    Compassionate, professional rehab with progress

I placed my father at Pruitt on Wheeler Rd for rehab and they nursed him back to health. The staff were consistently kind, respectful and professional; therapists were knowledgeable and built a tailored plan that delivered remarkable progress. The facility felt clean and calming, the director of nursing was available, and care was coordinated well with the social worker, nurse and therapy team. I'm grateful for the compassionate, attentive staff and would recommend touring the place.

    Summary of reviews

Reviews for PruittHealth - Richmond reflect a mixed experience with distinct strengths in rehabilitation and notable operational weaknesses. Many accounts highlight an effective therapy program: therapists are frequently described as knowledgeable, compassionate, and outcome-focused, with tailored plans that led to measurable progress and relatively quick recoveries. Several reviewers also praised caring direct-care staff, a welcoming atmosphere, well-maintained accommodations, and positive experiences with special-event dining and social engagement.

At the same time, a pattern of inconsistent clinical care and operational issues emerges. Staffing levels and response times are recurring concerns: families mention delays in assistance, long waits for help with toileting or personal needs, and periods when staffing felt stretched thin. These staffing constraints appear to contribute to inconsistencies in hygiene and incontinence care, periodic supply shortages for personal items, and variable adherence to bathing and toileting schedules.

Clinical-management and communication gaps are another theme. Reviewers describe inconsistent medication and treatment management, challenges in obtaining timely documentation, and uneven clarity from administrative or clinical leadership about care plans and incidents. A small number of serious individual claims — including concerns about prolonged unresponsiveness and clinical deterioration following therapy — prompted requests for records and, in at least one case, possible insurance involvement or further review. There is also an isolated allegation of inappropriate billing or service misrepresentation. These items suggest families should seek clear documentation and direct communication when evaluating care.

Facility and service quality appear uneven across units and over time. While some people note clean, comfortable rooms and friendly staff, others cite sanitation and odor concerns in certain common areas and issues with carpet cleanliness. Transfer and transport processes also drew caution, with some reviewers noting lapses in transfer safety or missing equipment during moves. Meal service received mixed feedback: several accounts praise well-presented meals and holiday dining, whereas others raise concerns about dietary management and inconsistent meal quality for residents with dietary restrictions.

Management impressions are similarly mixed. A number of reviewers singled out specific leaders — notably a director of nursing or an administrator — for positive, hands-on involvement that improved outcomes for their family member. Conversely, other families described difficulty getting satisfactory responses, perceiving a lack of follow-through on promises made during tours. The facility has been described as undergoing staffing or organizational changes, which may explain variability between experiences.

Overall, PruittHealth - Richmond shows demonstrable strengths in rehabilitation and pockets of compassionate caregiving, but also exhibits operational weaknesses that can affect day-to-day resident care: staffing levels, care coordination, communication, and certain sanitation and dietary practices. Prospective residents and families would benefit from an in-person tour, direct questions about staffing ratios and backup coverage, review of clinical documentation and medication-management practices, and clarification of dietary accommodation procedures before admission.

    About PruittHealth - Richmond

PruittHealth - Richmond sits a little under three miles outside of Martinez, Georgia, in Augusta, and has a capacity of 100 certified beds with about 85 residents on average each day and, you know, it's not small by any means, so you tend to see a range of folks coming in for different reasons, whether it's skilled nursing for those who need more help, extended care for long stays, or short-term rehabilitation for those just passing through between the hospital and home, and the facility, which used to be called Piedmont Extended Care Kentwood, takes Medicare and Medicaid and operates as a for-profit limited liability company owned mostly by United Health Services of Georgia, Neil Pruitt, and Uhs Pruitt Holdings, and it's part of the PruittHealth group. The building has both private and semi-private rooms, including companion layouts, all with things like private bathrooms, cable TV, kitchenette spaces, air conditioning, and Wi-Fi, and the staff tries to make the place feel warm and homely, with support for personal care like bathing and dressing and a 24-hour call system, so there's someone nearby if help's needed, and about 3.8 nurse hours per resident per day, but the nurse turnover rate of 59.6% is something to keep in mind.

People looking for activities will find community-sponsored programs, scheduled daily activities, movie nights, arts and music rooms, outdoor events, fitness programs, a library, a game room, and walking paths, plus the place is set up to be welcoming with lounges, a garden, activity spaces, and outdoor common areas, so there's plenty for residents to do if they want. Dining includes a restaurant-style dining room, special diets for diabetes or allergies when needed, and chef-prepared meals available throughout the day. In terms of care, PruittHealth - Richmond offers skilled nursing with both 12-16 hour coverage and a round-the-clock response, intermediate care for those who need daily attention, hospice services, wound care, pain management, and therapy with a focus on rehab, especially after hospital stays, and there's housekeeping, laundry, move-in help, and support for families as well.

The facility has a history of 16 total deficiencies in inspection reports, which include administrative problems like not submitting accurate staffing information to CMS, two infection-related issues, and some trouble with nutrition and dietary standards, like not always getting food from approved places or serving it right, and there's also been times they didn't do accurate health assessments or care planning for residents, so families looking at their inspection history might see those notes. Despite these points, PruittHealth - Richmond holds a four-star rating in the federal nursing home system and holds the spot as the sixth-highest rated community in Augusta, with an average rating of 7.1 out of 10, and it continues to be a place with various care options, a busy activities calendar, and a structure meant to support people's changing needs as they age.
