    Caring staff, clean facility, recommended

I was very pleased with the caring, attentive, and professional staff and excellent PT/OT - the facility was exceptionally clean, rooms and common areas spacious, and activities and food were solid. Communication and aftercare planning were helpful; our family is grateful for the recovery and support and I'd recommend this welcoming place.

    Summary of reviews

These reviews describe a facility with clear strengths in clinical rehabilitation and many aspects of daily care, but also recurring operational weaknesses that prospective residents and families should note. Positive comments frequently highlight compassionate, attentive nursing and aide teams, strong PT/OT services that produce measurable recovery progress, and proactive social-work and discharge planning including provision of homeโ€‘health equipment. Many families described the environment as clean and modern, with large rooms, spacious common areas, and an active, posted schedule of resident activities.

Care quality appears to be uneven across units and shifts. Several accounts praise skilled, patient staff and rapid recoveries, while others raise concerns about delayed assistance and lapses in routine care. The most consistent operational pattern is staffing variability: periods of understaffing are linked to long wait times for assistance and to reduced followโ€‘through on care tasks. Reviewers also described inconsistencies in medication administration timing and in the execution and documentation of rehabilitation care plans and followโ€‘up, which suggests gaps in protocols or in shiftโ€‘toโ€‘shift handoffs.

Dining and activities are generally viewed positively, with engaging programming and satisfactory meals for many residents, but food quality is described as inconsistent by some families. The facilityโ€™s physical environment receives mixed but largely favorable remarks: many reviewers note a clean, stateโ€‘ofโ€‘theโ€‘art setting with roomy accommodations and welcoming common spaces. At the same time, there are sanitation and incontinenceโ€‘care concerns reported in specific areas, along with comments about limited privacy in some shared bathrooms and commonโ€‘area configurations.

Management and communication draw mixed evaluations. Some families say administration is responsive and that case managers and social workers provide clear updates and effective aftercare plans. Others report unclear admission information, miscommunication about care expectations, and concerns regarding staff competence or conduct on particular shifts. There are also isolated but serious clinical flags โ€” for example, concerns about skinโ€‘integrity management and nutrition monitoring โ€” that prospective families should investigate directly.

Overall, the facility demonstrates notable strengths in rehabilitation services and in teams that provide compassionate care; however, variability in staffing, careโ€‘coordination, sanitation practices, and medication/therapy followโ€‘through create uneven experiences. Prospective residents and decision makers would be well served to ask targeted questions at tour and admission: typical staffing levels by shift, woundโ€‘care and nutrition monitoring protocols, medicationโ€‘administration safeguards, how therapy plans are created and followed up, and how admission/affiliation logistics are handled. Observing a mealtime and an activity, and requesting recent qualityโ€‘audit or incidentโ€‘response information, can help families assess whether the facilityโ€™s strong elements will be reliably available for a specific resident.

    About Memorial Care Center

Memorial Care Center is a skilled nursing and rehabilitation facility in Belleville, Illinois, that sits on the campus of Memorial Hospital, so help is always close by if someone needs it right away, and the place has been around since 1963, so they know a thing or two about helping people bounce back and get back on their feet. The center offers care for seniors who need short-term rehab or longer stays, and the staff has training in lots of medical specialties, including speech therapy, occupational therapy, physical therapy, nutritional counseling, and even respiratory care, so there's a whole team who knows how to address pretty much any health concern, and the staff includes registered nurses around the clock, plus a group of Care Givers, RNs, Med-Techs, LVNs, and physicians, which means there's always someone nearby when needed and they keep a staff-to-patient ratio of 1:10, which really helps residents get more attention.

They aim to make recovery feel comfortable, and the rooms have new furniture, a TV, and common areas are nicely set up, while the beauty shop, salon spa, and massage services help folks feel a bit more at home, and there are outdoor walking trails and gathering spots for visiting because socializing and getting outside makes a difference. The large modern Therapy Gym gives patients space and equipment for focused rehab, and there's even an activities calendar with games, music, movies, crafts, pet visits, and daily events, so boredom doesn't really have a chance. People living there can have visits from family and enjoy social hours, bingo, and more, plus there are spiritual and pastoral support services, including Sunday school, Catholic Mass, and Bible study, if that's important to someone, and both resident and family councils help everyone share feedback.

Memorial Care Center pays attention to people's individual needs with programs like "Helpful Healing" short-term rehab, and they offer both inpatient and outpatient rehab services. Seniors can get help with eating, dressing, or learning daily skills again through occupational therapy, and the speech-language pathologists help with everything from swallowing to communication issues. There's even kinetic consulting, stroke education, and support with rejoining the community. The kitchen has a chef-inspired menu tailored for different diets, including heart-healthy, vegetarian, and gluten-free meals, and the themed food carts make mealtimes a bit more enjoyable.

Additional features include a laboratory, pharmacy, X-ray services, and telehealth appointments for those who need them, as well as preadmission and financial counseling to help sort out paperwork or any questions. The care center holds over 100 beds on one floor, and it's a non-profit organization, so they're focused on care more than profit. The place has received high grades, with a Five-Star rating from CMS and an A+ in inspections, and U.S. News & World Report has named it one of America's Best Nursing Homes for twelve years running, showing some consistency over time. Occupancy holds around 69.5%, so space isn't too tight, and the environment is set up to feel homelike and welcoming, with the right supports in place to help each resident on their path to recovery or comfortable daily living.
