    Professional, attentive staff; excellent rehab

I trusted this facility with my mom and felt reassured from day one - professional, knowledgeable staff provided personalized, attentive nursing and excellent rehab care. The building is new, spotless and hotel-like with cozy rooms, great therapy amenities and plenty of activities; reception and social services were informative and helpful. Staff were consistently friendly, caring and responsive, and I would recommend this clean, well-run center to other families.

    Summary of reviews

The reviews for The Lakes of Clermont Health and Rehabilitation Center present a clear pattern of strong physical environment and rehabilitation services paired with recurring operational challenges. The facilityโ€™s physical plant and amenities receive consistent praise: reviewers describe a modern, hotel-like building with clean, well-appointed rooms (including large private bathrooms), attractive dining spaces, therapy gyms, theater and salon spaces, and pleasant outdoor areas. Many families and residents highlight successful short-term rehabilitation outcomes and strong skill among therapists, making the center an attractive option for post-acute rehab planning.

Staffing and direct care are the most frequently cited mixed elements. Numerous reviews commend front-desk personnel, concierge staff, certain CNAs and nurses by name for compassionate, attentive interactions; these staff members are credited with making residents comfortable and facilitating family communication. At the same time, a recurring theme is understaffing and inconsistent nursing responsiveness: long call-button wait times, delayed medication administration, staffing gaps overnight and on weekends, and variability in clinical skill across shifts. These operational weaknesses have translated, in some accounts, into delayed basic care tasks, missed or late medications, and safety concerns such as fall- and bed-safety incidents and gaps in post-event communication.

Dining and activities present a similar contrast. Meal presentation and some menu items are well regarded, and the facility offers a range of activities and social spaces that reviewers value. However, there are repeated comments about meal errors, temperature and timing issues, limited diabetic/low-sodium options, and occasional allergy risks. Activity programming and social engagement are available and meaningful for many residents, though some families find that organized activities require more proactive outreach or family initiation to engage certain residents.

Management, coordination and logistics emerge as areas for improvement. Reviews note frequent leadership and nursing-director turnover, delayed or inconsistent documentation and care-plan completion, maintenance delays (for example, accessibility modifications), and coordination problems for off-site appointments and transportation. Several reviews allege more serious administrative and accountability concerns; these commentaries raise questions about follow-through and leadership responsiveness. There are also intermittent reports of lost or mishandled personal belongings and inventory shortages that point to process and supply-chain weaknesses.

In sum, The Lakes of Clermont offers a high-quality physical environment and a generally strong rehabilitation program with many staff who deliver compassionate, outcomes-focused care. Prospective residents and families should balance these strengths against recurrent operational issues: staffing levels and consistency of nursing coverage, medication- and care-timeliness, weekend and after-hours management, dietary accommodations, and care-coordination processes. When evaluating placement, consider asking facility leadership for current staffing ratios by shift, nurse/therapist schedules, medication administration procedures, allergy/diet accommodations, weekend coverage plans, and examples of recent process improvements addressing the communication and maintenance gaps families reported. Conducting a tour that includes a conversation with therapy leadership and the on-duty nurse manager can help clarify how the facilityโ€™s strengths will meet a specific residentโ€™s clinical and safety needs.

    About The Lakes of Clermont

The Lakes of Clermont Health and Rehabilitation Center provides health and rehabilitation care for people who need long-term or short-term support, and the center has both private and semi-private suites, each with its own bathroom and shower, along with healthcare amenities that include physical, occupational, and speech therapy and a registered dietitian who helps with meals and nutrition. The center has 40 private suites and 20 semi-private suites, so folks can have some privacy, and there's also an enclosed courtyard where residents can get plenty of fresh air and an on-site movie cinema for entertainment when they want it, and that seems nice when someone wants a change of pace. The staff offers skilled nursing services, and there's a whole team of nurses, therapists, and support staff who aim to encourage residents and provide full-service care, and new residents work with staff to set up their own care plan to fit personal needs. The center offers regular health care services and outpatient rehab, and they try to help residents recover as best they can with specialized programs, but the nurse turnover rate is 47.6%, which is higher than the state's average, although there are slightly more nurse hours per resident than most other places. The center did have some recent deficiencies found in state inspections, like issues with food sourcing and handling, problems with labeling and storing medications, and some concerns about how care matched doctor orders and how well infection control was managed. Management changed hands in the last year, with direct ownership held by Clermont SNF Holdco LLC and other interests linked to Copper Fl Trust II, Gold Fl Trust II, and RM IRRV TR 07012022, while day-to-day things are handled by staff including James Clemons and Rahul Pathak. The Lakes of Clermont Health and Rehabilitation Center does accept referrals and offers tours for families who want to look around and learn more, and their website lists more information about all the services and amenities they provide.
