Solaris HealthCare Plant City sits in Plant City as a skilled nursing facility with 180 certified beds and usually about 172 residents living there each day, so it's almost always near full. Ashley Parker has managed the place since April 2021, and Jennie Weaver serves as the community liaison, with all of it run by Plant City Healthcare Holdings, LLC, which owns the whole place, and it's affiliated with Solaris Healthcare and the Solaris Foundation, Inc., a nonprofit. The staff provide care day and night, every day, offering both long-term nursing care and short-term rehabilitation, and they handle a lot-things like physical, occupational, and speech therapy, along with hospice, palliative, and dementia care, plus medical nutritional planning, diabetic management, medication help, IV antibiotics, respiratory services, and advanced wound care. The team there works on personalized wellness plans using a bunch of experts-doctors, nurses, therapists-to match what each resident needs. They also run infection prevention protocols, especially for visitors, which include face coverings and keeping distance, along with tight visitation rules meant to keep residents and staff safe, especially during the COVID-19 pandemic, when they kept families updated often. The nurse turnover rate sits at 38.5 percent, and residents reportedly get about 3.71 nurse hours each day. Reports have found deficiencies, especially with how they assess residents and plan their care (like F0641 and F0657), and inspectors have found infection control problems too. Even so, the place keeps up a warm and upscale look, with inviting landscaping and a main entrance, and says it aims for high standards and a team approach where residents can transition care types-rehab to long term or other changes-without needing to move to a different facility. Their director, staff, and structure aim to meet a range of medical and daily needs for the elderly, but they have had inspection and complaint reports noting some areas for improvement, so it's wise to consider those as part of the whole picture. Clean facility but unsafe, neglectful
I had a mixed but overall negative experience at Solaris Plant City. The building is clean and modern and a number of staff were excellent-the rehab team, some nurses, admissions and social worker Courtney were responsive and caring. Unfortunately restrictive, prison-like policies (including a $300 overnight fee), coercive billing threats, theft of belongings, toxic management and poor communication overshadowed the positives. Chronic understaffing and slow call-light response led to neglect, multiple ER visits (dehydration, UTI, breathing issues, dislocated shoulder), unsafe falls and failures around end-of-life notification. COVID handling was inconsistent. I appreciated many compassionate employees, but overall I would not recommend this facility.
