    Caring professional staff and facility

I'm very pleased with my loved one's stay - the staff were professional, consistently caring, and very attentive (special thanks to Wendy in PT and Nancy in admissions). Nursing, CNAs and rehab/PT/OT were excellent, and administration was communicative and family-centered. The facility is bright, clean, comfortable, with great food, engaging activities and a lovely courtyard - a very reassuring experience.

    Summary of reviews

The reviews for Autumn Lake Healthcare at Madison present a strongly mixed picture in which clear programmatic strengths coexist with notable operational weaknesses. Many families praise the clinical and therapy teams: physical and occupational therapy are repeatedly identified as a facility strength, and individual nurses, aides, and rehab staff receive frequent commendation for compassion, skill, and effective discharge planning. The environment itself is often described as bright and airy with a pleasant central courtyard, private bathrooms in double rooms, and engaging recreation offerings — bingo, singalongs, entertainers, special events and personalized celebrations (birthdays, themed meals) are called out as meaningful quality-of-life features.

At the same time, a consistent theme is variability in frontline care linked to staffing and management practices. Multiple accounts describe instances where residents experienced delays in assistance with personal care or toileting needs and limited overnight or weekend responsiveness; these accounts are most often associated with reports of understaffing and high turnover. Families also describe cleanliness and sanitation lapses in particular areas and inconsistent maintenance (including intermittent air-conditioning problems and furniture/bed issues). These operational gaps appear to correlate with periods when the facility is described as short-handed.

Dining and medication practices emerged as another area of divergence. Several families report positive dining experiences, fresh meals, and meaningful food-related events; others cite inconsistent meal quality, missing or incomplete trays, and concerns about how special-diet and allergy instructions are followed. A subset of reviewers raised questions about medication handling and clinical decision-making, including uncertainty among staff about medications and gaps in therapy availability after acute events. Rehabilitation is viewed positively when available and staffed, but some families reported limited therapy time or scheduling scarcity that constrained recovery potential.

Management and communication show both strong and weak elements. Admissions and some administrators receive praise for being helpful, knowledgeable, and family-oriented; conversely, there are repeated comments about slow or unresponsive administrative communication, billing surprises, and poor follow-up on family concerns. Reports of workplace-culture issues — gossip, retaliation, and morale problems — suggest that staff relations may be affecting consistency of care. A few reviews reference security or property-loss concerns related to personal belongings and caseworker supervision.

In sum, Autumn Lake Healthcare at Madison offers important strengths in therapy services, engaged recreation, and several highly praised caregivers and administrators. However, prospective residents and families should be aware of recurring operational themes: inconsistent staffing and turnover, episodic sanitation and maintenance issues, variability in dining and medication processes, and communication or administrative gaps. These patterns suggest the facility can provide excellent care under the right staffing conditions, but outcomes appear to depend heavily on unit-level staffing and managerial responsiveness. Visitors should tour the building, ask about current staffing levels, specific care plans for incontinence and medication management, therapy schedules, and the facility’s procedures for incident notification and property safeguards before making placement decisions.

About Autumn Lake Healthcare at Madison

Autumn Lake Healthcare at Madison sits on Wildwood Avenue in Madison, Connecticut, and has a calming, suburban setting close to the coast, and you'll notice right when you come up the walkway that it's got a welcoming courtyard and looks like a place set up for comfort and relaxation, with warm spaces both indoors and out. The facility's licensed for 90 certified beds and focuses mainly on nursing home care and rehabilitation, and they call themselves a Skilled Nursing Facility and a Physical Medicine and Rehabilitation Clinic. Autumn Lake Healthcare at Madison belongs to a bigger network of centers spread over Connecticut, New Jersey, Maryland, West Virginia, and Wisconsin, and they've even got other Autumn Lake facilities with names like Berkeley Heights, Oceanview, and Carneys Point, so you can find them in quite a few states.

The staff, which includes nurses, therapists, and physicians, offers round-the-clock care, but lately there's been a nurse turnover rate of 52.5%, which is higher than the state average, and daily nurse staffing comes to around 3.49 hours per resident per day, which is a bit less than the state average too, and that tells you they've had some trouble keeping the same nurses and keeping nursing hours higher. Mark Schwartz has managed the facility since November 2023, and the official owners are Kc Derby Ct Al Opco Jv Llc, with another company called Aut Ct7 Holdings Llc involved as indirect owners, and they license their name out to Autumn Lake Healthcare Facilities, which is a management company handling non-healthcare services.

The facility is promoted as having enhanced comfort and safety, and you'll find a 24-hour emergency alert and response system in place. Amenities are designed to support residents, and there's a gym with modern rehabilitative equipment, and a daily calendar of activities so residents have something to do, both inside and outside. The therapy team uses personalized recovery plans that change as residents get better, and the rehabilitation programs focus on hands-on, outcome-focused care with board-certified specialists handling complex medical conditions and advanced care programs. They offer skilled therapy, pain management, restorative care, wound care, IV therapy, stroke recovery, orthopedic rehab, and help with managing diabetes and bariatric needs, plus specialized care like audiology, dental, vision, psychiatric, podiatry, and respiratory services, and they're also set up for wound care and colostomy care. Doctors and the therapy teams work together to help people recover strength, balance, and independence, and there's both long-term care and short-term, post-hospital rehabilitation, as well as respite care, hospice, palliative care, and sub-acute rehabilitation services. Staff members mostly speak English but some know other languages too, and they work to keep things feeling homelike and active for residents.

Autumn Lake Healthcare at Madison takes Medicare, Medicaid, private insurance, and private pay, though they're not accepting new patients at this time, but you can still ask about future openings and they keep their online directory up to date monthly. Despite the good points, the facility's got a history of state inspection deficiencies, with 55 found on record, including two infection control issues and several deficiencies noted about resident safety and resident rights, such as not always reporting health changes in a timely manner to residents, families, or physicians and not always protecting residents from abuse, neglect, or exploitation. Recent complaint reports have pointed out these safety and care concerns, so it's important to pay attention to inspection reports along with the facility's medical, therapy, and activity offerings when considering a placement.
