    Caring staff and effective rehab

I had a very positive stay - the nurses, aides and therapy team were caring, attentive and organized, and helped me progress during rehab. The facility is clean, well-maintained and welcoming with friendly front-desk staff and good communication. I recommend it for skilled nursing and therapy and would choose it again.

    Summary of reviews

Reviews of Sandalwood Rehabilitation and Nursing Center are highly mixed, producing a polarized picture. A recurring positive theme is the quality of hands-on nursing and rehabilitative care: multiple accounts describe compassionate, attentive nursing staff, regular physical therapy, and competent clinical caregiving that supported discharge goals. The dining program is frequently noted as a strength, with meals described as palatable and well received. Likewise, an activities program offering a variety of events is cited as a contributor to resident engagement.

At the same time, a substantial set of operational concerns appears across reviews. Sanitation and odor issues in resident rooms and certain common areas, together with pest-control concerns, are raised frequently enough to suggest uneven environmental maintenance. Several reviewers described problems with incontinence-care management and with timeliness of assistance, which aligns with broader statements about chronic understaffing and high turnover. Staffing shortages are also linked to medication-administration delays and reduced responsiveness to resident needs.

Clinical coordination and discharge practices draw contrasting feedback: some families report effective rehabilitation and readiness for home, while others describe inconsistent therapy delivery, unmet clinical needs (for example, required treatments not provided), and abbreviated or rapid discharges. Safety-related practices are another area of concern; reviewers mention lapses in transfer and mobility-safety practices and the improper use or monitoring of safety devices. Facility environment issues include insufficient climate control in resident rooms and occasional concerns about mattress/equipment standards.

Management, communication, and administrative processes are a recurrent theme. Several reviewers report difficulty contacting the administrator or social work staff and describe limited direct lines for families to reach residents. Billing transparency and business-office reliability are questioned in a number of accounts, and there are serious allegations concerning staff theft and financial misconduct that warrant attention. Security procedures are also described as lax by some families, citing unsecured entrances and minimal visitor-control practices.

Overall, the pattern in these reviews is one of clear strengths in bedside nursing and rehabilitation when staffing allows, combined with facility-level operational weaknesses that impact sanitation, safety, communication, and administrative trust. Prospective residents and families should weigh the positive clinical reports alongside the operational concerns, ask for current documentation of staffing levels, infection- and pest-control measures, therapy availability, security procedures, billing practices, and opportunities to speak directly with nursing leadership before making a placement decision.

About Sandalwood Rehabilitation and Nursing Center

Under New Management: Sandalwood Rehabilitation and Nursing Center sits in Daytona Beach Shores, Florida, and has seen quite a few changes lately with new management, which might mean improvements in services and care quality. Staff get special training to watch for elder abuse or neglect, and there's a big focus on having enough caregivers to lower risks like injury, bed sores, or falls, with a professional team looking out for health and safety. The facility provides nursing care after hospital stays, surgery, or illness, and there's skilled support 12-16 hours a day, with 24-hour supervision for people who need more help, including those with complex or serious medical needs, so folks who need regular medication help, transfers, bathing, or dressing will get it from licensed nurses.

Sandalwood offers different kinds of living, including assisted living, skilled nursing, independent living, and memory care, with safe environments for people with Alzheimer's or other dementia, and activities to help keep memory sharp and stop wandering. Amenities keep residents comfortable, like restaurant-style dining, a wellness center, fitness room, game room, library, computer center, movie theater, beauty salon, and garden, with furnished rooms that have private bathrooms, kitchenettes, cable TV, telephones, air conditioning, and Wi-Fi, and there are pet-friendly policies and handicap-accessible spaces too.

The community hosts regular activities-planned trips, group outings, music programs, scheduled fitness, and social events-which can help people get to know each other and stay active, and there are services like housekeeping, laundry, transportation, and move-in coordination for practical support. Meals are prepared by a chef, follow dietary restrictions when needed, and the dining hall operates restaurant-style, which can be nice for people who like to eat together.

The place accepts Medicaid and Medicare and has 99 beds, with 37 certified beds open as of June 2025, and around 78 occupants currently living there. It provides respite care if someone needs a short stay, and support extends to family members too, with a resident and family council to address concerns. Memory care units have secure entry and staff monitor residents closely for their safety. The latest reviews give Sandalwood a score of 9.2 from 9 reviews, with recognitions like Best of Senior Living 2025, and it's cited for friendly, joyful staff and good dining options. Safety audits show three minor deficiencies and no fines. Sandalwood Nursing Center, as some call it, tries to help seniors live as independently as possible-even those who need help getting around or support all day-and wants everyone to feel safe, comfortable, and well looked after.
