    Caring clean responsive rehab community

I placed my mother here and I'm very pleased - the nurses, CNAs, therapists and administration are attentive, caring and always available, and PT/rehab helped her regain mobility. The facility is clean and secure (photo check-in), with easy parking, a pleasant courtyard and rooms with outside views. Management is responsive, remodeling is underway, and activities (Jobany runs engaging programs) keep residents social. Overall a warm, professional community where my mother is happy.

    Summary of reviews

Overall impression: Crescent Care is characterized by a strong core of clinical and therapy staff and an active activity program, combined with recent facility updates and some well-maintained outdoor areas. Many families praised the facility for compassionate nursing care, effective physical and occupational therapy that supported measurable recovery, and a responsive front-desk and administrative team. The community is described as personable and small, with helpful housekeeping and laundry services and secure entry controls that families find reassuring.

Care and staff: Reviewers commonly highlight caring, professional nursing staff, attentive CNAs, and therapists who produce positive rehabilitation outcomes. Communication with families is frequently described as regular and responsive, including formal updates and meeting reports. At the same time, there is a persistent pattern of inconsistent staffing and variable staff performance between shifts; night and weekend coverage is specifically identified as weaker. These staffing inconsistencies are tied to operational concerns such as inconsistent personal-care practices and delays in attending to resident needs during off shifts.

Dining and daily living: Comments about dining are mixed; food quality is generally described as adequate. Housekeeping and laundry work receive positive mentions for cleanliness of clothing and linens in many cases. However, reviewers also describe variability in room and unit cleanliness, with certain floors or units noted as less well maintained. There are recurring incontinence-care concerns and associated odor issues in some rooms and common areas, which reviewers link to inconsistent personal-care routines and occasional lapses in room maintenance.

Activities and social life: The facility's activity program is a clear strength for many residents. An engaged Activity Director and a schedule of social, recreational, and educational programs drew repeated praise. That said, a number of reviewers felt activity availability was uneven and that family/social spaces were limited, contributing to an institutional or hospital-like feel in some resident rooms and units.

Facilities and infection control: Crescent Care has undergone visible cosmetic improvements, including new paint and flooring in parts of the facility, and outdoor spaces such as a courtyard are well kept. Yet several reviews raise concerns about ventilation and infection-control practices, and some describe conditions that may facilitate spread of illness. Cleanliness appears to vary by unit, suggesting operational inconsistencies in environmental services and infection-prevention protocols.

Management and patterns: Reviewers note management and staff turnover and referenced an ongoing period of transition; some families perceived cost-containment pressures. Many reviewers praised recent management initiatives such as remodeling and security improvements, while others described uneven implementation of standards across shifts and units. Language-access limitations and variable attention to residents who do not speak English were also noted.

Summary assessment and guidance: Crescent Care offers strong rehabilitation services, a compassionate core clinical team, active programming, and tangible facility improvements, making it a reasonable option for residents seeking therapy-focused care and a small-community environment. Prospective residents and families should, however, probe weekend and night staffing patterns, incontinence-care and linen-change protocols, infection-control and ventilation measures, activity schedules, and language-access services. A focused tour of different units at various times of day (including weekends/evenings) and direct questions about staffing consistency and environmental procedures will help families assess whether the facility’s operational patterns align with their needs.

Deficiencies have appeared in recent inspection reports, including six infection-related issues and four overall deficiencies, such as not always giving help with daily living tasks, not always keeping the environment as clean and safe as it should be, and not always providing enough nursing staff on each shift. Reviews talk about patient and family satisfaction with care, but there have also been comments about concerns with cleanliness, laundry, medication being delivered on time, and periods when staff seem stretched thin. The look and feel of Crescent Care are often described as modern and attractive to visitors. Crescent Care opens its doors for tours, and prospective residents can come for lunch to see the place for themselves.
