    Kind attentive staff, excellent therapy

I was very pleased with the care my mother-in-law received - kind, attentive staff who knew our family, excellent PT/OT/speech therapy, and clear communication. The facility is spotless, homey and beautifully decorated for Christmas, set in scenic rural surroundings with a gazebo; meals were delicious with daily choices. Transfers and the hospital liaison were prompt and paperwork seamless, and there's ample seating and activities for visits. Overall a comfortable, well-run place I'd recommend and would consider returning to after a hospital stay.

    Summary of reviews

Reviewers describe a facility with several clear strengths, particularly in rehabilitation, cleanliness, and resident-centered interaction. Physical, occupational, and speech therapy services receive frequent praise for effective outcomes and attentive therapy staff. Many families highlight prompt hospital liaison work and seamless transfer paperwork, along with Medicaid admissions support and helpful billing/administrative assistance. The community is often characterized as home-like and well decorated seasonally, with ample communal seating for games and activities, accessible outdoor spaces, and modern room amenities in many units.

Care and staffing are a central positive theme: numerous accounts describe compassionate, patient nursing staff and aides, consistent communication with powers of attorney or family, and long-tenured staff who build trusting relationships. These relational strengths appear linked to positive family experiences and perceptions of safety and dignity in intimate care. At the same time, other reviewers raised substantive concerns about staff conduct and responsiveness. These critiques include reports of rude interactions, unanswered call lights, and supervision lapses during mealtimes and personal-care situations, which point to variability in frontline staffing and responsiveness.

Dining and activities are frequently cited as positives. Many reviews note varied, palatable meals with choices, fresh produce, three daily meals, and an active activities program that includes puzzles, games, music events, and holiday programming. The activities director is described as engaging, and common areas are furnished to support social engagement. However, dining quality is reported inconsistently across stays, so meal experience may vary by unit or by day.

Facility condition and housekeeping receive mixed but generally favorable comments. Several reviewers describe spotless common areas, modern rooms with large bathrooms and entertainment options, and pleasant rural or garden-facing views. Conversely, a number of accounts identify inconsistent housekeeping and sanitation practices in certain areas, and some reviewers noted older sections of the building where amenities feel less modern.

Operational and management patterns show both competence and areas for improvement. Positive operational notes include efficient hospital-transfer coordination, COVID-19 prevention and record-keeping, and helpful admission support. Conversely, reviewers described uneven leadership responsiveness to complaints, gaps in documentation and transparency around clinical incidents, inconsistencies in admission processes and bed availability, and challenges coordinating complex medical decisions across providers. There are also concerns about property security and missing belongings that suggest a need for stronger property-handling controls.

In summary, Farmington Village Nursing and Rehabilitation Center demonstrates notable strengths in therapy services, a generally clean and comfortable environment, engaging activities, and family-focused communication. These strengths coexist with recurring operational concerns—most prominently variability in staff conduct and responsiveness, inconsistent housekeeping and dining experience, and management follow-through on complaints and complex clinical coordination. Prospective residents and families would be advised to tour the facility, ask about current staffing ratios and supervision practices, review housekeeping and property-security procedures, and verify how clinical decisions and incident documentation are handled during transitions of care.

    About Farmington Village Rehab

Farmington Village Nursing and Rehabilitation Center, formerly called Farmington Country Manor Nursing and Rehabilitation Center, sits in Farmington, Illinois, and offers care for seniors needing both short-term and long-term help, whether it's skilled nursing or rehabilitation, and you'll find they have a total of 92 beds in this center, which accepts people for everything from everyday assisted living, memory care for Alzheimer's and dementia, to wound care, veterans services, and programs aimed at end-of-life support with hospice. The staff provides physical, occupational, and speech therapy and the therapy rooms, social spaces, and Fox Wellness programs are a part of their daily efforts, and there's even outpatient therapy with its own entrance at the northwest corner. People who stay here may get help with dressing, eating, medication, or mobility, and the care team works day and night, but it's important to know that the Center for Medicare and Medicaid Services has flagged this community for very serious quality and safety problems, including the lowest ratings in nearly every category and a real history of harm coming to residents during inspections, sometimes causing injury, impairment, or worse, and the center appears on the Nursing Home Watchlist for those reasons. The place is set up so residents can join activities based on their interests, get social services, and live in an environment that tries to be warm and calm, but facts show staffing often falls below what's considered safe or needed to prevent harm. It's got a connection with FOX Rehabilitation Services, runs clinical rotations for students, and offers insurance billing, while their rehabilitation services go every day, and they have specialized care goals for each person they admit, but families thinking about this place ought to pay close attention to the track record, especially when safety and quality of life matter most.
