    Compassionate attentive staff, clean facility

I'm very pleased with the care my dad receives here - the staff are loving, attentive and professional (Administrator Kathy, DON Tomeka, social worker Chonda and CNA Minerva made a real difference). The facility is spotless with fresh floors and no odors, Chapel services and plenty of activities add warmth, and rehab/therapy staff helped meaningful progress. Communication is clear, compassionate and accessible; I recommend taking a tour - this is a family-like, trustworthy place where residents are well taken care of.

    Summary of reviews

The body of reviews presents a polarized picture of Davenport Nursing and Rehab Center: many families and patients describe highly dedicated individual caregivers, strong therapy outcomes, and engaged administrative staff, while others raise significant operational and safety concerns. Positive feedback consistently highlights attentive CNAs, skilled physical and occupational therapists, and administrators who participate in care-planning and communicate with families. Several reviewers credited therapy staff with measurable rehabilitation progress and named specific clinicians as strengths of the program.

Care quality and staffing are the most mixed areas. On the positive side, reviewers describe compassionate bedside care, effective rehabilitation programming, and individualized attention from certain nursing and therapy staff. Conversely, recurring themes include long response times to requests, missed or delayed medication doses, documentation gaps, and reports of pressure-injury and infection-relapse concerns. These indicate variability in clinical consistency and suggest the facility may have episodic lapses in medication administration and monitoring. Multiple comments also point to staffing shortages among CNAs and therapy personnel, which reviewers link to delayed assistance and less frequent therapy than initially promised.

Dining and activities receive uneven evaluations. Some reviewers describe clean dining areas, adequate portions, and meaningful activities including chapel services and social programming. Others note monotony in menu choices, occasional cold or small portions, and interruptions to expected meal schedules. Activity availability appears present but may be impacted by staffing levels and resident mix.

Facility condition and cleanliness perceptions are mixed. Several reviewers praised recent renovations, fresh paint, new flooring, and generally clean rooms. At the same time, other reviewers expressed odor concerns, sanitation and pest-control issues in some areas, and aging infrastructure such as outdated furnishings and inconsistent HVAC/air-conditioning performance. Maintenance delays were reported by families who felt environmental or equipment problems were not addressed promptly.

Management, communication, and administrative processes show both strengths and weaknesses. Many families praised specific administrators and caseworkers for responsiveness, empathy, and clear communication during meetings. However, other reviewers described misinformation around admissions/discharge, billing disputes, voicemail and phone unresponsiveness, and examples of dismissive or poorly coordinated family communication. One serious theme raised by reviewers involves allegations of manipulation or suppression of online feedback; such claims point to trust and transparency concerns that would merit further review by oversight parties.

Notable patterns: the facility often elicits strong praise for individual staff members and therapy programs, yet systemic operational weaknessesโ€”particularly inconsistent staffing, medication/documentation processes, cleanliness variability, and communication breakdownsโ€”create a wide range of resident and family experiences. Prospective residents and families should consider an in-person tour and ask targeted questions about staffing ratios, medication administration protocols, infection-control and pest-management procedures, availability and frequency of therapy sessions, safety equipment (call systems, bedrails, monitoring), and how the facility handles billing, discharge coordination, and family communication. These focused inquiries can help distinguish unit-level strengths from the facility-level variability reflected in these reviews.

About Davenport Nursing and Rehab Center

Davenport Nursing and Rehab Center sits in Davenport, FL, and has 60 licensed and certified beds for residents who need skilled nursing care, rehab therapies, or memory programs, and the place really handles a lot, everything from physical and occupational therapy to mental health services and dental care, so you don't have to worry about finding different providers for each need, and the center even provides pharmacy services, diagnostic X-rays, and clinical laboratory work right there, which can make things a bit easier if going out for those things is hard. The staff includes healthcare professionals who are there at all hours, and you'll also find physician services and physician extenders, which means residents get medical attention even if the main doctor isn't always right there, and there's skilled help available for post-acute care after hospital stays, with the main goal being to restore everyone to their best possible level of health and function.

Residents have the option to join organized groups, and there's also support for family members, plus a calendar of activities for those who like to stay busy or social. The facility offers dietary services made for various needs, handles regular housekeeping, and keeps the living areas clean, though inspection reports have shown some infection control issues and deficiencies, including two related to infection standards and a total of 14 deficiencies marked in recent history, so families might want to review those inspection records which are available. The nurse turnover rate is quite a bit higher than the state average at 67.2% with about 3.22 nurse hours per resident per day, so staff may be newer or changing hands more often than in some places, but the staff is described as experienced and seems dedicated to making residents comfortable, helping with daily living needs, and focusing on treating each person with dignity and respect.

Davenport Nursing and Rehab Center takes both Medicare and Medicaid, has a Health Care Center, and is fully equipped with a range of services using up-to-date technology, and the managers-Catherine Valentino Keesling and Melanie Albors Mora-took on their roles in January 2025 with the facility itself owned by 206 W Orange St Davenport Holdco LLC and managed with some affiliation to Eliyahu Mirlis. The community keeps its doors open every day and works to balance safety and comfort, with an emphasis on restoring independence and making sure every person keeps as much self-respect and autonomy as possible. Anyone interested can see their website at davenportcenter.com for more details, and since the inspection reports show some deficiencies, it's a good idea for families to review those records or ask questions if particular care topics are important to them.
