    Attentive compassionate staff safe discharge

I'm very pleased with Four Fountains - the staff were professional, warm, and truly caring. Nursing and therapy worked with us to meet goals and got my mom home safely. The facility is clean, welcoming, conveniently located, with a large dining area and posted activities. Admissions and discharge were smooth, and the administrator and D.O.N. were helpful. I have peace of mind recommending their attentive, compassionate team.

    Summary of reviews

The reviews describe a facility with a pronounced mix of strengths and serious operational weaknesses. Positively, many families and former residents praise individual caregiversโ€”particularly CNAs and some RNsโ€”who are described as compassionate and attentive. The rehabilitation and therapy department receives consistent praise for helping residents return home, often with coordinated CNAโ€“therapy staffing. Admissions and discharge coordination, along with a posted activity calendar and a generally welcoming front-line staff, are other commonly cited strengths. The facilityโ€™s location and some clean common areas are additional practical conveniences.

Care quality appears inconsistent across shifts and units. Several accounts describe delays in addressing basic personal-care needs and incontinence-care management, missed treatments, and variable monitoring of clinical conditions. These patterns point to chronic understaffing and limited clinical coverage that can produce delayed responses to changing resident needs. There are recurring concerns about medication and therapy management, and instances that indicate gaps in transfer and fall-prevention practices. A subset of families described serious adverse outcomes, including concerns following a residentโ€™s death; those accounts raise questions about clinical escalation and end-of-life coordination.

Dining and activities show mixed feedback. The therapy and rehab dining support is generally viewed favorably, but meal quality and consistency are described as unreliable at times. Activities are organized with posted schedules and engagement opportunities, which families found helpful for daily structure, though room-level amenities and individual participation varied by unit.

Facility condition and infection-prevention practices are inconsistent. Some areas and communal spaces are clean and well-kept, while others prompt sanitation and odor concerns, pest-control concerns, and general maintenance issues. These physical-plant inconsistencies underscore variability in environmental-management practices across units.

Management and communication are important themes. Families note uneven administrative responsiveness, including prolonged phone wait times, billing disputes, restrictive visitation controls, and occasional requirements for additional verification to access a resident. Concerns about staff conduct and professionalismโ€”ranging from perceived unhelpful or dismissive interactions to allegations of unauthorized access and theftโ€”are raised alongside praise for specific managers and staff who were supportive. Given the mixture of high-performing clinical teams in parts of the building and significant operational failures in others, prospective residents and families should seek specific, up-to-date information on staffing ratios, clinical oversight, incident notification procedures, sanitation and pest-control protocols, and grievance resolution processes before making placement decisions.

    About Four Fountains

Four Fountains sits on South Belt West in Belleville, IL, and stands as part of the Helia Health group, which means the care here ties into the Four Fountains brand with knowledge in nursing, assisted living, and Alzheimer's care, and the place focuses on seniors needing convalescent care, memory care, or help with recovery and daily life. Residents get a tailor-made care plan, and nurses here-who are licensed and highly trained-show up day and night, and the attention stretches from medicine to help with things like dressing, walking, and even kitchen safety if someone wants to work on those skills, and there's always an RN available for regular or urgent needs. The facility has the ARCH Active Recovery Center, which runs advanced therapy programs for physical, occupational, speech, and recreational needs, with a focus on things like stroke therapy, new joints, wound healing, and strengthening for home life, and the staff aims to help residents either recover and return home or stay safe and comfortable if Four Fountains becomes their lifelong home. Some folks use the long-term care, and others might just stay short-term after surgery or illness, but everyone gets access to memory care and attention for chronic conditions like diabetes or for severe cases such as cardiac care, amputation, or advanced respiratory issues.

The community offers both private and semiprivate rooms which let in plenty of light, feel roomy and easy to manage, stay climate controlled, and feature flat-screen HD televisions, and each room's serviced regularly with laundry done for you. Residents have access to spa bathrooms, beauty and barber salon, and a fine dining program where guests can join for a meal. There's room service, WiFi everywhere, and lots to do with books, games, DVDs, and an outdoor terrace for some air or company, and you see both planned outings and daily activities meant for socializing, learning, or exercise, plus platforms where people can connect through Facebook, Messenger, or Instagram if they'd like to keep up with loved ones or share moments. Religious services include various denominations, and there's a private space for families to gather or celebrate, with regular events and guest meals to bring everyone together when the time feels right. There's always transportation to help folks get to appointments or home visits, and a concierge lends a hand for the little things, from arranging spa visits to answering everyday questions.

Dental, vision, and podiatric care come right to the building, and a house physician looks after everyone's basic and medical needs, so people living here don't have to leave for most of their care. Four Fountains is a smoke-free and vape-free community to keep the air clean and comfortable, and you can count on the staff's long commitment to helping residents make progress, enjoy their days, and feel at home among attentive people who understand that recovery and comfort take teamwork and time.
