    Compassionate, attentive care; excellent therapy

I had a very positive experience at Cedar Ridge. Therapy was excellent and staff across departments were compassionate, attentive and responsive - Vicki, Kenya, Tiffany and Nurse Kia stood out, kept pain controlled and preserved my mother's dignity while giving clear updates. The facility was clean, friendly, and offered activities and communication that gave us peace of mind. I'm grateful for the hardworking team; with a few small improvements this place would be outstanding.

    Summary of reviews

Cedar Ridge Health & Rehab Center presents a mixed profile. Many families and residents praise the facility for compassionate front-line staff, a strong rehab/therapy program, and engaging social programming. Several accounts highlight effective physical and occupational therapy, helpful admissions support, technology that facilitates virtual family visits, holiday events, and staff who go beyond basic duties to support residents' dignity and comfort. These strengths contribute to positive experiences described as home-like, social, and rehabilitative.

At the same time, reviews reveal recurring operational concerns that merit careful consideration. Staffing levels — particularly overnight — and responsiveness to call lights and toileting needs are frequently described as inconsistent, contributing to delays in assistance and monitoring. Reviewers also raise concerns about medication administration and clinical follow-up, including missed doses or delayed testing, which families link to adverse outcomes in some cases. A small number of accounts describe serious clinical events; these are serious enough to warrant direct inquiry during any placement conversation.

Dining and activities show a split: many families commend home-style meals, dietary accommodation, and active programming (outdoor visits, video calls, seasonal parties), while other families report inconsistent meal temperature, limited menu choices, and concerns about value. Activities staff and recreational offerings are generally noted as a strong point, with frequent references to organized events and staff efforts to maintain resident engagement during restricted visitation periods.

Facility condition and operations are similarly mixed. Several reviewers describe clean, modern, well-kept common areas and pleasant grounds, while others note maintenance shortcomings (worn furnishings, accessibility issues, HVAC or sidewalk concerns) and sanitation or pest-control problems in certain units. Room layout and capacity are recurring practical issues — many rooms are shared and bathrooms may be communal, which affects privacy and convenience for some residents.

Communication and management practices are another area of variability. Positive examples include proactive therapy teams and named staff who facilitated transitions, but other families describe weak interdepartmental coordination, billing or accounting errors, unresolved lost personal items, and limited responsiveness from administration. These operational gaps are often cited alongside staffing pressures and documentation issues.

Overall, Cedar Ridge shows notable strengths in therapy, activities, and many individual staff relationships, but also consistent patterns of operational weakness that affect safety, clinical reliability, and family communication. Prospective residents and families should balance the facility's rehabilitative and social programming against the reported variability in staffing, clinical processes, maintenance, and billing practices. Recommended next steps for decision-making include an in-person tour during multiple shifts (including evening/night), direct questions about staffing ratios and medication-administration protocols, review of recent inspection and staffing reports, and clear communication about private-room availability and billing practices before placement.

About Cedar Ridge Health & Rehab Center

Cedar Ridge Health & Rehab Center sits at 1 Perryman St in Lebanon, IL, and is a Skilled Nursing Facility with 116 certified beds for both short- and long-term stays, so families looking at somewhere with around-the-clock care and lots of therapy and rehab options find plenty here, though you'll want to know they've had 37 total deficiencies cited in recent inspection reports, including six related to infection control and some concerns about helping residents with everyday tasks and safe pain management. You'll find a staff that speaks English, and skilled nurses and therapy teams handle different services like physical, occupational, and speech therapy along with pharmacy, radiology, optometry, podiatry, and other medical support, so residents dealing with things like fractures, joint replacements, memory problems, diabetes, or stroke get help with recovery using state-of-the-art technology and specialized programs in things like Parkinson's rehabilitation, cardiac and orthopedic recovery, and stroke care.

While the center offers 24/7 nursing care, meals designed to meet nutritional needs, and both private and semi-private rooms in a single-story building that aims for a home-like feeling, the nurse turnover rate stands at 48.9%, which is above the state average, and average nurse time per resident each day is 3.07 hours, a bit lower than the state's 3.4 hours. Cedar Ridge has respite care for caregivers who need a break, outpatient therapy, an always available menu, individualized diets, regular housekeeping, laundry, pet therapy, gardening, religious programs, and community outings, so residents can get involved if they want, plus the center's got a tradition of being a part of the local community and says it values personal connections and "genuine hometown care."

The for-profit facility is individually owned and affiliated with Crest Healthcare Consulting, with Medicare and Medicaid certification and long-term care insurance options. It's been certified as a great place to work, though right now it isn't taking new patients, and there's no extra information on languages spoken or office hours. Cedar Ridge Health & Rehab Center takes a broad approach to care, aiming to help people recover or manage conditions while staying as independent and comfortable as possible, but it's clear families will want to look over inspection results and consider the reported deficiencies before making any decisions.
