    Pleased with caring, clean rehab

I placed my mom at Bear Creek for rehab and I'm very pleased. The staff are friendly, professional, and genuinely caring - nurses, aides and therapists were attentive and effective. The facility is spotless with no odor, on-site meals, and lots of engaging activities that made her stay comfortable and home-like. Admissions and administration were responsive and supportive; overall a reliable place I'd recommend.

    Summary of reviews

Overall impression: Reviews of Bear Creek Nursing Center present a mixed but generally positive picture dominated by praise for frontline staff and the rehabilitation program. Many families and residents describe attentive, compassionate nursing and caregiving, and the therapy teams (physical, occupational and speech) are repeatedly identified as effective and outcome-focused. The facility is frequently described as clean, bright, and well maintained, with a range of room options and amenities (hair/nails services, outings, pet visits) that contribute to a comfortable, family-friendly atmosphere.

Care and staff: Strengths center on the quality of clinical and rehabilitative care. Reviewers cite skilled therapy staff who provide daily, documented progress and help residents regain mobility and function. Nursing and CNA teams are often described as caring and responsive, with housekeeping praised for consistent room upkeep. At the same time, a pattern of inconsistent staffing and coverage emerges in a number of accounts: understaffing or perceived short-staffing is associated with delays in responses to calls, delayed medication administration, and occasional lapses in routine personal care. There are also isolated concerns about staff conduct and tone; while many interactions are characterized as respectful and family-like, others describe unprofessional behavior that families found troubling.

Dining and nutrition: Dining receives both commendation and criticism. Several reviewers note appetizing meal presentation and the facility's willingness to accommodate dietary needs, and some families praised a proactive nutritionist. Conversely, other accounts describe inconsistent food quality, service interruptions, and shortages during shifts. Prospective residents should ask about current menu rotation, accommodations for specific dietary needs, and meal-service continuity when evaluating the facility.

Activities and environment: The facility’s activity program is a consistent positive for many families. Reviewers describe a lively calendar of music, games, crafts, social meals, outings, and holiday events; residents benefit from outdoor seating and bright common areas. These social and recreational offerings are cited as contributing to improved mood and engagement for residents. A small number of comments indicate variability in activity delivery, so families may wish to observe the activity schedule directly.

Management, admissions and administration: Administrative responsiveness is commonly praised—admissions staff and leadership are credited with facilitating transitions, helping with payor issues, and communicating about care. Some reviewers singled out above-and-beyond efforts by administrators to secure approvals or expedite placement. However, there are also persistent concerns about the professionalism and consistency of the admissions process for some families, and occasional problems with upfront billing, payment handling, and refund timing. Communication gaps by phone or social work staff are also noted and can affect family confidence.

Safety, sanitation and serious concerns: While many reviewers report a clean, odor-free environment and effective housekeeping, other accounts identify sanitation and pest-control concerns in certain rooms and isolated equipment-maintenance problems (for example with lifts or mobility equipment). Safety-practice gaps and supervision concerns appear in multiple reviews, linked by families to delayed responses and inconsistent protocols. There are also a small number of serious allegations, including theft and potential financial misconduct; these claims are severe and uncommon in the dataset but warrant inquiry and, if appropriate, verification with regulatory authorities before placement.

Recommendation guidance: Bear Creek appears to be a strong option for short-term rehabilitation and for families prioritizing skilled therapy and a warm care culture. Prospective residents and families should balance those strengths against the documented operational weaknesses: ask directly about current staffing ratios and contingency plans, medication-administration protocols, call-light response times, equipment maintenance, infection-control practices, billing procedures, and how the facility handles family communication. Observing mealtime service, the activity schedule, and a clinical care meeting or care-plan review will help assess whether the current operational practices meet your expectations.

    About Bear Creek Nursing Center

Bear Creek Nursing Center sits at 8041 State Rd 52 and has space for up to 120 residents, and most days, about 102 people live there. The place offers a range of care including nursing care, assisted living, independent living, skilled nursing, memory care, respite care, stroke recovery, and both short-term and long-term rehabilitation, and it's run by Health Services Management, Inc. Nurses work around the clock, but the nurse hours per resident per day hit 3.49, which is a bit lower than the state's average of 3.9, and nurse turnover there is high at 64.5 percent. A team of managers, named Kevin Baxter, Scott Fisher, Brian Jackson, Jim Shatz, Mario Talanga, Teresa Timmons, and Joshua White, manage things. Bear Creek Nursing Center handles different needs, like physical, occupational, and speech therapy, diabetes care, health education, social services, and help with daily tasks like bathing, walking, or dressing, and they give care for folks who need IVs, injections, and medication management. The facility says it wants to create a comforting space for people recovering from surgery, illness, or stroke, and it offers quality-of-life programs and nutritional assessments tailored to each person. The place is wheelchair accessible and offers typical things like air conditioning, restrooms, and parking. Some folks have left reviews, and the facility has a rating of 3.8 out of 5 from 61 reviews. However, inspections have found 20 deficiencies, with problems in pharmacy service, accident prevention, and care planning, such as not having a licensed pharmacist review drug regimens each month and not making sure all accident hazards are fixed or that assessments line up with residents' needs. Bear Creek Nursing Center covers a wide mix of care for older adults with many kinds of needs, but it's had several issues noted by inspectors and faces some challenges with staff turnover and meeting some state requirements.
