the facility was spotless and safety-focused, meals were great, and the therapy team supported his recovery - communication was solid and I'd recommend it.

    Summary of reviews

Reviews depict a facility with sharply mixed experiences: several families describe caring, skilled employees, effective rehabilitation outcomes, and responsive administration, while others raise substantive operational and safety concerns. Positive accounts consistently praise individual caregivers, CNAs, admissions staff, and rehabilitation therapists; those reviewers note dietary accommodations, dementia-focused programming, and instances of hands-on leadership and family advocacy. In contrast, other accounts identify clinical-process failures and safety events that substantially differed from the positive descriptions.

Care quality shows high variability. Some families credit the team with life-saving interventions and progress through rehabilitation. Conversely, a number of critiques describe delays in medication and IV administration, questionable glucose/diabetes monitoring practices, and lapses in following physicians' orders. There are also serious concerns about how certain clinical incidents were handled, including deficient incident follow-up and transfers to acute care shortly after admission. These contrasting accounts suggest uneven clinical competency and inconsistencies in nursing oversight between shifts or units.

Staff behavior and communication are similarly mixed. Many reviewers highlight compassionate nursing and accessible administration; others describe unprofessional conduct, poor responsiveness to call systems, and inadequate communication about hospital transfers and care plans. The pattern suggests variability in professionalism and responsiveness across different staff members and shifts. Several reviews also indicate limited or ineffective investigation when personal property issues or safety incidents occur, which contributes to family frustration.

Dining and environmental services receive both praise and criticism. Some families report appropriate dietary accommodations and generally clean resident areas. However, there are sanitation concerns and pest-control concerns noted in service areas, along with facility maintenance issues such as peeling paint and nonfunctional equipment (for example, call buttons). Noise and behavioral-management challenges in communal areas were also mentioned, indicating a need for consistent supervision and programming to reduce disturbances.

Management and operations are characterized by a mix of engaged leadership and operational gaps. Positive comments point to hands-on directors and case managers who facilitate smooth admissions and discharge planning. Negative comments focus on weak incident follow-up, inconsistent security controls for residents' belongings, and variable staff training or oversight. The facility has undergone a name change to Yucaipa Post Acute, which prospective families may wish to confirm when evaluating current performance or policies.

Notable patterns across reviews are the degree of variability in experience and recurring themes around medication administration, responsiveness to resident needs, property security, and sanitation/maintenance. Prospective residents and families should consider an on-site visit that includes: verifying current staffing ratios and supervision practices, reviewing medication-administration and incident-investigation protocols, inspecting call systems and maintenance, asking about pest-control and kitchen sanitation processes, and requesting references from recent families. These steps can help clarify whether the facilityโ€™s strengths align with a prospective residentโ€™s needs and whether the operational weaknesses cited have been addressed.
