    Resident praises warm attentive care

I'm a resident at this small, home-like facility and I'm very pleased with the care. The staff - from Administrator Tara to long-time CNAs Brenda, Eva, April and Jennifer - are loving, friendly, and know everyone by name. The home is very clean, recent brightening and updates made it cheerful, medical care is excellent, and concerns are addressed promptly. I feel treated like family and safe in this warm, attentive place.

    Summary of reviews

The reviews for Axiom Healthcare of Harrisburg are highly mixed, with clear positive themes around the facility’s staff and small scale alongside significant operational concerns. Many reviewers praise the direct-care team—particularly CNAs—and describe warm, family-like relationships in a small, home-style setting. Recent cosmetic work and brighter common areas have been noted as improvements, and several families expressed satisfaction with clinical care and the facility’s emphasis on resident safety. The arrival of a new administrator elicited positive impressions from some reviewers, and some accounts indicate that staff address concerns promptly when raised.

Despite those strengths, a number of reviews identify recurring operational weaknesses. Sanitation and supply-management practices are inconsistent: reviewers described lapses in restroom and room cleanliness and shortages of basic supplies. Infrastructure maintenance issues have also been raised, including interruptions to water or other utilities, which affected resident comfort and access to restroom facilities. These kinds of problems point to gaps in environmental services and contingency planning.

Staffing and conduct are another area of divergence. While many families praised compassionate and dedicated CNAs, other accounts describe variable staff responsiveness, delays in attending to needs, and concerns about the tone of staff–family interactions. Reviewers also described uneven quality across shifts, suggesting that care consistency may depend on staffing patterns and individual caregivers. There are also allegations of resident property theft and instances where residents missed regular bathing or had limited access to scheduled personal-care services, indicating weaknesses in personal-item safeguards and personal-care scheduling.

Dining and daily living services appear inconsistent. Several reviewers described limited meal variety and repetitive menus, while others noted acceptable or enjoyable options. Activity programming was not widely discussed in the comments provided; the small, familiar atmosphere may offer informal engagement but prospective families should ask about structured activities and social programming during visits.

In summary, Axiom Healthcare of Harrisburg presents a mixed picture: strong points include a small, personable environment, several committed long-term CNAs, recent aesthetic improvements, and generally competent clinical care. Countervailing concerns center on sanitation consistency, infrastructure and supply reliability, variable staff responsiveness, gaps in personal-care scheduling, and serious allegations about personal-item security. Prospective residents and families should tour the facility, speak directly with the administrator about staffing patterns, sanitation protocols, supply-contingency plans, and personal-item policies, and request documentation of corrective actions for any known deficiencies before making placement decisions.

    About Axiom Healthcare of Harrisburg

Axiom Healthcare of Harrisburg sits in a calm spot and has a homelike feel, where folks get help with daily needs like bathing, dressing, medication, and help getting around, and you'll find people with memory care needs and people needing advanced care here, too, and there's staff on duty 24 hours with nursing support for a good chunk of the day, plus a 24-hour call system for safety. The place offers private and shared rooms, all with private bathrooms, and you can get furnished rooms, cable TV, a telephone, a kitchenette, and Wi-Fi, so residents can stay connected and keep some comforts of home, and people can pick from all-day dining and restaurant-style meals planned by a chef, with allergy-sensitive choices, and there's a focus on keeping things social, with movie nights, group activities, music programs, library time, exercise programs, and arts and crafts, and outdoor spaces like walking paths and garden areas mean you can get a bit of fresh air or join in outdoor programs. You'll see both staff-run and resident-run activities, and there's transportation and parking for those who need to head to doctor appointments or other outings, and the beauty shop, laundry, housekeeping, and even dry cleaning help out with everyday chores.

Services go beyond the basics, with skilled nursing for those recovering from illness or needing long-term care, and physical, occupational, and speech therapies to help folks get stronger or adjust after an injury, plus wound care, rehabilitation, on-site X-rays for quick check-ups, and respiratory and behavioral health programs for those who need regular care or extra support, and the staff works with each resident's family on planning and support. Axiom Healthcare of Harrisburg takes Medicare and Medicaid, has move-in help, a family and resident council system, and has both a fitness room and a movie theater. The place has 68 certified beds but usually has about 28 residents per day, and you'll find both community and individual activities happening all week, and a regular nursing team reports about three nurse hours per resident per day. Ownership changed recently and the community is affiliated with Petersen Health Care, and though the staff wants to keep everyone safe and well, reports show there have been some deficiencies, like not always having a registered nurse for eight hours a day, not enough pressure ulcer care, and some issues with room sizes and infection, so families might want to ask about these when visiting or reviewing the latest inspection reports. Everyday life is kept as comfortable as possible with a friendly staff who try to bring a steady, cheerful mood to the halls, and residents can expect both skilled nursing care and chances to have fun and stay active as long as possible.
