    Attentive, compassionate staff; successful rehab

I'm very pleased with the care my mom received - attentive, compassionate nurses and therapists helped her make tremendous strides in rehab and return home. The small, home-like facility has good food, lots of activities, a pleasant courtyard and helpful, professional staff who keep family well informed.

    Summary of reviews

Reviewers describe a facility with notably strong rehabilitation services alongside recurring operational concerns. Physical and occupational therapy staff receive consistent praise for professionalism, individualized plans, and measurable recovery progress; several families credited therapy teams with enabling discharge home. Individual caregivers and some nurses are described as compassionate and effective, contributing to positive experiences for rehabilitation and social engagement.

At the same time, a pattern of care-delivery issues emerges. Multiple accounts describe delayed responses to call bells and difficulty reaching nursing staff, which reviewers link to what they perceive as insufficient staffing levels. Comments indicate variability in nursing and nursing-assistant skill and conduct, with some staff characterized as attentive and others as inattentive or brusque. Medication administration and supply control were also identified as problem areas, including delayed pain medication and instances where medications were unavailable when needed. A fall with bruising prompted questions about prevention and follow-up in at least one case.

Dining and activities are mixed but include clear strengths. The dining room and the option to eat outdoors receive positive notes, and some reviewers found the food acceptable while others described inconsistent food quality. The activities program is a relative strength: reviewers report an active social calendar, holiday events, regular hairdresser visits, and a generally lively resident community that benefits social and mental well-being. The facility's smaller, home-like scale and shaded courtyard are also cited positively for comfort and convenience.

Facility maintenance and cleanliness are uneven. Some rooms and common areas are described as well maintained, but reviewers also raised sanitation concerns in bathing areas, shortages of towels and washcloths, limited bathing schedules (e.g., restrictive shower availability), and maintenance issues such as unsealed openings that could affect environmental control. Several reviewers flagged pest-control concerns. Privacy and personal-property safeguards received negative comments after at least one account of unauthorized access to personal documents.

Administrative practices show variability. Families described mixed experiences with the front office and billing: some encountered helpful, prompt communication, while others reported vague responses, delays in refunds, or difficulty contacting business office staff. Discharge timing and coordination were described as inconsistent, suggesting gaps in care planning and transition processes.

Overall, the facility appears to offer strong rehabilitative care and an engaging activities program within a small, home-like setting, but prospective residents and families should weigh that against operational weaknesses in staffing, responsiveness, medication management, cleanliness control, privacy safeguards, and administrative communication. When touring, ask specifically about staffing ratios and call-bell response metrics, medication administration processes, bathing schedules and linen provisions, pest-control and maintenance protocols, and discharge/communication procedures to assess whether the facilityโ€™s current operations align with your expectations.

About The Palms Rehabilitation and Healthcare Center

The Palms Rehabilitation and Healthcare Center sits in Palm Bay, Florida, and has been serving the community since 1998 with a range of skilled nursing and rehab services for elders and adults needing extra medical help. This center's well known for its care for seniors coming from the hospital or needing long-term support because they're very frail or facing serious illnesses, and there's everything here from short-term rehab to long-term skilled nursing. The facility holds a license under number #130470985. It's got options for Medicare and Medicaid, and helps people work through insurance for both short- and long-term needs, which does ease financial worries. The center often runs near full capacity, with 8 of 120 beds open as of June 2025 and 90 of those beds dedicated to skilled nursing.

Individuals find a choice of private or semi-private rooms, even spots set up for couples, with basics like private bathrooms, cable TV, kitchenettes, phones, air conditioning, and Wi-Fi. Many people here depend on nursing care or are recovering from surgery, and the center's staff handles everything from post-acute care and rehabilitation to memory care inside a secure locked unit for women with Alzheimer's or dementia. You'll see specialized services such as pain and symptom management, wound care, trach care, peritoneal dialysis, support for diabetic, COPD, and cardiac patients, and even care for folks needing up to 10 liters of oxygen or a Lifevest. There's a Certified Wound and Ostomy Nurse on staff, and the team can do IV antibiotic therapy and accept wound vac patients. For those recovering from surgeries, especially orthopedic ones, there are focused recovery programs built around therapy.

Therapy services here include speech, occupational, and physical therapy using things like Vitalstim Therapy for swallowing issues. There's a robust post-operative and inpatient rehab program for those moving from hospital to home. Residents get medication management, help bathing and dressing, supervision both day and night, transfer support for folks who aren't able to walk, and help with daily activities.

Amenities make life more comfortable, with outdoor recreation, gardens, scheduled daily activities, excursions, movies, arts and crafts, music, games, spa and wellness rooms, and a beauty/barber shop. Community dining happens in a shared dining room, with allergy-friendly meals, restaurant-style dining, foods for folks with diabetes, and even a professional chef preparing special options any time of day. There's move-in coordination, meal preparation, house cleaning, laundry, concierge, and transportation services. Within shared spaces, residents use a library, fitness room, movie theater, game room, activity room, arts room, and enjoy garden paths and outdoor common spots for walking or just talking.

The Palms has registered nursing staff hours, up-to-date inspections, and both a resident and family council to keep giving feedback and helping with care concerns. Government records show an A grade in inspections, no major deficiencies, and an overall facility rating of B+ with an average review score of 3.9 from 27 reviews-a decent score meaning people have a mix of opinions but seem generally satisfied. The facility has earned A+ inspection grades in prior years and stays known for high quality in both long-term and short-term care. The Palms belongs to Aspire at Palm Bay and the Consulate Health Care family. The staff, called Ambassadors of Care, focus on creating a caring, healing environment where therapy and medical plans get tailored to each person's health and well-being through a patient-focused approach. Opened as a for-profit, limited liability company, the place doesn't run a Continuing Care Retirement Community. That said, folks don't have to worry about being left alone, with a busy activity calendar, planned events, visitor programs, and responsive clinical care for both medical needs and daily living.
