    Compassionate staff, excellent rehab, recommended

I placed my loved one here and have been very pleased - the staff are consistently caring and compassionate, going above and beyond (Kim and the nursing team were especially dedicated). Excellent wound care and rehab helped restore independence, the home-like building is clean with good meals and activities, and they offer comprehensive therapies and services. Communication is prompt, CNAs are warm and attentive, and I'd happily recommend this lovely facility.

    Summary of reviews

Sears Coastal Nursing & Rehab presents a mixed but specific profile. Many families and visitors describe a facility with strong clinical capabilities: an experienced wound-care team, robust therapy services (physical, occupational, speech), and a broad range of clinical programs including subacute care, post-hospitalization rehabilitation, restorative care, memory-care support, and palliative/hospice options. Multiple accounts emphasize successful rehabilitative outcomes and cases where skilled clinical intervention had a meaningful impact on a residentโ€™s recovery. Private rooms with cable/internet, on-site pharmacy support, and a small, home-like atmosphere are additional strengths frequently noted.

Staffing and interpersonal care elicit divergent impressions. Numerous comments praise compassionate, attentive nurses and CNAs and highlight staff members who go above and beyond; at the same time, reviewers report variability by shift, with concerns about staffing levels, high turnover, and uneven training leading to differences in bedside manner and resident engagement. There are repeated references to effective family communication when staff are engaged; conversely, families have also experienced lapses in responsiveness and delays in attending to personal-care needs.

Dining and activities are generally characterized positively. Reviewers mention enjoyable meals, therapeutic diet accommodations, and regular social programming such as music and seasonal socials that contribute to resident well-being. Therapy-led rehab programs are often singled out for producing good outcomes. Common areas are commonly described as clean and inviting, though maintenance of resident rooms is inconsistent according to other accounts.

Facility condition and infection-control topics are areas of concern. Several reviews describe sanitation and odor issues in resident rooms and some common spaces, along with pest-control concerns and visible maintenance shortfalls consistent with an older building. These facility-level issues appear uneven across units, producing a mismatch between well-maintained public spaces and areas that require closer attention.

Leadership, operations, and administrative processes show variability. Families report instances of responsive management and prompt escalation when problems are raised, but there are also repeated complaints about unresponsive leadership, difficulty reaching the director of nursing or administrator, and delays in communication. Operational weaknesses noted by reviewers include billing errors or unexpected charges, interdepartmental miscommunication, and the increased use of telehealth encounters in place of in-person clinician visits. Limited on-site physician availability and gaps in clinical oversight were also mentioned as contributing to family concern.

Taken together, Sears Coastal appears to deliver substantive clinical strengthsโ€”particularly in wound care and rehabilitative therapyโ€”within an older, small-facility setting that offers a home-like environment and active programming. However, prospective residents and families should be aware of variability in staffing and cleanliness across units, administrative communication gaps, and potential billing or maintenance issues. Recommended steps before placement include an in-person tour focusing on resident rooms and cleanliness, observing staffing levels during different shifts, asking about physician visit frequency and wound-care protocols, clarifying billing policies, and inquiring about current pest-control and maintenance plans.

About Sears Coastal Nursing & Rehab

Sears Coastal Nursing & Rehab has a 100-bed capacity and offers both short-term rehabilitation and long-term care for seniors, so you'll find a place where skilled nursing, rehab services, and memory care meet under one roof. The staff provides 24-hour skilled nursing services and there's a strong focus on privacy, independence, and a home-like environment, with private rooms and suites available, each with flat screen TVs if you want some comfort during your stay. The team covers a lot of needs, from physical therapy, occupational therapy, and speech-language pathology to restorative programs for both physical and mental functions, and they partner with Integra Rehabilitation for tailored therapy plans. Special programs help folks with stroke recovery, heart conditions, orthopedic injuries, wound care, and swallowing difficulties, so care covers a wide range of diagnoses, and there's a positioning and contracture management program for those with mobility challenges. For those needing intravenous medicine, pharmacy services, or dental care, those are provided, with physician services and nutritional support as part of daily care. There's a rehab gym with state-of-the-art equipment and a model home setting, which helps residents get back on their feet and practice tasks so they feel ready to return to regular life. Free Wi-Fi and laundry services are available for everyone, there's cable TV and a beautician for regular grooming, and the facility runs daily activities along with recreation so people can stay social. Quarterly meetings include staff, residents, and families, which helps keep care plans up to date and open. Discharge planning, social services, and a continuum of care support each person's next steps, whether they're going home or staying a while. The multi-level dining program offers daily nutritional support, and the environment feels safe and supportive, with southern hospitality at its core. The facility received several awards, including for workforce excellence and quality care, and recognition from national organizations, so there's experience and effort behind the service. The staff aims to provide a good place for seniors who need extra help after a hospital stay, those who require ongoing nursing care, or anyone seeking memory care support or respite care, and the focus stays on practical, meaningful care rather than fancy extras.
