    Compassionate, professional care; highly recommended

I felt my sister was in mostly excellent, compassionate hands - caring, professional staff like Cassandra, Isabella and Pam Nelson treated her like family and went above and beyond. The facility is clean, calm and home-like with tasty, varied meals, strong therapy and attentive nurses/CNAs; I'm grateful and would recommend them.

    Summary of reviews

Overall impression Reviews for PruittHealth - Creekside present a polarized picture: many families and patients describe compassionate, skilled caregivers and an atmosphere that feels โ€˜โ€˜home-like,โ€™โ€™ while other accounts raise operational and clinical-safety concerns that warrant careful follow-up. Several reviewers praised nursing staff, CNAs, and therapy teams for personalized care, successful rehabilitation, and strong discharge planning. At the same time, a subset of reviewers raised serious clinical-safety issues; these are allegations that should be verified directly with the provider and through public inspection records. Care quality and staff A predominant positive theme is consistently respectful, attentive, and family-oriented caregiving. Reviewers highlight specific strengths: caring nurses and CNAs, patient and kind bedside manner, and effective therapy staff who deliver individualized plans. Admissions and community-liaison personnel are described as supportive and helpful with transitions. Offsetting this, multiple comments indicate uneven staff professionalism and variability in clinical competence and responsiveness. Concrete operational weaknesses implied by reviewers include inconsistent scheduling for personal care tasks, delays in attending to resident needs, and broader gaps in clinical-safety practices. Prospective families should ask about staffing levels, training programs, and incident-response protocols during a visit. Dining and activities Dining is frequently described positively: varied, home-style menus with items appreciated by residents and visitors. Some reviewers wished for larger portions or desired contact with kitchen leadership, suggesting opportunity for more resident feedback integration. Activity- and community-oriented notes include opportunities for social interaction and church-sponsored visits; reviewers describe a generally bright and engaging common-area environment that supports resident interaction. Facilities and environment The facility interior receives praise for bright, renovated, and cheerful common spaces; maintenance of exterior grounds is also noted positively. At the same time, the building itself is described as older in places, with uneven room-level upkeep and occasional odor concerns in patient areas. Parking capacity and lot condition (including potholes) were repeatedly identified as a practical limitation. Security procedures such as sign-in and secured entry are present and viewed as positive. Management and notable patterns Some reviewers credited improvements following new management and renovation efforts; others called for stronger hiring, training, and supervisory practices to address variability in staff conduct. Administrative communication and transparency were described as inconsistent in a portion of reviews. Importantly, a small number of reviewers made serious allegations regarding clinical-safety events (for example, catheter management, falls, and call-button responsiveness). These items are significant outliers that should prompt verification through direct questions to the facility, review of care plans and incident logs, and consultation of state inspection and enforcement records. Practical recommendations for prospective families When evaluating Creekside in person, focus questions on staffing ratios, staff training and turnover, clinical-safety protocols (falls prevention, catheter care, call-button response), room-level cleaning practices, odor-control measures, and parking access. Request recent state inspection reports and ask for names/roles of contact staff for transitions and discharge planning. Given the pattern of strongly positive caregiver experiences alongside isolated but serious allegations, an in-person visit, review of documentation, and direct discussion with management will provide the clearest picture for decision-making.
