    Compassionate care, clean and welcoming

I'm very pleased with the compassionate, person-focused care here - the facility is clean, welcoming, and the food is excellent. Staff like Debra, Nurse Tara, Willie Frazer and admission coordinator Jwana Carn went above and beyond; I felt well cared for, made friends, and would recommend this place for rehab or longer stays.

    Summary of reviews

Overall impression: Reviews of Westside Oaks Rehabilitation and Nursing Center are highly mixed, with a clear split between positive experiences tied to specific staff members and therapy services, and recurrent operational concerns that families identify as systemic. While some families report strong rehabilitative outcomes and describe individual nurses, therapists, and admissions staff as compassionate and effective, a substantial number of accounts raise persistent issues in daily operations and facility maintenance.

Care quality and clinical oversight: Therapy services (OT/PT/speech) are frequently described as a strength and are credited with positive functional outcomes for many residents. However, numerous reports highlight inconsistent nursing coverage, long delays in responding to call lights, and gaps in medication administration and pharmacy coordination. Reviewers also raised clinical-safety concerns such as pressure injuries, infection risk, and delayed recognition or treatment of clinical conditions. These point to inconsistencies in clinical oversight and in the handoff processes between shifts.

Staff and conduct: Several reviews praise individual caregivers and nurses for compassionate, person-centered interactions. At the same time, families commonly describe staffing shortages, high turnover, and variable staff professionalism. Specific concerns include tone and conduct during resident interactions, insufficient night-shift supervision, and at least a few serious-allegation items relating to missing personal items and inappropriate staff behavior. These patterns suggest uneven staff training, supervision, and accountability.

Dining and activities: Dining experiences are inconsistent across accounts. Some families reported acceptable meals, while a number of reviewers described poor meal quality, limited menu variety (including limited vegetarian options), and interruptions in timely feeding. Activity programming was less frequently discussed; when mentioned, reviewers noted limited resident engagement and reduced supervisory presence in common areas.

Facilities, housekeeping, and maintenance: Multiple reviews raise sanitation and cleanliness issues, odor concerns in halls and common areas, and pest-control concerns. There are numerous mentions of laundry problems, lost personal items, and building-maintenance problems such as plumbing leaks, moisture intrusion, and ceiling damage. These facility and housekeeping deficiencies contribute to perceptions of an environment that requires stronger routine maintenance and infection-control oversight.

Management, communication, and administration: A recurring theme is difficulty reaching management and inconsistent family communication, including delayed notifications about hospital transfers and limited responsiveness from case managers. Reviewers also describe billing and reimbursement disputes and frequent administrative turnover. Positive notes about helpful admission staff and a few responsive administrative leaders indicate variability in leadership effectiveness across time or by role.

Notable patterns and guidance for families: The facility appears to deliver solid rehabilitative therapy for some patients and has individual staff members who provide high-quality, compassionate care. However, the volume and consistency of operational concerns—staffing, response times, medication processes, sanitation and maintenance, and family communication—are sufficient to warrant careful due diligence. Prospective residents and families should request current staffing ratios, recent state survey results, documentation of pest-control and infection-prevention measures, examples of medication-safety protocols, and contact information for the on-call clinical manager. When possible, speak with recent family members or discharge planners about continuity of care and observe cleanliness and call-response times during a visit.

About Westside Oaks Rehabilitation and Nursing Center

Westside Oaks Rehabilitation and Nursing Center, formerly Jacksonville Rehab and Nursing, sits on Hyde Park Road in Jacksonville, Florida, and offers many care options for seniors who need rehabilitation, skilled nursing, or long-term stays, and you'll find short-term respite available for anywhere from a few days up to several months, along with seven days a week short-term stays after a hospital admission, and some folks might stay for permanent, residential care too. The center takes admissions from hospitals, assisted living, and from home after a doctor says it's okay, and you'll see private and semi-private rooms along with amenities like internet, cable TV, a smoking area for residents who need it, a dining room, and hair styling from their on-site stylist, plus you'll notice they provide daily activities and housekeeping as part of daily living support. The place provides therapy up to seven days a week, including physical, occupational, and speech therapy, and there's a rehabilitation gym for those who need help regaining strength or skills, as well as specialized clinical and senior rehabilitation programs, with a special focus on improving recovery times and supporting seniors after hospitalization for COVID-19 through their Post COVID-19 Recovery Program. Westside Oaks Rehabilitation and Nursing Center uses portable telemedicine equipment for care during the day, after hours, and even on weekends, which helps doctors and staff respond faster to unexpected health needs, and the place partners with Riverside Post Acute to support more complex needs with skilled rehabilitation services, clinical care, and acute care when needed. Residents and patients see compassionate caregivers day and night, with hospice services provided to focus on comfort and dignity, and the respite program includes 24-hour nursing care, activities, social engagement, meals, and support services. The grounds are landscaped for relaxation, and visiting policies include careful guidelines for infection control, which means visits happen indoors even during outbreaks if everyone uses protective equipment, and the rules change if the community transmission status changes, with some staff focused on tracking infection prevention and making sure visitors understand when they need to wear masks, sanitize hands, or sign waivers, and there are signs up about COVID-19 symptoms and prevention protocols. The center encourages visitors to get vaccinated and keeps designated areas clean after each visit, and you'll find most visits are allowed as long as health safety steps get followed, with the staff on hand to teach families and enforce compliance, while government officials have separate guidelines. Westside Oaks Rehabilitation and Nursing Center remains open for admissions all week, supports patient movement from different care settings, and follows policies to keep residents, families, and staff as safe as possible while offering the comfort and engagement that long-term or short-term residents need.
